3DTIC ADA242033: Upper Body Exercise: Physiology And Training Application For Human Presence In Space

In the weightless state astronauts do a substantial amount of work with their arms and hands. Upper body exercise elicits a peak oxygen uptake approximately 70% of that obtained during lower body exercise; in addition, upper body exercise requires a greater oxygen uptake at a given power output than low body exercise. Therefore, when performing exercise at a given power output, both the absolute and relative exercise intensity is greater during upper body exercise. Although cardiac output responses for a given oxygen uptake are similar, the heart rate, blood pressure and total peripheral resistance responses are greater, and the stroke volume responses are lower at a given oxygen uptake during upper than lower body exercise. Body temperature responses to both exercise type are similar, but the temperature are achieved by different heat exchange mechanisms. During upper body exercise, there is a greater reliance on torso dry heat loss for temperature regulation. Exercise training programs can improve aerobic exercise capabilities for the upper body, but there are minimal cross-training benefits between the arms and legs. Space physiologists and engineers in the manned space program should consider the unique physiology associated with upper body exercise

The physiological demands of spaceflight require astronauts to have certain physical abilities. They must be able to perform routine and off-nominal physical work during flight and upon re-entry into a gravity environment to ensure mission success, such as an Extra Vehicular Activity (EVA) or emergency egress. To prepare the astronauts for their mission, a Wyle Astronaut Strength Conditioning and Rehabilitation specialist (ASCR) works individually with the astronauts to prescribe preflight strength and conditioning programs and in-flight exercise, utilizing Countermeasure Systems (CMS) exercise hardware. PURPOSE: To describe the preflight and in-flight exercise programs for ISS crewmembers. METHODS: Approximately 2 years before a scheduled launch, an ASCR is assigned to each astronaut and physical training (PT) is routinely scheduled. Preflight PT of astronauts consists of carrying out strength, aerobic and general conditioning, employing the principles of periodization. Exercise programs are prescribed to the astronauts to account for their individual fitness levels, planned mission-specific tasks, areas of concern, and travel schedules. Additionally, astronauts receive instruction on how to operate CMS exercise hardware and receive training for microgravity-specific conditions. For example, astronauts are scheduled training sessions for the International Space Station (ISS) treadmill (TVIS) and cycle ergometer (CEVIS), as well as the Advanced Resistive Exercise Device (ARED). In-flight programs are designed to maintain or even improve the astronauts pre-flight levels of fitness, bone health, muscle strength, power and aerobic capacity. In-flight countermeasure sessions are scheduled in 2.5 h blocks, six days a week, which includes 1.5 h for resistive training and 1 h for aerobic exercise. CONCLUSIONS: Crewmembers reported the need for more scheduled time for preflight training. During flight, crewmembers have indicated that the in-flight exercise is sufficient, but would like more reliable and capable hardware.

As spaceflight durations have increased over the last four decades, the effects of microgravity on the human body have become far better understood, as have the exercise countermeasures. Through use of a combination of aerobic and resistive exercise devices, today's astronauts and cosmonauts are able to partially counter the losses in muscle strength, aerobic fitness, and bone strength that otherwise might occur during their missions on the International Space Station (ISS). Since 2000, the ISS has employed a variety of exercise equipment used as countermeasures to these risks. Providing reliable and available exercise systems has presented significant challenges due to the unique environment. In solving these, lessons have been learned that can inform development of future systems.

A historical perspective on in-flight exercise in the U.S. manned space program is given. We have learned a great deal in the 25 years since the inception of Project Mercury. But, as we look forward to a Space Station and long-duration space flight, we must recognize the challenge that lies ahead. The importance of maintenance of the crewmember's physical condition during long stays in weightlessness is a prime concern that should not be minimized. The challenge lies in the design and development of exercise equipment and protocols that will prevent or minimize the deleterious sequelae of long-duration space flight while maximizing valuable on-orbit crew time.

In response to the increased influence of space forces on today's battlefield, several theater level models were analyzed for the presentation of space forces. These models were the Extended Air Defense Simulation (EADSIM), the Joint Theater Level Simulation (JTLS), the Integrated Theater Engagement Model (ITEM), the Tactical Warfare Model (TACWAR), Thunder, Janus, and the Aggregate Level Simulation Protocol (ALSP). While ALSP is not a model but a simulation protocol connecting various models, it was studied because it appears to be the future of modeling. The consensus of the analysis was that space forces are virtually ignored by most of the models and when space was considered, the dynamic nature of the systems involved were not captured. The Air Force Cornmand Exercise System (ACES) was chosen to determine how the effects of space forces can be implemented into theater level models. ACES is a discrete event combat simulation designed to support intermediate and senior service schools in teaching Air Force doctrine within the context of a theater warfare exercise. Its primary focus is to allow specific educational goals to be taught. This research focused on both the present modeling of space forces within widely used theater level models and a methodology to incorporate space forces into models that lack the influence of space. The Gulf War exposed how important functions performed from space can be to the success of a combat forces. Because of the practicality of space's influence, a picture of today's battlefield that does not include space forces is incomplete.

