„Old Mortality” (Lipsk, 1846) - powieść historyczna Sir Waltera Scotta (1771-1832), opublikowana po raz pierwszy w 1816 roku. Akcja powieści rozgrywa się w południowo-zachodniej Szkocji w latach 1679 - 1689 i przedstawia klimat polityczny i religijny tamtych czasów oczami purytanina Henry'ego Mortona z Milnwood. Tytuł Old Mortality jest wynikiem błędu drukarza. Pierwotnie Scott chciał zatytułować powieść The Tale of Old Mortality. W istocie książka nie jest poświęcona starcowi o pseudonimie „Old Mortality” lecz relacjonuje historię, która miała być przez niego opowiedziana. W prologu autor opisuje rozmowę na opuszczonym cmentarzu pomiędzy Old Mortality'm a Peterem Pattysonem, fikcyjnym redaktorem książki. Old Mortality istniał faktycznie. Nazywał się Robert Paterson (1715-1801). Był kamieniarzem i spędził ostatnie 40 lat swojego życia podróżując po Szkocji, aby odnawiać inskrypcje na grobach prezbiteriańskich męczenników z XVII wieku. Scott spotkał go w 1793 roku na cmentarzu Dunnottar. Autor był szkockim powieściopisarzem i poetą. Wychowywał się i studiował prawo na Uniwersytecie w Edynburgu. Został adwokatem i sądowym urzędnikiem. Interesował się folklorem i historią Szkocji. Był pionierem romantyzmu nie tylko angielskiego, ale i europejskiego. Miał znaczący wpływ na rozwój literatury oraz zwiększenie literackiego prestiżu swojego kraju - Szkocji. Ożywił szkocką balladową poezję ludową, pieśni i legendy, przenosząc je na teren literatury uznawanej za artystyczną. W pierwszej fazie swej twórczości, czyli do 1812 roku, Scott uprawiał powieść poetycką. W późniejszym czasie wydał zbiór ballad szkockich, w których zawarł także swoje utwory. Od tego czasu stawał się coraz bardziej popularny. Jego twórczość poruszała temat baśniowo-rycerskich dziejów Szkocji, podejmował się także opisu angielskiego i francuskiego udziału w wyprawach krzyżowych. Pierwszą powieścią była „Waverley” (1814). Kolejne to m.in. „Rob Roy” (1818), „Narzeczona z Lammermooru” (1819), Ivanhoe” (1819), czy „Kenilworth” (1821). Scott pisał także dramaty, biografie i dzieła historyczne. Scott pisząc swoje powieści szczególnie skupiał się na jak wierniejszym odtworzeniu faktów historycznych oraz ukazaniu kolorytu danego regionu. Skupiał się na przedstawieniu fikcyjnych losów bohaterów, osadzając ich dzieje w czasie autentycznych wydarzeń historycznych. Stworzył ten gatunek praktycznie od podstaw. Scott był zwolennikiem odrębności politycznej Szkocji i piewcą jej walk o niepodległość. Przyczynił się do scementowania ustanowionej w 1707 roku unii realnej Szkocji i Anglii, dotąd połączonych jedynie unią personalną. Dzięki niemu Anglicy i Szkoci lepiej ją poznali. Także częściowo dzięki niemu historia stała się głównym przedmiotem zainteresowań naukowych na początku XIX wieku w Anglii i w Europie. Powieść Waltera Scotta wiązała się z wychowawczą, narodową misją i wywarła wpływ na inne literatury. W Polsce była kontynuowana przez Kraszewskiego i Sienkiewicza. Niniejszy egzemplarz powieści został opublikowany jako 97 tom ukazującej się od 1842 roku serii Collection of British Authors, w lipskiej oficynie wydawniczej Bernharda Tauchnitza (1816-1895).

