Abstract The authors wish to report a case of bilateral acute anterior uveitis and optic disc edema following a hemotoxic snake bite, in order to highlight the concomitant occurrence of these conditions and the potential adverse effects of anti-snake venom (ASV). A 35-year-old male was bitten by a viper at seventeen thirty hours, and was started on ASV. Two days following treatment he experienced sudden onset redness and painful diminution of vision in both eyes (OU). On examination, the patient's visual acuity (VA) in OU was 20/200. Examination revealed fresh keratic precipitates, cells, and flare in the anterior chamber (AC), posterior synechiae, sluggish and ill-sustained pupillary reaction, and hyperemic, edematous disc with blurred margins in OU. He was started on topical steroids, cycloplegics and intravenous methylprednisolone. Following treatment, the patient showed improvement and was continued on topical medications and oral prednisolone tapered over 3 weeks, after which VA OU improved, the AC showed no cells and flare and disc edema resolved. Uveitis and optic disc edema in snake bite can either be due to the direct toxic effects of the venom or the effect of ASV. Steroids have a beneficial role in the management of these symptoms.

This article is from North American Journal of Medical Sciences , volume 5 . Abstract Background:: Snake bite envenomation is a major public health concern in developing countries. Acute kidney injury (AKI) is as important cause of mortality in patients with vasculotoxic snake bite. Aims:: This study was to evaluate the clinical profile of snake bite patients and to determine the predictors of developing AKI following snake bite. Materials and Methods:: Two hundred and eighty-one patients with snake envenomation were included. Eighty-seven patients developed AKI (Group A) and 194 (Group B) did not. History, examination findings and investigations results were recorded and compared between the two groups. Results:: In group A, 61 (70.11%) patients were male and in group B, 117 (60.30%) patients were male. Out of 281 patients, 232 had cellulitis, 113 had bleeding tendencies, 87 had oliguria, 76 had neuroparalysis, and 23 had hypotension at presentation. After multivariate analysis, bite to hospital time (P = 0.016), hypotension (P = 0.000), albuminuria (P = 0.000), bleeding time (P = 0.000), prothrombin time (P = 0.000), hemoglobin (P = 0.000) and total bilirubin (P = 0.010) were significant independent predictors of AKI. Conclusions:: AKI developed in 30.96% of patients with snake bite, leading to mortality in 39.08% patients. Factors associated with AKI are bite to hospital time, hypotension, albuminuria, prolonged bleeding time, prolonged prothrombin time, low hemoglobin and a high total bilirubin.

This article is from Iranian Journal of Pediatrics , volume 23 . Abstract Objective: A high incidence of snake bite envenomation has been reported from rural India. Due to inadequate epidemiological data, the incidence is underestimated. This study analyses the pattern of snake bite and their management in children in rural areas of Maharashtra, India. To determine the age, mode of presentation, seasonal variation, clinical profile and outcome of patients with snake bite less than 15 years of age. Methods: This study is a retrospective, descriptive study including 162 patients, who presented with history of snake bite. Clinical data about age, sex, clinical manifestations, complications and outcome were obtained from case records and were analyzed. Findings: Out of the 162 patients 98 (60.49%) were males. The bites were vasculotoxic in 147 (90.74%) and neuroparalytic in 15 (9.25%) patients. Mainly bites occurred from July to September with 84 (51.85%) bites. Bites were more common in males in age more than 5 years (89%) with bite marks mainly on lower limbs in 120 (74.04%) patients. Deaths were reported in patients who reported late to the hospital with a mortality rate of 1.85%. Conclusion: Snake bite is a life threatening emergency. The key to minimizing mortality and severe morbidity is aggressive management of the ABC‘s of resuscitation, and timely and judicious administration of adequate dose of anti-venom.

