2Injury In China: A Systematic Review Of Injury Surveillance Studies Conducted In Chinese Hospital Emergency Departments.

This article is from BMC Emergency Medicine , volume 11 . Abstract Background: Injuries represent a significant and growing public health concern in China. This Review was conducted to document the characteristics of injured patients presenting to the emergency department of Chinese hospitals and to assess of the nature of information collected and reported in published surveillance studies. Methods: A systematic search of MEDLINE and China Academic Journals supplemented with a hand search of journals was performed. Studies published in the period 1997 to 2007 were included and research published in Chinese was the focus. Search terms included emergency, injury, medical care. Results: Of the 268 studies identified, 13 were injury surveillance studies set in the emergency department. Nine were collaborative studies of which eight were prospective studies. Of the five single centre studies only one was of a prospective design. Transport, falls and industrial injuries were common mechanisms of injury. Study strengths were large patient sample sizes and for the collaborative studies a large number of participating hospitals. There was however limited use of internationally recognised injury classification and severity coding indices. Conclusion: Despite the limited number of studies identified, the scope of each highlights the willingness and the capacity to conduct surveillance studies in the emergency department. This Review highlights the need for the adoption of standardized injury coding indices in the collection and reporting of patient health data. While high level injury surveillance systems focus on population-based priority setting, this Review demonstrates the need to establish an internationally comparable trauma registry that would permit monitoring of the trauma system and would by extension facilitate the optimal care of the injured patient through the development of informed quality assurance programs and the implementation of evidence-based health policy.

During the 1987-88 season the influenza virus again proved its unpredictable character. The dominant virus was an H3N2 subtype of influenza A. The season was unique in that the virus appeared remarkably early, in early November, and continued to cause sharp outbreaks in some segments of the civilian community well into March 1988. The virus showed considerable antigenic drift away from A/Leningrad/87 and furthermore behaved quite differently when grown in tissue culture or chick embryo. Despite the antigenic drift, the antibody response to the 1987 strains following vaccination with A/Leningrad/86 vaccine was remarkably good in recruits and somewhat less effective in previously vaccinated individuals in the permanent party. These 10 cases were spread over a period of 17 weeks and no more than three in any single week. Also of considerable interest was the demonstration of 21 cases of parainfluenza I infection, of which 19 occurred in students and two in permanent party members. Influenza B did not appear until May when three cases were detected. Keywords: Military medicine.

4The Validation Of A Three-stage Screening Methodology For Detecting Active Convulsive Epilepsy In Population-based Studies In Health And Demographic Surveillance Systems.

This article is from Emerging Themes in Epidemiology , volume 9 . Abstract Background: There are few studies on the epidemiology of epilepsy in large populations in Low and Middle Income Countries (LMIC). Most studies in these regions use two-stage population-based screening surveys, which are time-consuming and costly to implement in large populations required to generate accurate estimates. We examined the sensitivity and specificity of a three-stage cross-sectional screening methodology in detecting active convulsive epilepsy (ACE), which can be embedded within on-going census of demographic surveillance systems.We validated a three-stage cross-sectional screening methodology on a randomly selected sample of participants of a three-stage prevalence survey of epilepsy. Diagnosis of ACE by an experienced clinician was used as ‘gold standard’. We further compared the expenditure of this method with the standard two-stage methodology. Results: We screened 4442 subjects in the validation and identified 35 cases of ACE. Of these, 18 were identified as false negatives, most of whom (15/18) were missed in the first stage and a few (3/18) in the second stage of the three-stage screening. Overall, this methodology had a sensitivity of 48.6% and a specificity of 100%. It was 37% cheaper than a two-stage survey. Conclusion: This was the first study to evaluate the performance of a multi-stage screening methodology used to detect epilepsy in demographic surveillance sites. This method had poor sensitivity attributed mainly to stigma-related non-response in the first stage. This method needs to take into consideration the poor sensitivity and the savings in expenditure and time as well as validation in target populations. Our findings suggest the need for continued efforts to develop and improve case-ascertainment methods in population-based epidemiological studies of epilepsy in LMIC.

7The Use Of The Food And Nutrition Surveillance System (SISVAN Web) In Epidemiological Studies On The Dietary And Nutritional Profile Of The Brazilian Population: Scoping Review Protocol

