This study investigates psychological and contextual factors influencing individuals’ willingness to donate their electronic health record (EHR) data for medical research in Germany. As part of the national rollout of the EHR, the German government is planning to enable the so-called “research data donation,” which allows patients to voluntarily and anonymously share selected EHR entries for secondary data analysis via the Health Research Data Centre (“Forschungsdatenzentrum Gesundheit”). While research data donation is promoted as a key element for data-driven health innovation, it remains unclear which factors shape users’ willingness to donate their data. Previous qualitative research suggests that both the framing of benefits (e.g., whether donation is positioned as helping others vs. helping oneself) and the degree of control over what data is shared may significantly affect decision-making. To test these assumptions, we conduct a randomized controlled online experiment with a 4×2 between-subjects design. Two factors are manipulated: (1) Motivational framing of data donation (none vs. altruistic vs. utilitarian vs. mixed), and (2) data selection option (select specific records vs. all-or-nothing choice). Participants interact with a high-fidelity EHR prototype simulating the data donation process. Their actual decision to donate or withhold data is recorded, followed by a brief questionnaire assessing behavioral intention, trust, and perceived control. We expect that: Framing the benefits of data donation will significantly affect willingness to donate. Allowing granular data selection will increase perceived control and reduce perceived risk, thereby increasing donation rates. A mixed framing (altruistic + utilitarian) might yield the highest acceptance. The findings are expected to inform user-centered communication strategies for implementing EHR-based data donation in real-world healthcare systems.

Due to the delay of some input data sets, this formal version (Version 1) bias corrected CMORPH is produced manually once a month at a latency of 3-4 months. The PCE Risk Calculator, developed by the ACC/AHA, is frequently utilized in the United States for the purpose of averting the onset of Atherosclerotic cardiovascular disease (ASCVD) via first-line defense strategies. However, this calculator may not accurately estimate risk for certain populations, potentially leading to either under- or over-estimation of risk. We have created calculator for ASCVD risk specific to a population by leveraging advanced Machine Learning (ML) techniques and Electronic Medical Record (EMR) data. Our study involved comparing predictive accuracy of our calculator with PCE calculator. Between January 1, 2009, and April 30, 2020, data was gathered from 101,110 distinct EMRs of patients who were actively receiving treatment. Patient datasets underwent machine learning techniques containing Longitudinal (LT) and Cross-Sectional (CS) features, or solely CS features, derived from laboratory values and vital statistics. The models' effectiveness was assessed using fresh price metric (Screened Cases Percentage @Sensitivity level). In terms of prediction accuracy, every ML model that was tested performed better than the PCE risk calculator. Area Under Curve (AUC) score of 0.902 was obtained by Random Forest (RF) ML technique when CS and LT characteristics were combined (RF-LTC). Our machine learning model only needed to screen 43% of patients in order to identify 90% of positive ASCVD cases, in contrast to the PCE risk calculator, which required screening 69% of patients. Prediction models created using ML techniques reduce the amount number of tests necessary to forecast ASCVD and increase the accuracy of ASCVD prediction when compared to using PCE calculator alone. The combination of LT and CS features in these ML models leads to a significant enhancement in comparing the ASCVD prediction to utilizing CS features exclusively.

Anxiety and depression are common psychiatric illnesses with multiple causes and shared risk factors. To treat these conditions, the NHS launched the NHS Talking Therapies program, aiming to increase access to evidence-based psychological therapies. While the annual recovery rates following these treatments seem promising, the latest reports suggest that half of the patients are not meeting standard definitions of recovery by the end of their treatment. Therefore, it is essential to identify patients at risk of poor outcomes and consider treatment adaptations or augmentations. The NHS Talking Therapies service collects outcome information at every session, providing a unique opportunity to investigate predictors of treatment outcome in a large, community-based sample of individuals with anxiety and/or depressive disorders. Using this information, we aim to develop and internally validate multivariable prediction models that identify patients at risk of poor prognostic outcomes. We will develop models using (i) information collected at enrolment only; and (ii) session-by-session information. This work could inform screening tools to better target psychological therapy and suggest treatment adaptations, avoiding inappropriate or ineffective treatment allocations, and ultimately reducing the healthcare and economic burden.