International Space Station (ISS) astronauts receive supervised physical training pre-flight, utilize exercise countermeasures in-flight, and participate in a structured reconditioning program post-flight. Despite recent advances in exercise hardware and prescribed exercise countermeasures, ISS crewmembers are still found to have variable levels of deconditioning post-flight. This presentation provides an overview of the astronaut medical certification requirements, pre-flight physical training, in-flight exercise countermeasures, and the post-flight reconditioning program. Astronauts must meet medical certification requirements on selection, annually, and prior to ISS missions. In addition, extensive physical fitness testing and standardized medical assessments are performed on long duration crewmembers pre-flight. Limited physical fitness assessments and medical examinations are performed in-flight to develop exercise countermeasure prescriptions, ensure that the crewmembers are physically capable of performing mission tasks, and monitor astronaut health. Upon mission completion, long duration astronauts must re-adapt to the 1 G environment, and be certified as fit to return to space flight training and active duty. A structured, supervised postflight reconditioning program has been developed to prevent injuries, facilitate re-adaptation to the 1 G environment, and subsequently return astronauts to training and space flight. The NASA reconditioning program is implemented by the Astronaut Strength, Conditioning, and Rehabilitation (ASCR) team and supervised by NASA flight surgeons. This program has evolved over the past 10 years of the International Space Station (ISS) program and has been successful in ensuring that long duration astronauts safely re-adapt to the 1 g environment and return to active duty. Lessons learned from this approach to managing deconditioning can be applied to terrestrial medicine and future exploration space flight missions.

Recent investigations of fluid pressure, morpholo gy, and enzyme activities of skeletal muscle exercised eccentrically or concentrically in normal human subjects are reviewed. Intramuscular pressures were measured before, during, and after submaximal exercise and correlated with subjective muscle soreness, fiber size, water content, and blood indices of muscle enzymes. High intensity eccentric exercise is characterized by post exercise pain, elevated intramuscular pressures, and swelling of both type 1 and 2 fibers as compared to concentric exercise. Thus, long periods of unaccustomed, high level eccentric contraction may cause muscle injury, fiber swelling, fluid accumulation, elevated intramuscular pressure, and delayed muscle soreness. Training regimens of progressively increasing eccentric exercise, however, cause less soreness and are extremely efficacious in increasing muscle mass and strength. It is proposed that on Earth, postural muscles are uniquely adapted to low levels of prolonged eccentric contraction that are absent during weightlessness. The almost complete absence of eccentric exercise in space may be an important contributor to muscle atrophy and therefore equipment should be designed to integrate eccentric contractions into exercise protocols for long-term spaceflight.

The Interim Resistance Exercise Device (iRED), developed for the International Space Station (ISS), was evaluated using human subjects for a Man-In-The-Loop Test (MILT). Thirty-two human subjects exercised using the iRED in a test that was conducted over a 63-working-day period. The subjects performed the same exercises will be used on board ISS, and the iRED operating constraints that are to be used on ISS were followed. In addition, eight of the subjects were astronauts who volunteered to be in the evaluation in order to become familiar with the iRED and provide a critique of the device. The MILT was scheduled to last for 57,000 exercise repetitions on the iRED. This number of repetitions was agreed to as a number typical of that expected during a 3-person, 17-week ISS Increment. One of the canisters of the iRED failed at the 49,683- repetition mark (87.1% of targeted goal). The remaining canister was operated using the plan for operations if one canister fails during flight (contingency operations). This canister remained functional past the 57,000-repetition mark. This report details the results of the iRED MILT, and lists specific recommendations regarding both operation of the iRED and future resistance exercise device development.

