Diarrhea and typhoid fever pose significant public health challenges, particularly in low- and middle-income countries where vulnerable populations such as children, the elderly, and immunocompromised individuals are disproportionately affected. This review explores key risk factors that drive the spread of these diseases, including socioeconomic influences, malnutrition, and environmental challenges. Poverty, lack of access to clean water, inadequate sanitation, and overcrowded living conditions creates fertile grounds for the proliferation of waterborne diseases. Malnutrition further weakens immune systems, making at-risk populations more susceptible to infections and their complications. This review also examines how educational attainment and health awareness impact disease prevention, with a focus on vulnerable populations. Current interventions such as water, sanitation, and hygiene (WASH) programs, nutritional support, and vaccination campaigns are assessed, along with policy recommendations to address root causes. Future directions emphasize integrating health interventions with broader socioeconomic development strategies to sustainably reduce the disease burden among at-risk groups.

3Understanding The Mental Health Of Indonesian Academics In Higher Education: An Examination Of Risk And Protective Factors - A National Survey Study

University lecturers hold a strategic role in shaping the quality of higher education. Beyond their responsibility to transfer knowledge to students, lecturers contribute substantially to the advancement of science through research, academic publications, and community engagement. The quality of higher education is therefore strongly dependent on the professional and personal well-being of academics. However, over the past decade, numerous studies have shown a significant increase in mental health problems among academics, surpassing those in many other professions. Disorders such as depression, anxiety, and burnout have become increasingly prevalent, leading to reduced productivity and diminished work quality among lecturers. This study aims to map the mental health conditions of university academics in Indonesia using an explanatory mixed-methods approach. The quantitative phase will employ a survey to measure mental health conditions (stress, anxiety, and depression), risk factors (workload and publication pressures), and protective factors (psychological safety in the workplace and coping strategies). In the qualitative phase, the study will explore lecturers’ perceptions of personal conditions, institutional contexts, and higher education systems at the national and global levels that are perceived to influence their mental health. The quantitative phase will involve 1,500 lecturer respondents across Indonesia, recruited through quota sampling that takes into account geographic location, type of higher education institution, and gender. From this pool, up to 5% of respondents will be invited to participate in the qualitative phase. Quantitative data will be analyzed using Structural Equation Modelling (SEM), while qualitative data will be examined using Critical Discourse Analysis (CDA).

This article is from Translational Psychiatry , volume 4 . Abstract We have used a translational Convergent Functional Genomics (CFG) approach to discover genes involved in alcoholism, by gene-level integration of genome-wide association study (GWAS) data from a German alcohol dependence cohort with other genetic and gene expression data, from human and animal model studies, similar to our previous work in bipolar disorder and schizophrenia. A panel of all the nominally significant P-value SNPs in the top candidate genes discovered by CFG (n=135 genes, 713 SNPs) was used to generate a genetic risk prediction score (GRPS), which showed a trend towards significance (P=0.053) in separating alcohol dependent individuals from controls in an independent German test cohort. We then validated and prioritized our top findings from this discovery work, and subsequently tested them in three independent cohorts, from two continents. In order to validate and prioritize the key genes that drive behavior without some of the pleiotropic environmental confounds present in humans, we used a stress-reactive animal model of alcoholism developed by our group, the D-box binding protein (DBP) knockout mouse, consistent with the surfeit of stress theory of addiction proposed by Koob and colleagues. A much smaller panel (n=11 genes, 66 SNPs) of the top CFG-discovered genes for alcoholism, cross-validated and prioritized by this stress-reactive animal model showed better predictive ability in the independent German test cohort (P=0.041). The top CFG scoring gene for alcoholism from the initial discovery step, synuclein alpha (SNCA) remained the top gene after the stress-reactive animal model cross-validation. We also tested this small panel of genes in two other independent test cohorts from the United States, one with alcohol dependence (P=0.00012) and one with alcohol abuse (a less severe form of alcoholism; P=0.0094). SNCA by itself was able to separate alcoholics from controls in the alcohol-dependent cohort (P=0.000013) and the alcohol abuse cohort (P=0.023). So did eight other genes from the panel of 11 genes taken individually, albeit to a lesser extent and/or less broadly across cohorts. SNCA, GRM3 and MBP survived strict Bonferroni correction for multiple comparisons. Taken together, these results suggest that our stress-reactive DBP animal model helped to validate and prioritize from the CFG-discovered genes some of the key behaviorally relevant genes for alcoholism. These genes fall into a series of biological pathways involved in signal transduction, transmission of nerve impulse (including myelination) and cocaine addiction. Overall, our work provides leads towards a better understanding of illness, diagnostics and therapeutics, including treatment with omega-3 fatty acids. We also examined the overlap between the top candidate genes for alcoholism from this work and the top candidate genes for bipolar disorder, schizophrenia, anxiety from previous CFG analyses conducted by us, as well as cross-tested genetic risk predictions. This revealed the significant genetic overlap with other major psychiatric disorder domains, providing a basis for comorbidity and dual diagnosis, and placing alcohol use in the broader context of modulating the mental landscape.

