Abstract Background: Behcet's disease (BD) is a systemic inflammatory disease with the histopathological features of leukocytoclastic vasculitis that affects nearly all organs and systems. When it involves the intestine, it is called entero-Behcet's disease (entero-BD). Case presentation: Here we described a 23-year-old man with entero-BD refractory to conventional therapies who responded well to the combination therapy of infliximab, an anti-tumor-necrosis-factor (TNF)-alpha antibody, and thalidomide. After combination treatment, the patient’s symptoms improved greatly and his Crohn's Disease Activity Index (CDAI) score decreased from 344 to 52 points, accompanied by a body weight increase from 53 kg to 64 kg. A follow-up endoscopy performed 10 weeks after the treatment showed significant improvement and the patient's multiple ulcers had healed well. Conclusion: The combination therapy of infliximab and thalidomide appears to be clinically effective in a patient with refractory entero-BD. However, further studies need to be performed to evaluate the efficacy of this combination therapy.

Abstract A rapid, sensitive and specific analytical method based on high-performance liquid chromatography-tandem mass spectrometry (LC-MS/MS) has been developed for the determination of thalidomide concentration in human plasma. The analyte and internal standard were extracted by liquid-liquid extraction with ether-dichloromethane (3:2, v/v) and separated on a TC-C18 column using methanol-10 mM ammonium acetate-formic acid (60:40:0.04, v/v/v) as the mobile phase at a flow rate of 0.9 ml/min. The detection was performed using an API 4000 triple quadrupole mass spectrometer in the positive electrospray ionization (ESI) mode and completed within 3.0 min. The multiple reaction monitoring (MRM) transitions were m/z 259.1→84.0 for the analyte and m/z 195.9→138.9 for temozolomide. The calibration curve exhibited a linear dynamic range of 2–1500 ng/ml (r>0.9991). The intra-and inter-day precisions (as relative standard deviation; RSD) were 6.8–13.5% and 4.3–5.0% respectively and the accuracy (as relative error; RE) was 2.0–3.5%. The recoveries and matrix effects were satisfactory in all the biological matrices examined. This method was successfully used in a pharmacokinetic study of thalidomide in healthy male volunteers receiving an oral administration of a 200-mg dose.

Abstract The aim of the present study was to investigate the inhibitory effects of thalidomide in the hepatocellular carcinoma nude mouse model in order to provide new insights into a comprehensive clinical intervention for hepatocellular carcinoma. MHCC97 cells were routinely cultured, passaged and adjusted to a single cell suspension with a concentration of 2×107/ml. Six-week-old, BALB/C male nude mice were anesthetized and fixed in the prone position, then a subcapsular injection of the single cell suspension was administered into the spleen and their abdomens were closed. A laparotomy and left hepatic lobectomy was performed 14 days later and the abdomens were closed once again. Subsequent to the establishment of the hepatocellular carcinoma model, the nude mice were randomly divided into three groups, each consisting of 12 mice. The early intervention group were immediately provided with the post-operative thalidomide intervention, the late intervention group were provided with the post-operative thalidomide intervention one week subsequent to the surgery, and the negative control group were provided with a placebo intervention (0.9% physiological saline). Each intervention was continuously administered once per day for one week. The osteopontin (OPN) content of the liver tumors was detected using immunohistochemistry. The data were analyzed using an analysis of variance (ANOVA) test. There were significant differences in the OPN levels of the tumors among the early intervention, late intervention and negative control groups. Thalidomide may inhibit the generation of OPN and thereby inhibit the infiltration and metastasis of tumors; the immediate use of thalidomide following hepatectomy in the present study may block the invasion and metasis for liver cancer more effectively.

Abstract BACKGROUND: Leprosy can have its course interrupted by type 1 and 2 reactional episodes, the last named of erythema nodosum leprosum (ENL). Thalidomide has been the medication of choice for the control of ENL episodes since 1965. OBJECTIVES: These episodes can repeat and cause damages to the patient. In order to prevent these episodes, an extra dose of 100 mg/day thalidomide was used during six months, followed by a follow-up period of six more months after thalidomide discontinuation. METHODS: We included 42 patients with multibacillary (MB) leprosy who had episodes of ENL. They were male and female patients aged between 18 and 84 years. RESULTS: Of the 42 patients, 39 (92.85%) had the lepromatous form and three (7.15%) had the borderline form. We found that 100% of patients had no reactional episode during the use of the drug. During the follow-up period after thalidomide discontinuation, 33 (78.57%) patients had no reactional episode and nine (21.43%), all of them with the lepromatous form, had mild episodes, which were controlled using non-steroidal anti-inflammatory. There were no thalidomide-related side effects. CONCLUSION: A maintenance dose of 100 mg/day of thalidomide showed to be effective to prevent repeated type 2 reactional episodes of ENL.