Abstract Introduction: Calcific myonecrosis is a rare condition in which muscle in a limb compartment undergoes necrosis and becomes peripherally calcified with central liquefaction. The patient usually presents with a slowly progressive enlarged mass that sometimes can be misdiagnosed as soft tissue sarcoma. Most of the reported cases showed that the disease occurs often after trauma or compartment syndrome. However, the case of calcific myonecrosis following snake bite is rarely reported. Case presentation: A 66-year-old Thai woman presented with a gradually progressive enlarged mass over a period of 10 years in her left leg. She had a history of untreated compartment syndrome after she was bitten by a snake (Malayan pit viper) in her left leg when she was 14-years old. At presentation, a plain X-ray showed a large soft tissue mass at the anterior compartment of her left leg. A sheet-like mass with an enlarged central cavity combined with peripheral calcification and cortical erosion of her tibia were observed. A biopsy was performed and the result was negative for neoplastic cells. During a 5-year follow-up, the mass progressively enlarged and then became infected and finally broke through the skin. She was treated by excision of the mass and administration of antibiotics. The wound completed healed at 1 month postsurgery. There was no wound complication or disease recurrence at 1 year postoperation. Conclusions: The diagnosis of calcific myonecrosis was done by history taking and radiographic interpretation. In an asymptomatic patient the management should be observation and clinical follow-up. A biopsy should be avoided due to the high rate of postoperative infection. Treatment of choice in a symptomatic condition is mass excision.

In developed countries the most common chronic wound are leg ulcers. Ulcer can be defined as ”˜’a break in the epithelial continuity’’1 a chronic ulcer unresponsive to dressing and simple treatment, should be biopsied to rule out neo plastic change. Surgical treatment is only indicated if non operative treatment has failed or if the patient suffers from intractable pain. Wound healing is a mechanism whereby the body attempts to restore the integrity of the injured part delayed healing may result in loss of function or poor cosmetic outcome. Here we are reporting a case of snake bite ulcer in a 37 yr male patient since 1999.This case was treated in IPD with only Ayurvedic preparations followed by regular cleaning and dressing with normal saline and prapaundrikadi ghrita for approx two months. 2 months procedure resulted in complete wound healing and restored ability to perform daily routine. Prapaundrikadi Ghrita in this case resulted in enhancing wound healing in this patient. Here we report a case that the 44-year-old female patient bitten by snakes CRPS type 1 was treated consecutive intravenous regional block, lumbar sympathectomy and antiepileptic drug therapy, also discuss the possible pathophysiology.

3Ramona

Ramona, a novel written by Helen Hunt Jackson (1884), is the story of a part-Scottish and part-Native American orphan girl growing up and getting married in Southern California, suffering racial discrimination and hardship. Originally serialized in the Christian Union on a weekly basis, the novel became immensely popular. Overall, it has had more than 300 printings, been made into four film versions, and has been performed as an outdoor play annually since 1923. The impact the novel had on the culture and image of Southern California was enormous. Its romanticization of Mexican colonial life gave the region a unique cultural identity and its publication coincided with the arrival of railroad lines to the region, bringing in countless tourists who wanted to see the locations in the novel. Volume One of the New and Complete Newgate Calendar, covering the years 1700 through 1723, relates stories of British trials, the persons accused and the crimes committed. It is primarily based upon contemporary newspaper and pamphlet accounts of these trials. Set in Old California in the wake of the Mexican-American War, Ramona is two stories at once. It is the story of the love between a part-Native American orphan girl, Ramona, and Alessandro, a young Indian sheepherder. It is also the story of racial prejudice and the clash between cultures as California changes from a Spanish colony to an American territory. Ramona is the ward of Señora Gonzaga Moreno, who despises the girl for her race but honors the dying wish of the Señora's sister, Ramona's foster-mother, to raise her as her own. Señora Moreno embodies the aloof arrogance of Spanish nobility, hating both the Americans who dispute her claim to her vast rancho, and the Indians, whom she places in the same social class with slaves. Her only semblance of love is reserved for her son Felipe.<br><br> Despite the Señora's machinations, Ramona and Alessandro fall in love, and eventually elope. But their life together is not an easy one, as they roam the Southern California searching for a home. He was born to a slave woman in Virginia, who then took him as a young boy to West Virginia after the emancipation.