11Understanding Constipation In Stroke Patients With Dysphagia: Incidence, Risk Factors, And Management Strategies - A Scoping Review

Background: Constipation and dysphagia are common complications among stroke survivors, impacting quality of life, recovery, and rehabilitation outcomes. Dysphagia can lead to decreased fluid and fibre intake, which, combined with reduced mobility, increases the risk of constipation. While each condition has been studied independently, there is limited research focusing on their co-occurrence and management in post-stroke populations. Objective: This scoping review aims to examine the incidence, risk factors, and management strategies for constipation in stroke patients with dysphagia, synthesizing current knowledge to inform clinical practice and future research. Design: Using the Arksey and O’Malley framework and PRISMA-ScR guidelines, we conducted a comprehensive search of MEDLINE, EMBASE, and CINAHL databases for peer-reviewed studies published between 2003 and August 2024. Studies were eligible if they included adult stroke patients with both constipation and dysphagia and reported on incidence, risk factors, or management strategies

This report attempts to demonstrate that dropouts follow a path inverse to the one that students follow to academic excellence by presenting a review of literature pertaining to dropouts and students at risk. It examines the process by which at-risk students are separated from academic excellence, and attempts to understand the factors behind academic excellence and student achievement within the context of findings relating to at-risk students and dropouts. The following subjects are addressed in the literature review: (1) the characteristics of at-risk students; (2) the causes of the dropout rate, both internal and external to school; (3) effective methods of dropout prevention and support programs for at-risk students; and (4) academic excellence based on findings about at-risk students. The results from each section of the review are discussed. The complete review indicates that programs designed to support at-risk students and programs that effectively encourage academic excellence are strikingly similar in most respects. In addition, the process through which at-risk students drop out corresponds to the process through which students move towards academic excellence. Further research, particularly with ethnographic methodology, on the parallels and differences between dropouts and high achievers is recommended. Six figures and a list of 70 references are included. (JS)

Prostate cancer is the most commonly diagnosed cancer in men. Among African American men, the incidence of prostate cancer is approximately 60 higher and the mortality rate in this population is 2 to 3 times greater compared with European American men. The reasons for this disparity are not completely understood. Current tools in hand to study these differences, such as genetically altered mouse models, are useful for dissecting roles of specific genes and signaling pathways in intact animal, but have limited utility for understanding differences in disease susceptibility in humans. The overall objective of this application is to model prostate epithelial cells to understand the molecular basis for the disparities in prostate cancer risk between white Caucasian and black African-American men. The specific aims are: 1) to establish conditions that promote differentiation of human neonatal foreskin skin fibroblast-derived iPSC into cells with characteristics of prostate epithelium, 2) identify differences in gene expression and epigenetic signatures between prostate epithelial cells derived from iPSC of Caucasian and African-American foreskin fibroblasts and 3) compare and establish methods to transform differentiated prostate epithelial cells to identify differences in susceptibility to transformation.

Since the formation of the Islamic State of Iraq and the Levant (ISIL) and its so-called Caliphate, the terrorist organization has demonstrated its capability and willingness to project force beyond its immediate area of operations in the Middle East, extending to Western countries. Rather than solely dispatching trained foreign fighters, in the United States, ISILs strategy has involved homegrown violent extremists (HVEs) with a limited range of connectivity to the group. This thesis explores the threat posed by a subgroup of HVEs identified as redirected radicals, aspiring foreign fighters who, when prevented by counterterrorism actions from traveling overseas, decided instead to alter their targeting trajectory and commit violence in their home countries.

