1DTIC ADA327615: Project DEPRAD (Deployable Radiology And Teleradiology System) In Bosnia/Hungary.

As part of the NATO implementation forces, peace keeping mission, 20,000 U.S. troops were deployed to Bosnia-Herzegovina. The U.S. Army. chose to deploy telemedicine and teleradiology in support of the deployed personnel. This provided state-of-the-art diagnostic services to the troops, along with real-time immediate diagnosis of the radiological images by a remote Radiologist. The Deployable Radiology(DEPRAD)network was designed by personnel at the Imaging Science and Information Systems(ISIS)Center at Georgetown University Medical center to provide rapid access to radiological images acquired in Bosnia. In less than 8 weeks, a complete system was designed, developed and deployed utilizing 11 vendors equipment and their standard off the shelf components. Wide area communications were designed and coordinated by the U.S. Army Medical Research and Materiel Command. The Defense Information Systems Agency (DISA) and Department of Defense, Health Affairs, contracted commercial satellite support and terrestrial communication lines to support the project in Bosnia and Hungary. The success of the telemedicine network prompted the U.S. Army to consider and arrange for deployment of similar technologies in other parts of the world where troops are deployed.

2DTIC ADA409610: Asynchronous Transfer Mode, Computer Aided Detection: Teleradiology

A fiber-optic network derived by a set of ATM (asynchronous transfer mode) switches in association with CAD (computer aided diagnosis/detection) algorithms has been set up as the first prototype of on-line, real-time cancer detection/diagnostic system. Expanding diagnostics and treatment beyond the local level will bring better health care to a wider range of people at a reduced cost. This work describes the Asynchronous Transfer Mode (ATM) Teleradiology Network (ATMTN) and the high-speed fiber backbone architecture as a new technology that offers real-time, on-line , more accurate screening, detection and diagnosis of breast cancer with less need for invasive treatment. ATMTN is a fully automatic, robust and high-speed network integrated with DICOM standards, and IP protocols and associated with Computer Assisted Detection/Diagnostic (CAD) methods for digital mammography.

Abstract Teleradiology is an established mechanism to overcome the lack of on-site radiologists and can benefit children in developing countries. In this “perspective” on teleradiology for pediatric care in underdeveloped countries, three low-cost teleradiology programs are discussed from experiences of one teleradiologist, in relation to previous publications on this subject. Key issues discussed include mechanisms for sustainability, cost-effectiveness, resources, and barriers to success. Reliance on each link of a telereading chain is highlighted as a constant source for concern.

This article is from BMC Health Services Research , volume 14 . Abstract Background: The growing cost of health care and lack of specialised staff have set e-Health high on the European political agenda. In a prospective study we evaluated the effect of providing images for remote consultation through an iPad on the number of in-hospital orthopaedic consultations for children with bone fractures. Methods: Children from 0 to 18 years diagnosed with a bone fracture by the radiologist during the hours when an orthopaedic service is provided only on-call were eligible for enrollment. Cases were enrolled prospectively during September and October 2013. A standard approach (verbal information only, no X-Ray provided remotely) was compared to an experimental approach (standard approach plus the provision of X-ray for remote consultation through an iPad). The primary outcome was the number of orthopaedic in-hospital consultations that occurred. Other outcomes included: immediate activation of other services; time needed for decision-making; technical difficulties; quality of images and diagnostic confidence (on a likert scale of 1 to 10). To the editor: The request for diagnostic and image analysis in radiology is growing rapidly all over the world. This is especially due to the lack of enough number of staff and the lack of specialty expertise. Many hospitals have CT scanners but qualified and competent radiologists are few and far between. These problems can be overcome by transferring images to the distantly located radiologists who have specialty expertise. All these work, in which radiologic images are sent to distant locations for comments and diagnosis, is termed teleradiology. Telecommunication systems are used to send the radiological images from one location to another. Interpretation of all noninvasive imaging studies, such as digitized x-rays, CT, MRI, ultrasound, and nuclear medicine studies, can be carried out in such a manner. This paper recommends teleradiology for a new and additional indication: the radiological diagnosis of a new disease, which is COVID-19 pneumonia, because many radiologists do not have enough experience to make a certain diagnosis.

Radiographic studies representative of the spectrum of trauma cases that might be present in an emergency room were selected from actual cases presented to the emergency room at Brooke Army Medical Center (BAMC) in San Antonio, Texas. The studies were then transmitted from a local television studio via satellite back to BAMC and three other Army Hospitals on two separate occasions. The transmitted images were viewed on commercial grade television sets by panels of physicians (radiologists and other specialties) at the receiving hospitals. The panel members were asked to give their diagnostic impressions regarding the specific cases and to comment on the applicability and usefulness of teleradiology. Overall, it was found that an image of diagnostic quality could be transmitted for most, but not all radiographic cases and that the participating physicians felt this approach had great potential for the practice of radiology both in field and fixed AMEDD facilities.

This handbook is the second publication of the SCAR Technology Forum. The first publication, "Understanding PACS," was published in 1992. The purpose of both publications is to provide the reader with a basic understanding of the essential principles associated with these important areas in medical imaging. In addition, the handbooks are designed to serve as resource materials to assist the reader in determining where to go for more detailed information. In today's environment it appears that the move to managed care and regionalized health care is inevitable. Key to the success of these endeavors is the accurate and timely transmission and interpretation of medical information. Teleradiology is a requisite technology to achieve these goals. Teleradiology has already proven to be the first application of PACS to reach fruition. It is hoped that this handbook will serve as an indispensable guide and reference for those presently in or about to enter the exciting world of teleradiology. Each chapter in this publication focuses on the key issues and attempts to provide a high degree of clarity. Chapter 1: "Introduction" addresses the history and development of teleradiology, from initial revelations to present day achievements. Chapter 2: "The Teleradiology Attraction" discusses the practice benefits and economic imperatives encouraging the development of teleradiology. Chapter 3: "Technical Factors in Teleradiology" provides an overview of the capabilities of typical teleradiology systems and serves as an introduction to the specifications. The final chapter, Chapter 4: "General Considerations" details the considerations of interfacing the teleradiology system to other parts of the network, and the cost of acquiring and operating the system. The "Suggested Readings" are chosen for their contemporary applicability and their contribution to understanding the technology, applications and issues involved in teleradiology. Accompanying each reference is a paragraph discussing the relevance of the cited work. The "Glossary" defines terms pertinent to the clinical application and technology of teleradiology components and systems.