Significant progress has been made in the understanding of key factors that regulate the cell-cell interaction in the context of the microenvironment of prostate cancer. This includes technical advances in getting information from small amounts of tissue to forward understanding of the molecular determinants of progression. We have developed tissue micro arrays (TMAs), and stained them for candidate factors implicated in stromal epithelial interaction and have demonstrated that they are expressed in the context of Thalidomide treated patients. This information will be used to compare these results to the expression patterns in similar prostate cancers not exposed to Thalidomide. We are requesting a no-cost extension of 6 months to allow completion of the planned studies. A formal letter will be sent separately.

Abstract Hereditary hemorrhagic telangiectasia is characterized by mucocutaneous and visceral telangiectasia and involves several organs with vascular malformations. It is an autosomal dominant disease and is distinguished into three types, which are due to mutations in different genes. The common symptom is anemia, causing a continous need of blood transfusion. Depending on the severity and disease manifestation, there are various forms of therapy ranging from local therapy activities to operations or drug therapy. Here we describe a dramatic improvement of a patient with a high transfusion frequency due to severe recurrent anemia successively treated with thalidomide and bevacizumab.

The purpose of this award id to evaluate the: 1) Safety and toxicity of neo-adjuvant thalidomide therapy prior to radical prostatectomy in patients with locally advanced prostate carcinoma (PCa); 2) Efficacy of neo-adjuvant thalidomide treatment, as measured by the rate of tumor reduction/ PSA decline; 3) Qualitative measurements of the in vivo effect of thalidomide therapy on the Endothelial and Epithelial compartment. Significance: The ability to assess in vivo the effects of thalidomide as well as identify surrogate markers of anti-angiogenic activity is in valuable to the design of new effective therapies. Prior to the DOD's final decision (April 2002) to sponsor the correlative studies proposed in the grant 18 patients had been entered in this study. Their clinical information is attached. Pending some additional HSRRB required amendments, the patients' research tissue was stored but the funds awarded were not used to perform any of the studies until we obtained the final DOD approval. After April 2002 (when DOD was cited as a partial sponsor) and until we solved all the remaining issues and obtained DOD clearance to proceed (9/2003) we held accrual in this trial. We have re-activated the study and are confident that we will complete patient accrual within 1 year. We are requesting a no-cost extension of 1 year to allow completion of the planned studies.

Abstract INTRODUCTION: Thalidomide was available for use over-the-counter between 1958 and 1962, and more than 300 thalidomide-impaired people have been confirmed in Japan. Currently, thalidomide-impaired people are nearing the age of 50 years and sometimes require medical treatment or surgery. However, a sphygmomanometer cannot be used to measure the blood pressure in some thalidomide-impaired people because of upper-limb shortening or hypoplastic defects. We encountered a patient with thalidomide-related upper limb defects who required abdominal ovarian cystectomy. PRESENTATION OF CASE: The patient was a 49-year-old woman (146.5 cm, 35.9 kg) with thalidomide-related upper-limb defects, but no dysplasia of the lower limbs, who underwent abdominal ovarian cystectomy. During the surgery, the patient's arterial blood pressure was monitored in her lower limbs by both non-invasive and invasive methods, and almost the same variations of the blood pressure between the invasive and non-invasive measurements were observed. DISCUSSION: Usually, blood pressure measurements are performed in a non-invasive manner in the upper limbs, however, such measurement could not be performed in the present case. There are few reports of measurement of the blood pressure or surgery under anaesthesia in thalidomide-impaired patients, and we report here that it was useful to measure the blood pressure in the lower limbs in the current patient. Invasive arterial pressure measurements showed almost the same changes as the non-invasive pressure measurements, although the systolic blood pressure was 10–20 mmHg lower than the noninvasively measured systolic blood pressure. CONCLUSION: Non-invasive blood pressure measurements in the lower limbs might be useful in thalidomide-impaired patients requiring blood pressure monitoring, but further studies are required to validate this method.

Abstract Background/Aims: The clinical efficacy and safety of a three-drug combination of melphalan, prednisone, and thalidomide were assessed in patients with multiple myeloma who were not candidates for high-dose therapy as a first-line treatment. Because the side effects of thalidomide at a dose of ≥ 100 mg daily can be a barrier to effective treatment for these patients, we evaluated the efficacy and safety of a reduced dose of thalidomide, 50 mg, for non-transplant candidates. Methods: Twenty-one patients were treated in 4-week cycles, receiving 4 mg/m2 melphalan and 40 mg/m2 prednisone on days 1-7 and 50 mg thalidomide daily. The primary efficacy outcome was the overall response rate. Aspirin (100 mg daily) was also provided as prophylactic treatment for thromboembolism. Results: The overall response rate was 57.1%; a complete response was seen in 23.8% of patients, a partial response in 33.3%, and stable disease in 9.5%. After a median follow-up time of 16.1 months, the median time to progression was 11.4 months (95% confidence interval, 2.1 to 20.6); the median overall survival was not reached. Grades 3 and 4 adverse events included infection (10%), peripheral neuropathy (5%), diarrhea (5%), thrombosis (10%), and loss of consciousness (10%). Two patients discontinued treatment due to loss of consciousness and neuropathy. Conclusions: Low-dose thalidomide (50 mg) plus melphalan and prednisone is an effective combination drug therapy option for newly diagnosed myeloma patients who are ineligible for high-dose chemotherapy.

