Abstract The purpose of this study was to establish audiology referral protocols for post meningitis paediatric populations in two academic hospitals in Gauteng, South Africa. Specific objectives of this study included determining if audiological assessment referrals were made following infection; determining the time of referral post meningitis diagnosis; establishing what audiological assessments were conducted on this population, as well as determining any correlations between signs and symptoms of meningitis and referrals for audiology assessments. Medical records of 47 children admitted to hospital with a diagnosis of meningitis between the ages of birth and 6 years were reviewed following a retrospective record review design. Data relevant to the current study were obtained from hospital records and this was captured in a data spreadsheet. Both descriptive and inferential statistics were implemented in analysis of the data. Inferential statistics in the form of logistic regression analysis was used to establish any significant factor that may predict referral for audiological assessment. The findings indicated that almost half (40%) of the cases were not referred for audiological services. Of those cases referred for assessment, 89% were referred as in-patients before hospital discharge, with minimal referrals occurring after discharge from hospital. In a variety of insect species, temperature plays a fundamental role in determining the outcome of insecticide exposure. However, surprisingly little is known about how temperature affects the efficacy of chemical interventions against malaria vectors. Methods: Anopheles stephensi, with no recent history of insecticide exposure, were exposed to the organophosphate malathion or the pyrethroid permethrin at 12, 18, 22, or 26°C, using the WHO tube resistance-monitoring assay. To evaluate the effect of pre-exposure temperature on susceptibility, adult mosquitoes were kept at 18 or 26°C until just before exposure, and then moved to the opposite temperature. Most fractures of the cervical spine are considered stable and are treated with a rigid collar. In fact, most trauma guidelines recommend the use of cervical collars following a fracture. However, there are significant adverse events to the long-term use of cervical collars including the development of pressure ulcers, impaired respiratory function and dysphasia. In the frail elderly, immobilization has been associated with aspiration and respiratory failure. Given the high incidence of adverse events it is imperative that we identify the best immobilization protocols that may lead to better overall outcomes. Background Nearly 1 billion people globally are estimated to live with a mental disorder, and among this 1 billion, 5% of adults have major depression.1 The symptoms of major depressive disorder (MDD) can be categorized into emotional, neurovegetative, and neurocognitive domains.1 These symptoms can profoundly impar quality of life, psychosocial functioning, and the economic productivity of the individuals. In 2008, WHO ranked MDD as the third leading cause of burden of disease worldwide and estimated that it will be ranked first by 2030.2 The development of effective treatment modality for psychiatric disorders is an enduring goal of translational research and evidence-based medicine. Psychopharmacological treatment focuses on the link between specific neurotransmitters and a proposed pathophysiology, while treatment response to this approach may be suboptimal in several major psychiatric disorder, including MDD.3 In recent decades, progress in neuroscience has identified the disordered brain circuits and networks that may underpin the pathogenesis of psychiatric disorders.4 Non-invasive brain stimulation (NIBS) is a set of technologies and techniques that modulate excitability of a specific region or large-scale networks in the brain.5 Studies have showed promising results in circuit-based psychiatric treatments in either diagnosis- or symptom-based clinical conditions.6-8 Transcranial magnetic stimulation (TMS) is one of the most widespread NIBS techniques in research and clinical practice. The application of TMS involves a strong, pulsed magnetic field to a targeted brain region.5 A coil generating an electromagnetic field is placed on the scalp, such that strong magnetic pulses are delivered to a relatively focal area of cerebral cortex, resulting in regional neuronal depolarization and generation of action potentials. In treatment protocol, TMS is typically delivered in bundles or “trains” of pulses, separated by periods of rest. Such protocol is the relevance for application of TMS in treating psychiatric disorders and now is referred as repetitive TMS (rTMS).5 In 2008, the Food and Drug Administration agency of the United States approved rTMS as a treatment for medication-resistant patients with MDD (also known as treatment-resistant depression [TRD]).9 The suggested protocol of rTMS was the delivery of 3,000 pulses per session of excitatory rTMS (10 hertz (Hz)) at 120% resting motor threshold in left dorsolateral prefrontal cortex (DLPFC), and a series of 20–30 once-daily sessions are given over the course of four to six weeks. Of note, a large variability existed in the published protocols of rTMS studies, such as the stimulation intensity, the number of sessions proposed (10–30), the number of stimuli per session (120–3000), the targeted brain regions (e.g., left DLPFC, right DLPFC, bilateral DLPFC), and the included population (MDD or TRD). The number of sessions and stimuli are usually lower when applying low frequency rTMS of the right DLPFC as compared to high frequency rTMS of the left DLPFC. One recent meta-analysis showed that