Engineers evaluating Space Shuttle flight data and performance results are using a massive data base of wind tunnel test data. A wind tunnel test data base of the magnitude attained is a major accomplishment. The Apollo program spawned an automated wind tunnel data analysis system called SADSAC developed by the Chrysler Space Division. An improved version of this system renamed DATAMAN was used by Chrysler to document analyzed wind tunnel data and data bank the test data in standardized formats. These analysis documents, associated computer graphics and standard formatted data were disseminated nationwide to the Shuttle technical community. These outputs became the basis for substantiating and certifying the flight worthiness of the Space Shuttle and for improving future designs. As an aid to future programs this paper documents the lessons learned in compiling the massive wind tunnel test data base for developing the Space Shuttle. In particular, innovative managerial and technical concepts evolved in the course of conceiving and developing this successful DATAMAN system and the methods and organization for applying the system are presented.

Long duration spaceflight has a negative effect on the human body, and exercise countermeasures are used on-board the International Space Station (ISS) to minimize bone and muscle loss, combatting these effects. Given the importance of these hardware systems to the health of the crew, this equipment must continue to be readily available. Designing spaceflight exercise hardware to meet high reliability and availability standards has proven to be challenging throughout the time the crewmembers have been living on ISS beginning in 2000. Furthermore, restoring operational capability after a failure is clearly time-critical, but can be problematic given the challenges of troubleshooting the problem from 220 miles away. Several best-practices have been leveraged in seeking to maximize availability of these exercise systems, including designing for robustness, implementing diagnostic instrumentation, relying on user feedback, and providing ample maintenance and sparing. These factors have enhanced the reliability of hardware systems, and therefore have contributed to keeping the crewmembers healthy upon return to Earth. This paper will review the failure history for three spaceflight exercise countermeasure systems identifying lessons learned that can help improve future systems. Specifically, the Treadmill with Vibration Isolation and Stabilization System (TVIS), Cycle Ergometer with Vibration Isolation and Stabilization System (CEVIS), and the Advanced Resistive Exercise Device (ARED) will be reviewed, analyzed, and conclusions identified so as to provide guidance for improving future exercise hardware designs. These lessons learned, paired with thorough testing, offer a path towards reduced system down-time.

The chief goals of the summer project have been twofold - first, for my host group and myself to learn as much of the working details of Taguchi analysis as possible in the time allotted, and, secondly, to apply the methodology to a design problem with the intention of establishing a preliminary set of near-optimal (in the sense of producing a desired response) design parameter values from among a large number of candidate factor combinations. The selected problem is concerned with determining design factor settings for an automated approach program which is to have the capability of guiding the Shuttle into the docking port of the Space Station under controlled conditions so as to meet and/or optimize certain target criteria. The candidate design parameters under study were glide path (i.e., approach) angle, path intercept and approach gains, and minimum impulse bit mode (a parameter which defines how Shuttle jets shall be fired). Several performance criteria were of concern: terminal relative velocity at the instant the two spacecraft are mated; docking offset; number of Shuttle jet firings in certain specified directions (of interest due to possible plume impingement on the Station's solar arrays), and total RCS (a measure of the energy expended in performing the approach/docking maneuver). In the material discussed here, we have focused on single performance criteria - total RCS. An analysis of the possibility of employing a multiobjective function composed of a weighted sum of the various individual criteria has been undertaken, but is, at this writing, incomplete. Results from the Taguchi statistical analysis indicate that only three of the original four posited factors are significant in affecting RCS response. A comparison of model simulation output (via Monte Carlo) with predictions based on estimated factor effects inferred through the Taguchi experiment array data suggested acceptable or close agreement between the two except at the predicted optimum point, where a difference outside a rule-of-thumb bound was observed. We have concluded that there is most likely an interaction effect not provided for in the original orthogonal array selected as the basis for our experimental design. However, we feel that the data indicates that this interaction is a mild one and that inclusion of its effect will not alter the location of the optimum.

A complete description of an instrumented ergometer system, including the sensors, the data acquisition system, and the methodologies to calculate the kinematic parameters were initially developed at Tulane University. This work was continued by the PI at NASA Johnson Space Center, where a flight ergometer was instrumented and tested during a KC-135 Zero-Gravity flight. The sensors that form part of the system include EMG probes and accelerometers mounted on the subject using the ergometer, load cells to measure pedal forces, and encoders to measure position and orientation of the pedal (foot). Currently, data from the flight test is being analyzed and processed to calculate the kinematic parameters of the individual. The formulation developed during the initial months of the grant will be used for this purpose. The system's components are compact (all sensors are very small). A salient feature of the system and associated methodology to determine the kinematics is that although it uses accelerometers, position is not determined by integration. Position is determined by determining the angle of two frames of reference for which acceleration at one point is known in coordinates of both frames.