This article is from Harm Reduction Journal , volume 9 . Abstract Background: In India, as in rest of the world, HIV prevention programs have focused on HIV transmission through unsafe injecting practices with less attention on sexual risk behaviors among injecting drug users (IDUs). This study examines the sexual risk taking behaviors of IDUs associated with their pattern of drug use in India. Methods: Data were obtained from the behavioral tracking survey conducted in 2009 among 1712 IDUs in two districts each of Manipur and Nagaland states in Northeastern part of India. Sexual risk behaviors among IDUs were assessed in terms of multiple sex partners, sex with a paid female partner in the last 12 months and inconsistent condom use with any female partner. Results: More than one-fourth (27%) in Manipur and almost one in two (47%) IDUs reported having had sex with two or more female partners in the past 12 months. In Manipur where heroin is commonly used, the odds of having multiple sex partners were higher among non-heroin users than heroin users (42% vs. 23%, Adjusted Odds Ratio (AOR): 1.7, 95% Confidence Interval (CI): 1.1-2.6) and who shared needles/syringes in the last one month than who did not share (46% vs. 26%, AOR: 2.2, CI: 1.2-4.0). In Nagaland, where Spasmoproxyvon (SP, a synthetic opioid analgesic that contains dextropropoxyphene, dicyclomine hydrochloride and paracetamol) is most common, regular injectors as compared to occasional injectors were more likely to report multiple sex partners (67% vs. 42%, AOR: 2.7, CI: 1.8-4.1) and sex with paid partners (13% vs. 3%, AOR: 6.0, CI: 3.0-12.1). Sharing of needles/syringes was positively associated with multiple sex partners (51% vs. 44%, AOR: 1.6, CI: 1.2-2.2), and inconsistent condom use (93% vs. 80%, AOR: 3.0, CI: 1.8-5.1). Conclusions: IDUs with unsafe injecting practices also engage in risky sexual practices magnifying the risk of HIV infection. There is a need to focus on prevention of sexual transmission among high-risk IDUs, particularly in areas where Spasmoproxyvon is commonly used.

Prostate cancer is the most commonly diagnosed cancer in men. Among African American men, the incidence of prostate cancer is approximately 60% higher and the mortality rate in this population is 2 to 3 times greater compared with European American men. The reasons for this disparity are not completely understood. Current tools in hand to study these differences, such as genetically altered mouse models, are useful for dissecting roles of specific genes and signaling pathways in intact animal, but have limited utility for understanding differences in disease susceptibility in humans.The overall objective of this application is to model prostate epithelial cells to understand the molecular basis for the disparities in prostate cancer risk between white Caucasian and black African-American men. The specific aims are: 1) to establish conditions that promote differentiation of human neonatal foreskin skin fibroblast-derived iPSC into cells with characteristics of prostate epithelium, 2) identify differences in gene expression and epigenetic signatures between prostate epithelial cells derived from iPSC of Caucasian and African-American foreskin fibroblasts and 3) compare and establish methods to transform differentiated prostate epithelial cells to identify differences in susceptibility to transformation.

The purpose of the study was to determine whether individual differences in at-risk 4th graders' language comprehension, nonverbal reasoning, concept formation, working memory, and use of decimal labels (i.e., place value, point, incorrect place value, incorrect fraction, or whole number) are related to their decimal magnitude understanding. Students (n = 127) completed 6 cognitive assessments, a decimal labeling assessment, and 3 measures of decimal magnitude understanding (i.e., comparing decimals to the fraction benchmark task, estimating where decimals belong on a 0-1 number line, and identifying fraction and decimal equivalencies). Each of the domain-general cognitive abilities predicted students' decimal magnitude understanding. Using place value labels was positively correlated with students' decimal magnitude understanding, whereas using whole-number labels was negatively correlated with students' decimal magnitude understanding. Language comprehension, nonverbal reasoning, and concept formation were positively correlated with students' use of place value labels. By contrast, language comprehension and nonverbal reasoning were negatively correlated with students' use of whole number labels. Implications for the development of decimal magnitude understanding and design of effective instruction for at-risk students are discussed. Understanding human development is perhaps more relevant today than at any other moment in history. Although the United States is very good at using destructive power against its adversaries, preventing conflict and reaching a desirable end state often eludes it. A better understanding of human behavior in general, and human development specifically, may help it do both. If capability and will together are given as the ultimate, necessary and sufficient cause of violent conflict, then understanding human motivation and behavior is essential. Human behavior is the outcome of individual motivation and interaction with other people, institutions, and the environment. The conditions that these elements constitute significantly affect the ability of individuals to live, grow, develop and live the life that the individual values. Human development is a sustainable increase in individual capability to meet one's needs when conditions do not permit this; individual frustration may lead to violent behavior. This paper explores the lack of human development as the cause of intrastate conflict. The research includes an analysis of human needs and the means to satisfy them at the individual and national levels. Risk assessment models for case studies in Latin America provide for a richer understanding of human development and its link to violent conflict. Furthermore, the risk assessments depict trends in human development and risks of intrastate conflict. Ultimately, the model is both a framework for investigation as well as a practical tool for strategic warning and nation building.

