‘Mindful eating’ is a terminology that has been coined to describe the application of mindfulness to eating related thoughts, emotions, bodily sensations and behaviours (Tapper, 2022). Mindful eating is increasingly promoted as a strategy for weight management (Fuentes Artiles et al., 2019; Mason et al., 2016; Smith et al., 2018). A systematic review by Fuentes Artiles et al. (2019) examined 10 studies and found that mindful eating strategies significantly contributed to weight loss when compared to control interventions. However, several authors have highlighted that there is no consensus definition of mindful eating and interventions often incorporate a diverse range of practices (Mantzios, 2023; Tapper, 2022). Therefore, it is unclear which specific practice(s) of mindful eating affect dietary behaviour (Tapper, 2022). In our previous work, we conducted a multi-site systematic review and meta-analysis of all experimental studies that have manipulated different mindful eating practices and have examined effects on food intake and appetite (see PROSPERO registration here; https://www.crd.york.ac.uk/prospero/display_record.php?ID=CRD42022346160). Based on this review, we identified ten mindful eating practices that tend to be manipulated within experimental food intake studies. Following this review, utilising an online survey, we designed a new questionnaire to assess these ten mindful eating practices with the aim to explore how they relate to each other conceptually (factor structure) and to explore correlations with self-reported BMI. Using exploratory factor analysis, we identified four factors of mindful eating with good or acceptable reliability: 1. Cravings, 2. Attention and awareness of food, 3. Acceptance of food-related thoughts and 4. Food-related cues. The 'Cravings' factor includes four aspects of mindful eating: 1) present moment awareness of cravings, 2) acceptance of hunger or craving, 3) decentering of hunger or craving and 4) decentering from food-related thoughts. The 'attention and awareness of food' factor includes two aspects of mindful eating: 1) present moment awareness of sensory properties of food and 2) attention regulation. 'Acceptance of food-related thoughts' has one aspect of mindful eating: acceptance of food-related thoughts. 'Food-related cues' includes three aspects of mindful eating: 1) present moment awareness of internal bodily sensations relating to hunger, fullness and eating, 2) present moment awareness of cues that elicit eating of the urge to eat and 3) present moment awareness of food-related thoughts. Re-analysis of our earlier systematic review using these four conceptual factors, revealed that only the attention and awareness of food factor was related with both acute food intake and self-reported BMI. In my PhD, I intend to explore this factor further within an experimental study and a real-world feasibility study. These studies will run in parallel and this pre-registration relates to the real-world feasibility study (to see the pre-registration for the experimental study please see: https://doi.org/10.17605/OSF.IO/2PRUF). Attention and awareness of food encompasses the following practices: present moment awareness of sensory properties of food (e.g. paying attention to whether the food is chewy, sweet, crunchy etc. while eating) and attention regulation (ability to keep attention on the food itself and not to let the mind wander). With regards to present moment awareness of sensory properties, we note that in our systematic review the majority of studies focus on this aspect. However, it is unclear whether specifically focusing on the sensory properties of a food is important or whether generally paying more attention to the food that you are eating and not being distracted by environmental influences is the key factor. Indeed, we note that studies show that cognitive distraction (e.g., eating lunch while watching TV) is associated with consuming more at a subsequent snacking opportunity (Higgs & Woodward, 2009). Few studies have explored the attention regulation component of mindful eating. Four papers that addressed attention regulation were found in our systematic review (Arch et al., 2016; Bennett et al., 2020; Cavanagh et al., 2014; Mantzios et al., 2020). Among these, three of them found that increased attention regulation reduced food intake (Arch et al., 2016; Cavanagh et al., 2014; Mantzios et al., 2020) and one reported no impact on food intake (Bennett et al., 2020). Moreover, we note that few studies have explored the impact of this type of intervention on longer term measures of eating behaviour (e.g., changes in body weight). Many studies has investigated the effects of mindful eating practices on body weight, but few of them have taken a ‘dismantling’ approach to mindful eating (i.e., they combine multiple mindful eating practices within an intervention) to determine which specific practices affect body weight (Daubenmier et al., 2016; Spadaro et al., 2017). On this basis, in this study, we plan to explore the feasibility and efficacy of manipulating present moment awareness of the sensory properties of food and attention regulation on eating behaviour in a real-world setting and over a longer time period. Our manipulation of mindful eating will include a combination of both practices: ‘present moment awareness of the sensory properties of food’ and ‘attention regulation’. We plan to conduct an online study to examine the effects of mindful eating outside the laboratory setting. This 14-day feasibility and pilot study will involve two groups: a mindful eating group and a control group. The mindful eating group will focus on present moment awareness of food's sensory properties and attention regulation and at the same time receive standard NHS dietary advice. The control group will only receive standard dietary advice from the NHS. The primary outcome will be the feasibility outcomes associated with the intervention. We will assess the feasibility of the mindful eating instruction. The secondary outcome will measure overall weight reduction for both groups (post-intervention body weight minus pre-intervention body weight).
