5NASA Technical Reports Server (NTRS) 20060048266: The Making Of A Self-Neglect Severity Scale

Research in elder self-neglect has lagged behind that of other forms of mistreatment, despite the fact that self-neglect is the most common allegation reported to Adult Protective Service agencies throughout the US. The lack of a gold-standard to measure self-neglect has hampered efforts to study this phenomenon. Researchers designed the Self-neglect Severity Scale (SSS) based on interviews with Adult Protective Service workers and a national expert panel. The SSS is based on observation and interview and is administered in the home to include an environmental assessment. It was piloted, extensively field tested and then revised. The CREST SSS was developed using survey data and consultation with experts in the field. This instrument utilizes observer ratings, interview responses, and assesses subjects physical and environmental domains. It also assesses functional status as it relates to health and safety issues. After field and pilot testing the SSS was finalized and is currently undergoing reliability and validity testing. The CREST SSS was developed as a state scale to provide a common language for describing cases of self-neglect. It is the first self-neglect severity scale available to researchers. If found to be both reliable and valid it can be used in future intervention studies.

7NASA Technical Reports Server (NTRS) 20110023880: The First Intervention Study In Elder Self-Neglect: A Randomized Clinical Trial To Improve Vitamin D Levels

Despite high mortality rates, elder self-neglect is characterized by refusal of medical and social interventions. To date there have been no tested clinical interventions in elders who self-neglect. Previous research from the TEAM Institute has shown significantly low vitamin D levels in this population. This study aimed to determine the feasibility of a clinical intervention. Replacement of vitamin D was chosen because of its ease of administration and favorable safety profile. Methods: A randomized clinical trial using directly observed therapy of vitamin D was conducted using 50 elders, >65 years of age, with Adult Protective Services (APS) validated self-neglect. A staggered intervention with waiting controls was used to maximize statistical power. One-third (n=17) of the group was administered 50,000 IU vitamin D2 (ergocalciferol) monthly and the remainder (n=33) were administered 400 IU monthly. Serum 25-OH vitamin D was assessed at baseline and 5-months. Results: 69% agreed to participate in the study and of those n=40 (80%) remained at 5-months. At baseline, 12% (n=7) were deficient in vitamin D (<30nmol/L) and approximately 38% (n=22) had inadequate vitamin D levels (<50nmol/L). The baseline 25-OH vitamin D level was 59 nmol/L +25 (mean SD), and increased significantly to 72nmol/L +21 nmol/L at 5-months. Conclusion: These data are the first to provide evidence that clinical interventions are feasible in elders who self-neglect. The increase in vitamin D levels confirmed that the study personnel were able to successfully intervene community-dwelling elders with self-neglect. This study sets the precedent for future intervention and prevention studies

Irish-born author Fitz-James O'Brien is a underrated genius of weird fiction, serving as the missing link between Edgar Allan Poe and later purveyors of the form like Algernon Blackwood, Ambrose Bierce and Arthur Machen. This collection contains several of O'Brien's most well-known weird tales including "The Diamond Lens" (a seance, a murder and a world beneath a microscope!) and "What Was It?" (an invisible murderous creature that predates Wells' The Invisible Man and Ambrose Bierce's "The Damned Thing"), both stories taking a scientific approach to the fantastical and supernatural. 7Liberation: Marines in the Recapture of Guam

Guam became a territorial possession of the United States with the signing of the Treaty of Paris in 1899, ending the Spanish-American War. Earlier, on 21 June 1898, First Lieutenant John Twiggs “Handsome Jack” Myers had led a party of Marines ashore from the protected cruiser Charleston to accept the surrender of the Spanish authorities. Thus began a long Marine presence on Guam. The island, southernmost of the Marianas chain, was discovered by Ferdinand Magellan in 1521, but not occupied until 1688 when a small mission was established there by a Spanish priest and soldiers. When control of the rest of the Mariana Islands was given to Japan as a mandate power in 1919, Guam became an isolated and highly vulnerable American outpost in a Japanese sea. This American territory, 35 miles long, nine miles at its widest and four at its narrowest, shaped like a peanut fell quickly and easily in the early morning of 10 December 1941. With the opening of a two-hour, ever-increasing bombardment by six battleships, nine cruisers, a host of destroyers and rocket ships, laying their wrath on the wrinkled black hills, rice paddies, cliffs, and caves that faced the attacking fleet on the west side of the island, liberation day for Guam began at 0530, 21 July 1944. - Summary by Cyril J.