32Understanding Golfers’ Acceptance Behavior Toward Robotic Golf Caddies By Merging The Task Technology Fits Theory And The Perceived Risk Theory

The current paper was designed to understand how to form the acceptance behavior of golfers toward robotic golf caddies, which conducted a hypothetico-deductive approach. The study focused on two questions: i) Can the TTF theory explain the acceptance behavior of golfers toward robotic golf caddies? ii) Do perceived risks negatively affect the acceptance behavior of golfers toward robotic golf caddies? Thus, the study postulated the impacts of task/technology characteristics and the five perceived risks (i.e. financial, time, privacy, performance, and psychological) on task technology fit, and the link between task technology fit and behavioral intentions. The data was collected from 387 golfers in South Korea, and the hypotheses tests were conducted by structural equation modeling. The results of the data analysis indicate that both task and technology characteristics increase task technology fit, and the four dimensions of perceived risks, which include time, privacy, performance, and psychology, have a negative influence on task technology fit. In addition, task technology fit also increases behavioral intentions. The study provides theoretical contributions by filling the acknowledged research gaps, and it also presents managerial implications in regard to commercializing robotic caddies in the golf industry.

35Understanding Caregivers’ Experiences Supporting Loved Ones At Clinical High Risk For Psychosis

Caregivers of individuals at clinical high risk (CHR) for a first episode of psychosis (FEP) experience high levels of distress and isolation in their daily lives as they navigate the diagnosis and treatment process with their loved one (McCann et al., 2011; Napa et al., 2017; Oluwoye et al., 2019; Petrakis et al., 2012). Extant literature has examined caregivers’ concerns along this journey, modes of support caregivers provide for their loved ones, and challenge caregivers experience when providing support (Franz et al., 2010; Lavis et al., 2015; Luckstead et al., 2016; Mui et al., 2019; Napa et al., 2017; Petrakis et al., 2012; Skubby et al., 2015). However, researchers have yet to investigate how intersections of sociocultural location (e.g., one’s race, gender identity, socioeconomic class) and oppression may influence caregivers’ subjective accounts of their emotional, relational, and support-related experiences of supporting their loved ones at CHR for a FEP. To this end, this study will employ semi-structured interviewing and qualitative content analysis through a relational-cultural and critical feminist lens to describe (1) caregivers’ emotional experiences of supporting their loved ones at CHR for a FEP, (2) caregivers’ experiences of receiving support from their social support networks, (3) ways in which caregivers’ sociocultural locations influence their caregiving experiences, and (4) caregivers’ recommendations for caregiver support groups. Findings from this community-based participatory research (CBPR) partnership between a research institution and a local community clinic will elucidate the often neglected emotional, relational, and sociocultural needs of caregivers of individuals at CHR for a FEP in order to inform the development of a caregiver support group at a community clinic.