Abstract Objective: To describe the methods for developing a scoping review on the use of information from the Food and Nutritional Surveillance System (SISVAN Web) in scientific production on the dietary and nutritional profile of the Brazilian population assisted in the Brazilian Unified Health System (SUS). Introduction: SISVAN monitors the population's food and nutrition conditions, integrating itself with health surveillance. Food and Nutritional Surveillance (VAN) is a field of activity of the SUS and was incorporated into the National Food and Nutrition Policy (PNAN, 1999 and 2011), which emphasizes the importance of VAN to individually and collectively diagnose the food and nutritional situation, identify risk and protective factors and improve the management and organization of nutritional care in the SUS. The practice of VAN is crucial to combat malnutrition and ensure Food and Nutritional Security. Methods: This protocol was based on the Joanna Briggs Institute manual, which incorporated the recommendations of the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR). The design of the research questions, objectives and eligibility criteria was based on the population-concept-context model. Databases relevant to the research subject will be consulted and the selection of studies will be conducted by two independent researchers following standardized steps. Primary studies, literature reviews and gray literature that produced evidence based on quantitative information from SISVAN Web will be included. Conclusion: The protocol should guide the conduct of a scoping review that produces useful and reliable results, in a transparent manner. 8DTIC ADA591392: Multiple Hypothesis Tracking (MHT) For Space Surveillance: Results And Simulation Studies

The need to accurately track breakups and satellite clusters, as well as to resolve uncorrelated tracks (UCTs), especially uncorrelated optical observations (UCOs), requires new, robust, and autonomous methods for space surveillance to enable the development and maintenance of the space catalog and to support the over- all SSA mission. This paper presents results on the performance of a newly-developed, statistically-robust, system-level, multiple hypothesis tracking (MHT) capability for joint catalog maintenance, UCT/UCO resolution, and initial orbit determination, in multiple regimes of space. A recent companion paper described some of the unique components contained within the authors' implementation of MHT, including the multi-frame data association problem and the statistical framework used for scoring the likelihood that a sequence of measurements, UCTs/UCOs, and other reports emanate from a common object (either newly-discovered or already catalogued). As demonstrated by the results in this paper, the MHT system provides superior tracking performance (compared to existing methods for data association) in realistic multi-sensor multi- object tracking scenarios over multiple regimes of space. Specifically, we demonstrate that the prototype MHT system can accurately and efficiently process tens of thousands of UCTs/UCOs emanating from thousands of space objects in real-time on a single processor.

Virtual environments are being used more frequently by researchers to collect human performance data before and in lieu of costly field experiments. The McKenna Military Operations in Urban Terrain (MOUT) site at Fort Benning, Georgia, is used for training and experimentation with Soldiers using new equipment and tactics. A low-cost reusable virtual representation of the McKenna MOUT site was built with the use of a popular first-person shooter PC game, Tom Clancy's Rainbow Six 3: Raven Shield, which focuses on small unit tactics in complex urban environments. The focus of this report is the process of building a new, cost-efficient, high-fidelity custom environment complete with accurate representations of buildings, terrain, vegetation, and combinations of enemy units and objectives to model real training situations.

Virtual environments are being used more frequently by researchers to collect human performance data before and in lieu of costly field experiments. The McKenna Military Operations in Urban Terrain (MOUT) site at Fort Benning, Georgia, is used for training and experimentation with Soldiers using new equipment and tactics. A low-cost reusable virtual representation of the McKenna MOUT site was built with the use of a popular first-person shooter PC game, Tom Clancy's Rainbow Six 3: Raven Shield, which focuses on small unit tactics in complex urban environments. The focus of this report is the process of building a new, cost-efficient, high-fidelity custom environment complete with accurate representations of buildings, terrain, vegetation, and combinations of enemy units and objectives to model real training situations.

Scale magnification and transformation errors are considered in the application of stereographic projection to the planar representation of target latitude and longitude relative to the reference ellipsoid in a mulitiple radar surveillance system. The representation is accomplished by a two-stage procedure. First, there is a conversion of target altitude together with target azimuth and slant range relative to a radar into a point on a plane unique to the radar. This is followed by a transformation that carries points on the local radar plane into points on a single master plane. Transformation error is viewed as the separation of images on the master plane of the same point in the local radar plane under an ideal mapping formula and polynomial approximations thereof. Tight upper and lower bounds are derived for the errors generated by these approximations under parameter constraints consistent with the structure of practical coverage regions. The transformation process is intimately connected with the factor by which the length of an infinitesimal arc on the reference ellipsoid is magnified under sterographic projection on the master plane. Minimax procedures for maintaining this magnification factor close to unity are considered. These procedures provide an upper bound on the deviation of the magnification factor from unity over the coverage region that is dependent on coverage region size. (Author)