A comprehensive cost analysis of the duplicate elimination operation is presented. This analysis is based on a combinatorial model developed for estimating the size of intermediate runs produced by a modified merge-sort procedure. The performance of this merge-sort procedure is demonstrated to be significantly superior to the standard duplicate elimination technique of sorting followed by a sequential pass to locate duplicate records. The results can also be used to provide critical input to a query optimizer in a relational database system. (Author)

Abstract Objectives: Extension of the standard model while retaining compliance with it is a challenging issue because there is currently no method for semantically or syntactically verifying an extended data model. A metadata-based extended model, named CCR+, was designed and implemented to achieve interoperability between standard and extended models. Methods: Furthermore, a multilayered validation method was devised to validate the standard and extended models. The American Society for Testing and Materials (ASTM) Community Care Record (CCR) standard was selected to evaluate the CCR+ model; two CCR and one CCR+ XML files were evaluated. Results: In total, 188 metadata were extracted from the ASTM CCR standard; these metadata are semantically interconnected and registered in the metadata registry. An extended-data-model-specific validation file was generated from these metadata. This file can be used in a smartphone application (Health Avatar CCR+) as a part of a multilayered validation. The new CCR+ model was successfully evaluated via a patient-centric exchange scenario involving multiple hospitals, with the results supporting both syntactic and semantic interoperability between the standard CCR and extended, CCR+, model. Conclusions: A feasible method for delivering an extended model that complies with the standard model is presented herein. There is a great need to extend static standard models such as the ASTM CCR in various domains: the methods presented here represent an important reference for achieving interoperability between standard and extended models.

Methods: For hemodialysis data modeling, we identified common data elements for hemodialysis information (CDEHI). We used ASTM continuity of care record (CCR) and ISO/IEC 11179 for the compliance method with a standard model for the CDEHI. According to the contents of the ASTM CCR, we mapped the CDHEI to the contents and created the metadata from that. It was transformed and parsed into the database and verified according to the ASTM CCR/XML schema definition (XSD). DialysisNet was created as an iPad application. The contents of the CDEHI were categorized for effective management. For the evaluation of information transfer, we used CarePlatform, which was developed for data access. The metadata of CDEHI in DialysisNet was exchanged by the CarePlatform with semantic interoperability. Results: The CDEHI was separated into a content list for individual patient data, a contents list for hemodialysis center data, consultation and transfer form, and clinical decision support data. After matching to the CCR, the CDEHI was transformed to metadata, and it was transformed to XML and proven according to the ASTM CCR/XSD. DialysisNet has specific consideration of visualization, graphics, images, statistics, and database. Conclusions: We created the DialysisNet application, which can integrate and manage data sources for hemodialysis information based on CCR standards.

The data presented in this report summarize the background characteristics of 234,796 college students who took the Graduate Record Examinations (GRE) Aptitude Test during 1975-1976 and who responded to at least one of the thirteen background questions. The data is roughly at 16 km resolution over the entire globe with 90 observations per scan using channels - 89, 150, 157, 183 +/- 1, 183 +/- 3, 183 +/- 7, 190 GHz. The AMSU-B sensor was flown on NOAA-15, -16, and -17 satellites. The MHS sensor was flown on NOAA-18, -19 and MetOp-A satellites and replaced the AMSU-B sensor (which has very similar attributes) and was flown on NOAA-15, -16 and -17. Visual inspections and verification of the various corrections were applied to the data to improve data accuracy.