This article is from Social Science & Medicine (1982) , volume 102 . Abstract Some types of intravaginal practices (IVP) may increase the risk for HIV acquisition. This is particularly worrisome for populations with dual high prevalence of HIV and IVP. Women involved in transactional sex are at increased risk for HIV infection in sub-Saharan Africa. Social, cultural and economic influences are strong drivers of IVP in this population. To explore this, we carried out a qualitative research study to investigate the drivers and motivations for using IVP within a large observational study of women at high risk of HIV in Tanzania and Uganda from September 2008 to September 2009. Of the 201 women selected, 176 women took part in a semi-structured in-depth interview. Additionally, in Tanzania, eight focus group discussions among study participants and community members were carried out to obtain information on community norms and expectations. IVP were motivated by overlapping concerns with hygiene, morality, sexual pleasure, fertility, relationship security, and economic security. These motives were driven by the need to meet cultural and social expectations of womanhood, and at the same time attend to personal well-being. Among women involved in transactional sex in East Africa, interventions aimed at modifying or eliminating IVP should attend to local cultural and social norms as well as the individual as an agent of change.

A recent systematic review and meta-analysis by Blanchard et al. (2021) found that Autism Spectrum Disorder (ASD Baron-Cohen, 2017) was associated with a substantial increase in probability of suicidality in both children and adults. However, research has yet to adequately explain how challenges unique to autistic people lead to increased suicide risk (Cassidy & Rodgers, 2017; Segers & Rawana, 2014). Turning to theories of suicide risk which were developed in psychopathology research fields other than the ASD field (e.g. Joiner, 2005) may prove useful in this regard. Indeed, readily available, and robust theoretical frameworks may further our understanding of those general mechanisms underlying risk for suicidal ideation and/or escalation to suicide attempt regardless of a particular type of psychopathology. Joiner's (2005) Interpersonal Theory of Suicide is a widely cited theory of suicide risk which has contributed to substantial advances in the scientific and clinical understanding of suicide and related behaviours (Chu et al., 2017). The theory posits that an individual will not die by suicide unless they have both the desire to die by suicide and the acquired ability to enact lethal self-injury (Joiner et al., 2005). More specifically, the psychological states of ‘Thwarted Belonging’ and ‘Perceived Burdensomeness’ together lead to the desire for suicide. While the acquired ability to enact lethal self-injury (‘suicidal capability’) occurs as a result of a biological change in the body’s fear and pain systems which allows - as opposed to inhibits - a suicide attempt” (Joiner et al., 2005). Previous reviews on the topic of suicidality and self-harm within the autistic population have been largely restricted to prevalence and risk factors (Hannon & Taylor, 2013; Hedley & Uljarević, 2018; Richa et al., 2014; Segers & Rawana, 2014). As such, there remains an incomplete understanding of the breadth and efficacy of the Interpersonal Theory to address those mechanisms underlying the increased risk for suicide in autistic people and whether this is different from non-autistic people. We set the context for capital approximation within the framework of the Basel II / III regulatory capital accords. This is particularly topical as the Basel III accord is shortly due to take effect. In this regard, we provide a summary of the role of capital adequacy in the new accord, highlighting along the way the significant loss events that have been attributed to the Operational Risk class that was introduced in the Basel II and III accords. Then we provide a semi-tutorial discussion on the modelling aspects of capital estimation under a Loss Distributional Approach (LDA). Our emphasis is to focus on the important loss processes with regard to those that contribute most to capital, the so called high consequence, low frequency loss processes. This leads us to provide a tutorial overview of heavy tailed loss process modelling in OpRisk under Basel III, with discussion on the implications of such tail assumptions for the severity model in an LDA structure. This provides practitioners with a clear understanding of the features that they may wish to consider when developing OpRisk severity models in practice. From this discussion on heavy tailed severity models, we then develop an understanding of the impact such models have on the right tail asymptotics of the compound loss process and we provide detailed presentation of what are known as first and second order tail approximations for the resulting heavy tailed loss process. From this we develop a tutorial on three key families of risk measures and their equivalent second order asymptotic approximations: Value-at-Risk (Basel III industry standard); Expected Shortfall (ES) and the Spectral Risk Measure. These then form the capital approximations.