In Part I, non-coherent detection of narrowband signals in Gaussian- Gaussian mixture noise is considered. The forms of the optimum detectors are found and evaluated for single and multiple observations. These detectors are sensitive to information on the signal and noise parameters. Also evaluated are several suboptimum detectors which perform well under certain conditions without requiring knowledge of signal and noise parameters. In Part II, a new technique for detecting signals using arrays of sensors is investigated, based on the principles of canonical correlation. The theoretical results indicate that for a sufficient number of sensors, multiple, spatially separate signals, at the same frequency can be individually detected without knowledge of sensor position or using conventional beamforming. If sensor positions are known the algorithm supplies the direction of the signal arrivals. The limited numerical results show that a few sensors can automatically steer on and detect a single signal, but are incapable of resolving multiple signals successfully. Surveillance has continued on various samples of aluminum hydride including: unstabilized macrocrystalline, magnesium-doped, and magnesium doped, DPA (diphenylacetylene) treated AlH3-1451. These samples are providing valuable decomposition rate data both neat at -15, 25 and 40 C., and in propellant at 25, 40 and 60 C. Results of surveillance of neat aluminum hydride continue to indicate greater improvement in 25 C. stability than was observed with these samples at 60 C. Two samples which reached 1% decomposition in 18.8 and 25.5 days at 60 C. have decomposed only 0.05 and 0.019% respectively after 1 year at 25 C. Results of surveillance of aluminum hydride propellant are continuing to show decreasing gas generation rates with time. At 25 C., double base propellant containing unstabilized microcrystalline hydride decomposed approximately 0.28% and 0.09% during the first and second year respectively and samples containing stabilized aluminum hydride, decomposed only approximately 0. 10% during the first year. Small amounts of water in the environment which the aluminum hydride is subjected to have been found to prevent the typical autocatalytic shaped decomposition curve. Instead, an initial gassing period is observed followed by excellent stability. Treatments utilizing small amounts of hydrolysis to improve stability are being developed.

The last annual report covering the period through 30 June 1986 left unanswered the question of why the vaccine apparently had been less effective during the Influenza B outbreak than in previous years. Studies were completed during the summer months which showed that when the sera of vaccinated individuals were tested against strains isolated from the Lowry outbreak or against B/Ann Arbor/86 virus, the protective level of antibody was clearly demonstrated to be 32. This was not made clear when sera were tested with ether split B/USSR/83. The 1986-87 season was notable for a sharp outbreak of H1N1 Influenza A, which began before the Christmas break, probably reached a peak during the Christmas holiday, and tailed off during January, with the last case detected during mid-February. Again it was found that the H1N1 virus caused very little illness in individuals over the age of 35. Attack rates in students were difficult to estimate because the outbreak appeared to be peaking at the time the students took a two-week Christmas break. In the permanent party, which for the most part remained on the Base over the Christmas holiday, attack rates were relatively high in the younger segment of the population. No Influenza B viruses were isolated during this season. Influenza A of the H3N2 subtype was isolated from two civilians during this period but none were found at Lowry Air Force base. In the civilian population the virus appeared to spread very poorly, in each case causing a cluster of only two or three other identified illnesses.

The purpose of this contract was to study methods for improving images of space objects obtained with earth-based telescopes. The results of the four contractual tasks are presented. With regard to Task I, recommendations are given for the best intensifier/SIT combinations and relay optics for the Cloudcroft satellite imaging program. As discussed in the section on Task II, it has been concluded that the wavefront-determination-correction method of image restoration is probable not feasible if only linear, postdetection inverse- filtering techniques are used. A discussion of the telescope calibration instrumentation is also included. With regard to Task III, information is listed that allows the optimum film/f-number combination to be determined for use with the f/3 focal reducer. The design and operation of the f/3 focal reducer are discussed in the section on Task IV.

This report presents a surveillance radar model to support the ongoing multi-sensor data fusion performance evaluation study for potential application to the Canadian Patrol Frigate midlife upgrade. This surveillance radar model takes into account the sensor's design parameters and external environmental effects such es clutter, propagation and jamming. To a large extent, the model is based on the available literature and on relevant experience and recent studies in the field. In addition, the latest findings regarding the dominant perturbing effects affecting the radar detection performance are included. The radar model can be used to generate contacts and false alarms in scenarios for multi-sensor data fusion studies while the scenario is running.

Conversion error is considered in the application of stereographic projection to the planar representation of targets in multiple radar surveillance systems. The error is treated as the separation of images in a local radar plane of target position (in terms of altitude, azimuth, and slant range) under ideal mapping formulae and system implementations thereof. These images are viewed as elements of a complex plane in which the center of coordinates represents the radar site. Thus, conversion error can be expressed in terms of a range error equal to the difference in moduli of two complex numbers and an angle error equivalent to the difference between the arguments of the same numbers. As the image of target position under ideal conversion traverses a circle centered on the origin of coordinates, both the angle error and the range error oscillate about median values. The amplitude of each oscillation increases with the radius of the circle. In addition, the median range error is strongly dependent upon the radius of the circle. On the other hand, the median angle error is identically zero. An error correction method and a commonly used minimax technique are considered as alternative means for controlling conversion error.

This is the first annual assessment of longitudinal injury studies and surveillance conducted by the Injury Prevention Division (IPD), U.S. Army Public Health Center (APHC) and the U.S. Army Medical Command (MEDCOM) in support of the U.S. Armys implementation plan for gender integration.