Identification of very old persons at high risk of recurrent CVD is difficult, since traditional risk markers loose predictive value with age. Methods: In a population-based sample of 282 85-year old participants with established CVD from the Leiden 85-plus Study, we studied predictive values of traditional cardiovascular risk markers, a history of major CVD (myocardial infarction, stroke or arterial surgery), and new cardiovascular biomarkers (estimated glomerular filtration rate (MDRD), C-reactive protein (CRP), homocysteine and N-terminal pro B-type natriuretic peptide (NT-proBNP)) regarding 5-year risk of recurrent cardiovascular events and mortality (composite endpoint). Results: During complete 5-year follow-up 157 (56%) participants died. 109 (39%) had a cardiovascular event or died from cardiovascular causes. Individually related to the composite endpoint were: a history of major CVD (HR 1.5 (95%CI 1.03-2.3)), CRP (HR 1.3 (95%CI 1.03-1.5)), homocysteine (HR 1.4 (95%CI 1.2-2.6)) and NT-proBNP (HR 1.7 (95%CI 1.4-2.1)). A prediction model including all traditional risk markers yielded a C-statistic of 0.59 (95%CI 0.52-0.66). Of all five new markers only addition of NT-proBNP improved the C-statistic (0.67 (95%CI 0.61-0.74, p=0.023)). The categoryless net reclassification improvement for NT-proBNP was 39% (p=0.001), for a history of major CVD 27.2% (p=0.03) and for homocysteine 24.7% (p=0.04). Conclusions: Among very old subjects with established CVD, NT-proBNP was the strongest risk marker for cardiovascular events and cardiovascular mortality. When estimating risk in secondary prevention in very old age, use of NT-proBNP should be considered.

However, this suggestion has been questioned due to inconsistent operationalisations of social support among studies and the small number of longitudinal studies available. This study aims to investigate the influence of perceived social support, understood as the emotional component of social support, on cognition and mortality in old age as part of a prospective longitudinal multicentre study in Germany. Methods: A national subsample of 2,367 primary care patients was assessed twice over an observation period of 18 months regarding the influence of social support on cognitive function and mortality. Perceived social support was assessed using the 14-item version of the FSozU, which is a standardised and validated questionnaire of social support. Cognition was tested by the neuropsychological test battery of the Structured Interview for the Diagnosis of Dementia (SIDAM). The influence of perceived support on cognitive change was analysed by multivariate ANCOVA; mortality was analysed by multivariate logistic and cox regression. Results: Sample cognitive change (N = 1,869): Mean age was 82.4 years (SD 3.3) at the beginning of the observation period, 65.9% were female, mean cognition was 49 (SD 4.4) in the SIDAM. Over the observation period cognitive function declined in 47.2% by a mean of 3.4 points. Sample mortality (N = 2,367): Mean age was 82.5 years (SD 3.4), 65.7% were female and 185 patients died during the observation period. Perceived social support showed no longitudinal association with cognitive change (F = 2.235; p = 0.135) and mortality (p = 0.332; CI 0.829-1.743). Conclusions: Perceived social support did not influence cognition and mortality over an 18 months observation period. Further research is needed to describe the complex interactions between components of social support. Longer observation periods are necessary and standardised operationalisations of social support should be applied.

This article is from BMC Geriatrics , volume 12 . Abstract Background: Social support has been suggested to positively influence cognition and mortality in old age. However, this suggestion has been questioned due to inconsistent operationalisations of social support among studies and the small number of longitudinal studies available. This study aims to investigate the influence of perceived social support, understood as the emotional component of social support, on cognition and mortality in old age as part of a prospective longitudinal multicentre study in Germany. Methods: A national subsample of 2,367 primary care patients was assessed twice over an observation period of 18 months regarding the influence of social support on cognitive function and mortality. Perceived social support was assessed using the 14-item version of the FSozU, which is a standardised and validated questionnaire of social support. Cognition was tested by the neuropsychological test battery of the Structured Interview for the Diagnosis of Dementia (SIDAM). The influence of perceived support on cognitive change was analysed by multivariate ANCOVA; mortality was analysed by multivariate logistic and cox regression. Results: Sample cognitive change (N = 1,869): Mean age was 82.4 years (SD 3.3) at the beginning of the observation period, 65.9% were female, mean cognition was 49 (SD 4.4) in the SIDAM. Over the observation period cognitive function declined in 47.2% by a mean of 3.4 points. Sample mortality (N = 2,367): Mean age was 82.5 years (SD 3.4), 65.7% were female and 185 patients died during the observation period. Perceived social support showed no longitudinal association with cognitive change (F = 2.235; p = 0.135) and mortality (p = 0.332; CI 0.829-1.743). Conclusions: Perceived social support did not influence cognition and mortality over an 18 months observation period. However, previous studies using different operationalisations