increasing the number of sessions and the total number of pulses per session (with an optimal value of 1200–1500 pulses/session) was associated with an increased antidepressant efficacy of high frequency rTMS of the left DLPFC.10 A few studies compared the both types of stimulation (low frequency rTMS on the right vs. high frequency rTMS on the left) and rather showed a similar antidepressant efficacy, even when rTMS was used in augmentation or as an add-on treatment to antidepressants in pharmacological refractory MDD.11 Aims The aim of the current study is to examine the dose-response relationship between different rTMS parameters and clinical effectiveness of depression improvement in patients with TRD. The parameters of rTMS included intensity, frequency, pulse per session, treatment duration, total session, and total pulses. Total pulses is a product of total sessions and stimuli per session. For example, the dose of a protocol of 20 sessions with 3000 stimuli per session will be calculated as 60000 pulses, and the dose of a protocol of 5 sessions with 3000 stimuli per session will be calculated as 15000 pulses. Methods The protocol of the current systematic review and meta-analysis will be conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) 2020 statement. Ethical approval is waived in this meta-analytic study. Search strategy The MEDLINE, Cochrane Central Register of Controlled Trials (CENTRAL), EMBASE, PsycINFO, Clinical trials.gov, and World Health Organization (ICTRP) will be systematically searched to identify RCTs testing different antimanic agents without language restriction. The specific search terms are to be determined and will be adapted to each database. We will manually search the bibliographies of selected studies and reference lists of review articles meeting the inclusion criteria to locate additional relevant studies. At least two authors will independently screen and select the potentially eligible studies. Any discrepancies will be resolved by consensus. PICO The PICO (population, intervention, comparison, outcome) settings of the current meta-analysis are: P: Adults patients with treatment-resistant depression I: rTMS over left DLPFC C: Sham control or another protocol of rTMS over left DLPFC O: Improvement of depressive symptoms Inclusion and exclusion criteria We will only include blinded randomized controlled trials (RCTs) that reported the necessary information (e.g., standard deviations, means, and sample sizes). We will exclude: (1). Nonblind RCT. (2). Non-RCT. (3). RCT that enrolled participants without TRD (4). Relapse prevention studies (i.e., RCT addressing on maintenance treatment). (5). RCT of other brain regions (6). RCT only including participants <18 years old Outcome definition and data extraction The primary outcome will be improvement of depressive symptoms. The secondary outcomes will be acceptability and serious adverse events. For each study, we will extract the following data: (1). Trial characteristics (e.g., sample size, authors, publication years). (2). Treatment protocols (e.g., Hz, total session, pulses per session) (3). Standard deviation (or variance, standard error) and means of changes in depressive symptoms over the study period for both active and control groups, as measured with standard rating scale for depression (e.g., Hamilton Depression Rating Scale; Montgomery Asberg Depression Rating Scale, Beck Depression Scale). Where multiple publications of the same study existed, maximal data will be extracted from these subsequent publications and included as part of the initial publication. In some studies, after a defined period of blinded treatment, continuation treatment was offered only to the responders (resulting in different patient retention rate between conditions) that became unblinded or open-label conditions. As for these studies, we will use only the data from the blinded acute treatment period, which will include the original randomized sample. At least two authors will double-check the data-transfer accuracy and calculations. The quality of the included studies will be rated using the Cochrane Risk of Bias Assessment Tool. 12 Any discrepancies will be resolved by consensus. Data analysis Data management and analysis will be carried out using Stata (version 16) and R-Project (V.4.0.3, R Foundation). A p value of <0.05 is considered significant (two-tailed). The standardized mean difference in improvement of depression scores will be used as the effect size with a 95% confidence interval. A dose-response meta-analysis with a random-effects model will be conducted. An intention-to-treat approach will be used. PCA will be performed for dimension reduction. The I² statistic will be used to evaluate heterogeneity. Subgroup analysis, meta-regression, and assessment of possible publication bias (Egger’s test and visual inspection of funnel plots) analyses will be conducted if feasible. Heartworm disease, caused by Dirofilaria immitis, is a potentially life-threatening heart disease in dogs with zoonotic potential. While the American Heartworm Society recommends melarsomine dihydrochloride for the treatment of adult heartworm infection, its limitations, such as high costs and post-treatment complications, prompt the need for alternatives. A prominent emerging treatment involves Macrocyclic Lactones (MLs) combined with Doxycycline, with topical moxidectin showing significant efficacy. However, concerns remain, including potential pulmonary pathology. As of October 2023, no systematic review and meta-analysis has addressed the combined use of topical moxidectin and doxycycline for heartworm in dogs. This research aims to fill this gap by evaluating the efficacy and safety of this combination.