Abstract Studies have shown that the bortezomib-based retreatment of patients with multiple myeloma (MM) may prolong control of the disease. The optimal duration of bortezomib-based retreatment in relapsed or refractory MM is unknown. The present retrospective study evaluated the efficacy and safety of short-course bortezomib-based retreatment in patients who had received bortezomib-thalidomide-dexamethasone (VTD) treatment for the initial therapy of newly diagnosed MM. The clinical records of 20 patients who had received short-course bortezomib-based retreatment in a single center were reviewed. Patients received a median of two cycles of bortezomib as the retreatment and the overall response rate was 90%. Six (30%), eight (40%) and four (20%) patients achieved a complete response (CR), a very good partial response and a partial response, respectively. Of the 10 patients who had achieved a CR during the initial VTD treatment, six experienced a repeat CR during the retreatment. The median duration of the response was nine months and the median time to progression was 10.5 months. The most common grade I and II adverse events were thrombocytopenia and neutropenia. The short-course bortezomib-based retreatment was well tolerated and the favorable response rates observed suggest that it may be an effective and convenient treatment option for certain patients, particularly elderly patients.

Due to its anti-inflammatory, anti-angiogenic, and immunomodulatory effects, thalidomide is increasingly used in the treatment of various diseases. Previous studies have demonstrated that thalidomide may cause sinusal bradycardia. Thalidomide-associated complete atrioventricular block has been reported in only a single case, but there is no case of second-degree atrioventricular block associated with thalidomide in the literature. Here, we present the case of a 50-year-old patient who had been using thalidomide for multiple myeloma. The patient was admitted to the emergency service with syncope, and had second-degree (2:1) atrioventricular block. The patient was using thalidomide (100 mg/day) for the previous 5 months and had not had any abnormal findings in previous electrocar-diographies. His heart rate on presentation was variable, from 45 to 75 bpm. He was treated with a temporary pacemaker because of hypotension and syncope. Five days after the completion of the thalidomide treatment, the patient’s heart rate was 80 bpm. The following day, the pacemaker was removed and a Holter ECG did not show any abnormalities. Patients undergoing thalidomide treatment should be followed up for cardiac and electrocardiographic parameters.

Abstract BACKGROUND:: The aim of the study was to assess the effects of combination of bortezomib moderate dose and continuous oral low dose melphalan and thalidomide and dexamethasone (BMTD regimen) in elderly patients aged ≥ 65 years with relapsed multiple myeloma (MM). METHODS:: Twenty four patients with advanced MM were enrolled to receive eight 3-week treatment cycles with bortezomib 1.3 mg/m2 on days 1, 4, 8, and 11 followed by three 5-week cycles with bortezomib 1.3 mg/m2 on days 1, 8, 15, and 22. Within all cycles, dexamethasone 24 mg/d was given intravenously on the day of bortezomib injection and the day thereafter. In addition, patients received oral treatment of melphalan at a dose of 5 mg/d continuously for twenty days for every cycle. RESULTS:: Mean age of study patients was 72.8 ± 6.4 years. All patients that completed at least one treatment cycle were evaluated for response. Complete, partial, and minor responses occurred in 19%, 65% and 6% of patients, respectively. Overall response rate was 90% (efficacy analysis). CONCLUSIONS:: This study demonstrated potent in vivo activity of combination therapy with BMTD regimen in patients with relapsed MM, with an acceptable safety profile and high overall response rate.

The purpose of this award is to evaluate the: 1) Safety and toxicity and neo-adjuvant thalidomide therapy prior to radical prostatectomy in patients with locally advanced prostate carcinoma (PCa); 2) Efficacy of neoadjuvant thalidomide treatment, as measured by the rate of tumor reduction/PSA decline; 3) Qualitative measurements of the in vivo effect of thalidomide therapy on the Endothelial and Epithelial compartment. Significance: The ability to assess in vivo the effects of thalidomide as well as identify surrogate markers of anti-angiogenic activity is invaluable to the design of new effective therapies. Prior to the DODs final decision (April 2002) to sponsor the correlative studies proposed in the grant 18 patients had been entered in this study. Their clinical information I attached. Pending some additional HSRRB required amendments, the patients' research tissue was stored but the funds awarded were not used to perform any of the studies until we obtained the final DOD approval. After April 2002 (when DOD was cited as a partial sponsor) and until we solved all the remaining issues and obtained DOD clearance to proceed (912003) we held accrual in this trial. We have re-activated the study and are confident that we will complete patient accrual within I year. We are requesting a no-cost extension of I year to allow completion of the planned studies.

Estimates are that the drug caused thousands of deformities and an unknown number of stillbirths, until it was taken off the market at the end of 1961 when the links became clear.