This report examines the feasibility of implementing a student unit record system to replace the student-related components that currently are based on aggregate institution-level data collected as part of the Integrated Postsecondary Education Data System (IPEDS). The feasibility study was initiated by the National Center for Education Statistics (NCES), a part of the Institute of Education Sciences (IES) within the Department of Education (ED), in response to renewed interest within the postsecondary education community for more accurate measures of net price and graduation rates, especially measures that take into account institutional mission and student mobility. This interest parallels a growing congressional desire to hold postsecondary institutions accountable for student outcomes. The feasibility study was conducted between October and December 2004. As this report has outlined, a central question for a UR system is "Could it be done?" Have the information technologies and infrastructures at the campus and state levels matured, could the current IPEDS web?based reporting system be adapted to a UR system, and would there be adequate technical and legal protections in place at IES/NCES? The report has addressed some of the technical and system problems associated with the design and development of a new IPEDS UR system. At the technical level, a UR system could be done at most institutions given time for implementation. The following are appended: (1) Technical Review Panels; and (2) Estimates of Burden. Their guide, slashing through the rampant growth with his machete, leads them to a structure with steps up the side, shaped like a pyramid. Next they see a stone column, fourteen feet high, sculptured on the front with a portrait of a man, “solemn, stern and well fitted to excite terror,” covered on the sides with hieroglyphics, and with workmanship “equal to the finest monuments of the Egyptians.” Stephens records these discoveries and also his travels in Central America, where he had been sent by President Van Buren as special ambassador to the ill-fated Republic of Central America. The republic being engulfed in civil war when Stephens arrives in Guatemala, he finds himself dodging revolutionary armies while he hunts for a “legitimate government” to which to present his credentials. Catherwood, meanwhile, directs his immense artistic talent to illustrating views of Mayan architecture. Stephens, the Anglican Dean of Winchester, writes a short, lively biography of the great church reformer, Hildebrand of Sovana (1015-1085), afterwards Pope Gregory VII, setting his life within the larger context of the struggle for dominance between the Holy Roman Empire and the Papacy during the Middle Ages. The roots of the conflict can be traced to the alliance made between Pope Stephen II and his successors and the Frankish King Pippin and his son Charlemagne to break the power of the Lombard Kingdom in Italy. Later emperors sought first to reform and then to dominate the Papacy, but they finally met their match in Hildebrand, leading to the famous confrontation between Pope and Emperor on the snowy steps of Canossa Castle. Facing an imperial invasion, Pope Gregory took the fatal step of summoning his fierce Norman allies. They sacked and burned Rome and carried Hildebrand off to Salerno where, his body weak but his spirit unbowed, he breathed his last crying, "I have loved righteousness and hated iniquity--therefore I die in exile." This is a comic novel written by Irish author James Stephens, a quick-witted storyteller whose pantheistic philosophy is revealed in his adult Irish fairy tales. His first novel, The Charwoman's Daughter (1911), humorously examines the life and fantasies of a poor Dublin mother and daughter. His second, The Crock of Gold (1913), again showcases his unique writing style, quirky thoughts, grasp of irony and cleverness of phrase. No conformity here, lots of head-scratching twists and turns that reveal odd bits of wisdom too! The main characters are an extremely pedantic Philosopher, his revengeful wife, their sweet innocent children lured down leprechaun holes, a teenage girl seduced by Pan (the god of lust and carefree living) then saved by Angus Og (the Irish god of youth, love and beauty), culminating in a giant parade of Irish gods. Stephen's serious philosophy is on display here and there, and we learn many useful tips for dealing with fairies, goblins and gods, especially that no good comes of stealing a leprechaun's crock of gold! ~Summary by Michele Fry

In this memorialized Irish tale, 3 huge winged demi-gods (part god/part human) have descended to earth to experience human existence. They encounter Patsy Mac Can and his daughter Mary at their campfire one evening, two vagabonds who wander about Ireland with donkey and cart, eating off the land, meeting a variety of dubious characters along the way, who in the great Irish tradition of storytelling, all have stories to tell. The Mac Cans welcome the strangers, feed them, show them around, along the way spinning yarns and explaining their gypsy "philosophies". Those familiar with Stephens's quirky writing style appreciate him as a quick witted, keen observer, seeing both the ridiculous and the sublime in everyday things, exploring the points of view of various animals and insects as well as the human characters. Eventually the troupe parts company, having forged close bonds of friendship. This is an entertaining adult love story that would not offend a child, read charmingly by authentic Irishman Joe Bergin. Written during the Civil War, Ann Stephens had no concept of the war's conclusion or how long it would last. An ardent unionist, she writes a war history of the kind no longer in fashion - extirpating on the glories of battle and the valor of patriotic sacrifice. In our post-Red Badge of Courage world, this type of writing seems strange. In extreme detail, she recounts both the famous battles of the early war, as well as skirmishes forgotten today.