This article is from Journal of Physical Therapy Science , volume 26 . Abstract [Purpose] VO2 is expressed as the product of cardiac output and O2 extraction by the Fick equation. During the incremental exercise test and constant high-intensity exercise test, VO2 results in the attainment of maximal O2 uptake at exhaustion. However, the differences in the physiological components, cardiac output and muscle O2 extraction, have not been fully elucidated. We tested the hypothesis that constant exercise would result in higher O2 extraction than incremental exercise at exhaustion. [Subjects] Twenty-five subjects performed incremental exercise and constant exercise at 80% of their peak work rate. [Methods] Ventilatory, cardiovascular, and muscle oxygenation responses were measured using a gas analyzer, Finapres, and near-infrared spectroscopy, respectively. [Results] VO2 was not significantly different between the incremental exercise and constant exercise. However, cardiac output and muscle O2 saturation were significantly lower for the constant exercise than the incremental exercise at the end of exercise. [Conclusion] These findings indicate that if both tests produce a similar VO2 value, the VO2 in incremental exercise would have a higher ratio of cardiac output than constant exercise, and VO2 in constant exercise would have a higher ratio of O2 extraction than incremental exercise at the end of exercise.

The new architecture for the Joint Maritime Command Information System, referred to as JMCIS'98, seeks to provide uniform access to tactical and non-tactical information. The goal is to allow access to such information using Wide Area Network technology and Personal Computers running Windows NT in a web environment. This architecture relies on web servers to deliver executable content, such as Java applets, to clients and gateway servers to route requests to the appropriate servers and/or databases. This architecture raises new security risks which must be addressed. Two of these risks are addressed in this thesis: executing downloaded code from a web server and transmitting sensitive information, such as passwords, to gateway servers. We investigate three cryptographic protocols to address these risks. The first protocol treats the risk of executing downloaded code from a web server by using digital signatures. The second protocol addresses the transmission of sensitive information to a gateway server by using certificates and symmetric key cryptography. Finally, we explore an alternative approach, that of the Secure Sockets Layer, which provides mutual authentication. We discuss how the first two protocols can be implemented in Java using the Java Developer's Kit (JDK) 1.1 and the Java Cryptography Extension (JCE) 1.1

The Verifiability Approach (VA; Nahari, Vrij & Fisher, 2014a) is a verbal lie-detection technique that uses the frequency (and proportion) of details than can be checked by investigators (i.e. verified) to distinguish genuine and fabricated statements. The verbal differences predicted by the VA can be further amplified by using the information protocol (IP; see Nahari, Vrij & Fisher, 2014b). The IP informs interviewees, inter alia, of the importance of reporting verifiable details. The IP instruction has been shown to amplify ability of the VA to accurately distinguish between genuine and fabricated statements using reported checkable information. Previous research investigating the VA has explored using a single IP instruction, administered in a single ‘block’ (all-at-once) at the start of the interview to amplify verbal differences between liar’s and truth-teller’s statements in terms of verifiable detail reported (e.g. Nahari et al., 2014b; Harvey et al., 2017a). In these studies, the interview is simple, consisting of a few (often a single) recall attempts after the IP is administered. Hence, the current research literature presents a practical problem for investigators: it is unclear how the full VA should be integrated within a longer, more complex interviewing framework (for example, that of a cognitive interview) often used in practice. One solution is to develop an IP that, rather than being administered all-at-once in a single block (best suited for interviews comprised of a single recall attempt), can be administered in gradual stages (in stepped increments) before multiple recall attempts (suited for more complex interviews). The aim of the current research is to investigate the effectiveness of incrementally disclosing the IP instruction across multiple steps for distinguishing between genuine and fabricated statements using the VA.

Point of care ultrasound (POCUS) assessments of the lung and thoracic regions are being performed by physiotherapists around the world. POCUS can be used for baseline measurements, to guide assessments, and to assist with the selection of appropriate interventions. Unfortunately, there is limited evidence on the type and content of competency training physiotherapists receive prior to implementing POCUS into their clinical practice. There is a need to map the evidence to determine what protocols are currently in use, their details, time to administer, measurement properties, and which populations/conditions they are currently being used to assess. This will help guide the development of a training program for graduate and post-graduate physiotherapy students and practitioners, guided by the most appropriate POCUS protocol(s).

This research project is a systematic review investigating how post-activation performance enhancement (PAPE) protocols affect barbell velocity during the bench press in trained individuals. By examining various interventions—such as submaximal loads, blood flow restriction (BFR), and ischemic preconditioning (IPC)—the aim is to determine which methods most effectively and reliably boost peak and mean velocity in the upper body. A broad database search (Scopus, SpringerNature, EBSCO, Web of Science) was conducted up to 31 January 2025, focusing on studies involving adults aged 18–50 with at least two years of resistance training experience and measuring barbell velocity or power parameters. The included studies, all scoring moderate quality on the PEDro scale, reveal that submaximal loads of 70–80% 1RM, combined with four to five minutes of rest, frequently enhance velocity (up to +7%) and peak power (up to +15.9%), whereas loads above 85% 1RM often induce excessive fatigue that negates these benefits. BFR or IPC tend to show positive effects chiefly with lighter loads (20–50% 1RM). Through synthesizing this evidence, the project aims to provide practical guidance for strength coaches, athletes, and researchers seeking to implement PAPE in upper-body training. It also underscores the need for more rigorous future studies—particularly randomized controlled trials with larger, more diverse populations, including female athletes—to refine these protocols and expand the current understanding of short-term performance enhancements in the bench press.