44Patterns In The Dark : Understanding Risk And Financial Crisis With Complexity Theory

We set the context for capital approximation within the framework of the Basel II / III regulatory capital accords. This is particularly topical as the Basel III accord is shortly due to take effect. In this regard, we provide a summary of the role of capital adequacy in the new accord, highlighting along the way the significant loss events that have been attributed to the Operational Risk class that was introduced in the Basel II and III accords. Then we provide a semi-tutorial discussion on the modelling aspects of capital estimation under a Loss Distributional Approach (LDA). Our emphasis is to focus on the important loss processes with regard to those that contribute most to capital, the so called high consequence, low frequency loss processes. This leads us to provide a tutorial overview of heavy tailed loss process modelling in OpRisk under Basel III, with discussion on the implications of such tail assumptions for the severity model in an LDA structure. This provides practitioners with a clear understanding of the features that they may wish to consider when developing OpRisk severity models in practice. From this discussion on heavy tailed severity models, we then develop an understanding of the impact such models have on the right tail asymptotics of the compound loss process and we provide detailed presentation of what are known as first and second order tail approximations for the resulting heavy tailed loss process. From this we develop a tutorial on three key families of risk measures and their equivalent second order asymptotic approximations: Value-at-Risk (Basel III industry standard); Expected Shortfall (ES) and the Spectral Risk Measure. These then form the capital approximations.

Findings from the first round of evaluation of an afterschool youth development program are presented. This program, the 4H Afterschool Activity Program, incorporated a specific curriculum of aggression reduction, the BrainPower program, into its ongoing activities, which cover a wide range from homework assistance to arts and crafts. The BrainPower curriculum is a systematic application of principles of attribution theory. The experimental group for this study consisted of 50 children, aged 7 to 11 years, from 2 Los Angeles housing projects. A comparison group of 40 children did not participate in the afterschool program. Teacher and parent ratings of aggressive behavior were collected, and children's intentionality beliefs and beliefs about aggression were assessed with an instrument designed for this research. Data support the characterization of a supervised program of activities as a protective factor in the face of high rates of community crime. Afterschool program participants were perceived by teachers and parents to display fewer problem behaviors, and the differences between experimental and comparison groups increased over the 6-month study. This study examined pre-college variables from an admission-office perspective and the ability of these variables to predict college grade point average (GPA) for students specially admitted into an academic support program for at-risk students. The research was conducted at a private, highly-selective, research university in the southwest United States. The primary determining factors for this special admission program are lower-than-average high school GPA and/or standardized test scores. Pre-college variables that most significantly predicted college GPA were high school GPA, gender of student, and leadership experience prior to applying. Scholastic Aptitude Test (SAT) scores failed to predict success as measured by college GPA. (Contains 4 tables.)

Today's military doctrine fails to provide commanders and staffs with a process for exploring risk in military operations. The Joint Force Commander (JFC) requires a better method of understanding, estimating, and communicating risk to his/her troops to ensure the effective evaluation of potential hazards to joint operations. Building on civilian literature, current U.S. and Allied doctrine, and historical case studies, this thesis proposes a lexicon to enable a more meaningful understanding of risk, and to create a baseline for understanding and communicating risk. In a similar manner, the thesis develops a mental model for risk analysis to help planners examine the strategic and operational environment and better estimate the risk in proposed joint military operations. This methodology is similar to other mental processes utilized in joint operations planning, and it provides a sound basis for estimating risk. U.S. doctrine must change to provide essential guidance and tools to equip commanders and staffs to conduct worthwhile risk assessments to inform seniors and subordinates alike. The thesis proposes methods and processes to reach that objective.

Elizabeth Stern was two and a half years old, when her family emigrated from Poland to Pittsburgh. <i>My Mother and I</i> is the story of Stern's Americanization and how it ultimately alienated her from her parents. Stern's father had been a small village rabbi. Strict and traditional in his views, he sends Elizabeth to learn Hebrew at age four, so she can fulfill her destiny "as the wife of a rabbi or scholar," but he opposes letting her attend high school. Stern's mother tries fitfully to pry open doors for her daughter. When Stern's father finds Elizabeth reading a secular book, and, in a fit of rage, flings the offending novel onto the top of a tall bookcase, her mother climbs on a chair and retrieves it for her. But Stern's mother never learns English even as it becomes her daughter’s primary language--and she is burdened by endless pregnancies (she ultimately bears 11 children, only the first 4 of whom survive). Stern's relationship with her mother is loving, but when Elizabeth goes to college, they draw apart. Her mother becomes a "shadowy figure," standing with "questioning, puzzled eyes", eyes in which there is love, "but no understanding, and always an infinite loneliness." - Summary by Sue Anderson