of social support and longer observation periods indicate that such an influence may exist. This influence is rather small and the result of complex interaction mechanisms between different components of social support; the emotional component seems to have no or only a limited effect. Further research is needed to describe the complex interactions between components of social support. ABSTRACT Aims: Our study aims to determine the impact of malnutrition on the quality of life of elderly patients receiving palliative care and identify the factors influencing quality of life. Methods: The study included patients aged 65 and older who were admitted to the Kırıkkale University Medical Faculty Hospital Palliative Care Service between November 2019 and February 2020. Patients with a history of active malignancy, hospitalization within the past 3 months, severe dementia precluding obtaining written consent, and patients under 65 years of age were excluded. Scores obtained from the Mini Nutritional Assessment-Short Form (MNA-SF) were recorded, with scores of 12 or higher considered as normal nutrition. Those with scores below 12 were reevaluated for malnutrition according to the Global Leadership Initiative on Malnutrition (GLIM) criteria, and those meeting any of the phenotypic and etiological criteria were considered malnourished. Malnutrition severity was reclassified according to the GLIM criteria. The 13-item OPQOLbrief questionnaire was used to evaluate their quality of life. The questionnaire was published in Turkish by Çalışkan et al. Each question was rated on a scale of 1-5, with “Strongly Disagree” scored as 1, “Disagree” as 2, “Undecided” as 3, “Agree” as 4, and “Strongly Agree” as 5. Abstract Objective: In a cohort of children less than 5 years old exposed to adult intrathoracic tuberculosis (TB) in 1996–1998, we found 66% increased mortality compared with community controls. In 2005, we implemented isoniazid preventive therapy (IPT) for children exposed to TB at home, and the present study evaluates the effect of this intervention on mortality. Setting: This prospective cohort study was conducted in six suburban areas included in the demographic surveillance system of the Bandim Health Project in Bissau, the capital city of Guinea-Bissau. Participants: All children less than 5 years of age and living in the same house as an adult with intrathoracic TB registered for treatment in the study area between 2005 and 2007 were evaluated for inclusion in the IPT programme. Main outcome measures (end points): The all-cause mortality rate ratio (MRR) between exposed children on IPT, exposed without IPT and unexposed community control children. Results: A total of 1396 children were identified as living in the same houses as 416 adult TB cases; of those, 691 were enrolled in the IPT programme. Compared with community controls, the IPT children had an MRR of 0.30 (95%CI 0.1 to 1.2). The MRR comparing exposed children with and without IPT was 0.21 (0.0 to 1.1). The relative mortality in IPT children compared with community controls in 2005–2008 differed significantly from the relative mortality of exposed untreated children compared with the community controls in 1996–1998 (test of interaction, p=0.01). Conclusions: In 2005–2008, exposed children on IPT had 70% lower mortality than the community control children, though not significantly. Relative to the community control children, the mortality among TB-exposed children on IPT in 2005–2008 was significantly lower than the mortality among TB-exposed children not on IPT in 1996–1998.

Abstract Little is known about the association between total cholesterol (TC) and all-cause mortality in the elderly (especially the very elderly). Here we examined the association between TC and all-cause mortality in 207 very elderly (85-year-old) participants. In 2003, we performed a baseline laboratory blood examination, and blood pressure (BP) and body mass index (BMI) measurements, and lifestyle questionnaires were completed by the participants. The participants were followed for the subsequent 10 years. As of 2013, of the 207 participants in 2003, 70 participants had survived, 120 individuals had died, and 17 were lost to follow up. The TC values were divided into high-TC (≥209 mg/dL), intermediate-TC (176–208 mg/dL), and low-TC (≤175 mg/dL) categories. With the Kaplan–Meier method, we found that both the high-TC and intermediate-TC participants survived longer than the low-TC participants. The men with high TC survived longer than those with low TC, but no corresponding difference was found for the women. A multivariate Cox proportional hazards regression model, with adjustment for gender, smoking, alcohol intake, history of stroke or heart disease, serum albumin concentration, BMI, and systolic BP, revealed that the total mortality in the low-TC group was 1.7-fold higher than that in the high-TC group. Mortality, adjusted for the same factors, decreased 0.9% with each 1 mg/dL increase in the serum TC concentration and decreased 0.8% with each 1 mg/dL increase in the serum (low-density lipoprotein) LDL-cholesterol (LDL-C) concentration. Our results indicate an association between lower serum TC concentrations and increased all-cause mortality in a community-dwelling, very elderly population. Mortality decreased with the increases in both TC and LDL-C concentrations, after adjustment for various confounding factors. These findings suggest that low TC and low LDL-C may be independent predictors of high mortality in the very elderly.
