Background: The new drugs currently used in the treatment of myeloma are available in Africa but inaccessible because of their high cost. That's why treatment of multiple myeloma remains a challenge in our sector. Objective: To evaluate the VACD-Thal protocol in the treatment of myeloma in Côte d'Ivoire. Materials and Methods: It’s was a prospective and descriptive study with duration of two years at the department of clinical hematology in Côte d'Ivoire. We included the patients with multiple myeloma, diagnosed according to the IMWG 2015 criteria, treated and follow with protocol VACD-Thal. The variables analysed were demographic, paraclinical, therapeutic and evolutionary data. Survival curves were plotted using the Kaplan Meier method. Results: We included 33 patients. There were 22 men (66.7%) and 11 women (33.3%). The average age was 60.5 years with extremes of 42 and 79 years. 24.2% of patients worked in industrial enterprises and 24.24% were exposed to herbicides and pesticides. HIV and arterial hypertension were the comorbidities. The symptoms were dominated by deteriorisation of general state (63.3%), bone pain (48.5%), pathological fractures (18.2%), anemia (93.9%) and hypercalcemia (45.4%). The typical myeloma was predominant (81.8%). Our patients were classified ISS III in 78.8% of case. The overall therapeutic response was observed in 84.8% of cases with a very good partial response in 63.64% of cases and a partial response in 21.2% of cases. Overall survival at 6 months was 87% and 88.5% at one year of survival. Conclusion: Combination of immunomodulator with conventional chemotherapy such as VACD-Thal protocol has done the better results compared to chemotherapy alone.

In other tumor systems, super-enhancers associate with tumor specific oncogenes and dependencies and provide a rationale for super-enhancer mapping to characterize tumor oncogees and dependencies.

Abstract Lichen myxoedematosus (LM), a form of primary cutaneous mucinosis, may present either as localized less severe form called papular mucinosis or diffuse more severe form called scleromyxoedema. The diffuse form is almost always associated with monoclonal gammopathy, whereas localized form is not. We report an atypical case of localized form of LM associated with multiple myeloma in a 66-year-old male, who presented with asymptomatic waxy papular eruption on extremities, which on histopathological examination confirmed the diagnosis of cutaneous mucinosis. After initially being put on steroids and hydroxychloroquine with minimal improvement, patient subsequently presented with encephalopathy and on evaluation revealed hypernatremia, hypercalcemia, hypergammaglobulinemia, reversal of albumin-globulin (A/G) ratio, azotemia, and lytic lesions in skull X-ray. Bone marrow aspiration and biopsy confirmed multiple myeloma. Patient was successfully treated with standard treatment regimen for multiple myeloma with bortezumib and dexamethasone and his skin lesions subsided completely.

Here we report the discoveries that BET bromodomains are required to communicate enhancer mediated pro-inflammatory signal dependent transduction (Brown et al., 2014), that translocations of the IgH enhancer to the MYC locus in MMexpose both enhancer driven and MYC/E2F driven regulatory programs to BET bromodomain inhibition (Fulciniti et al., in preparation), and that master transcription factors cooperate with one another and BET bromodomains to establish oncogenic state in MM and other tumors (Lin et al., Nature in press). Inanticipation of clinical drug resistance to BET bromodomain inhibition, we identified an epigenetic mechanism of resistance in which hyper-phosphorylation of BET bromodomains enables tumor cells to recruit these proteins to chromatin independent of bromodomain activity (Shu et al., Nature in press). These effortshave yielded insights into the dynamic transcriptional control of oncogenic cell state, the development of novel and extendable experimental andcomputational approaches, and therapeutic insights into the consequences of systemic in vivo BET bromodomain inhibition.

This study aims at describing initial clinical and basic laboratory features of multiple myeloma in Douala general hospital. Methods: This was a retrospective cross sectional review of the medical records of all patients diagnosed with multiple myeloma in Douala general hospital between 2007 and 2013. Results: A total of 62 patients were included in the study with an annual rate of 10-11 cases per annum. There was a female predominance with a male female sex ratio of 0.6:1 and a mean age of 57.6±12.2 years. Clinical features at presentation were low back pain (72.6%), and anemia (70.9%), while neurologic impairment and nephropathy were found in 66.2% and 17.5% of the patients respectively. Most (63%) of the patients were diagnosed at stage three of the disease at presented with already established complications.  Conclusion: Multiple myeloma is more common in our context than described in literature presenting in the 5th and 6th decades of life. Most patients present late with already established complications.

Introduction: Serum immunofixation electrophoresis (sIFE) and serum free light chains (sFLC) immunoassays are fundamental for serum monoclonal protein diagnosis and monitoring. In this study, we compared the ability of these assays to detect monoclonal proteins in stem cell transplanted patients. Methods: Three hundred sixty sera belonging to 145 patients affected by MM after autologous stem cell transplantation [113 intact immunoglobulin multiple myeloma (IIMM) and 32 free light chains multiple myeloma (LCMM)] were analyzed by both sIFE and sFLC assay, respectively performed with the Binding Site Minifix Kit and the Binding Site Freelite kit on Cobas-Integra 400 Plus (Roche). Results: Two hundred one samples (55.8%) had an abnormal κ/λ ratio and 32.7% had a monoclonal restricted band in sIFE. For 61.38% samples, there was a concordance between sIFE and sFLC and there wasn’t for 139 samples (p<0.0001). Kappa Cohen’s coefficient was weak (κ= 0.25). When compared to sFLC immunoassay for IIMM, sIFE demonstrated a weak sensitivity of 80.7%, which increases to 94.74% when we compared only LCMM results. Accuracy of sIFE respect to sFLC was the better for IIMM (70.7%), than for all type MM (61.4%), and there was a significant difference between the accuracy of either of these techniques, when performed singly for all MM type, IIMM and LCMM. Conclusion: sFLC assay role in IIMM monitoring is to be confirmed by further studies. sFLC assay is a sensitive tool for the monitoring of patients after transplantation, and that it may be associated to sIFE assay to better individualizing minimal residual disease.

Clonal proliferation of plasma cells within the bone marrow (BM) affects local cells, such as mesenchymal stromal cells(MSCs), leading to osteolysis and fatality in multiple myeloma (MM). Consequently, there is an urgent need to find better mechanisms of inhibiting MM growth and osteolytic lesion development. To meet this need and accelerate clinical translation, better models of MM within the BM are required. We developed a clinically-relevant, 3D myeloma BM co-culture model that mimics bone cell-cancer cell interactions within the bone microenvironment. The co-culture model and clinical samples were utilized to investigate myeloma growth, osteogenesis inhibition, and myeloma-induced abnormalities in MM-MSCs. This platform demonstrated myeloma support of capillary-like assembly of endothelial cells and cell adhesion mediated-drug resistance (CAM-DR). Also, distinct normal donor (ND)- and MM-MSC miRNA signatures were identified and used to uncover osteogenic miRs of interest for osteoblast differentiation. More broadly, our 3D platform provides a simple, clinically-relevant tool to model cancer growth within the bone, useful for investigating skeletal cancer biology, screening compounds, and exploring osteogenesis. Our identification and efficacy validation of novel, bone anabolic miRs in MM opens the floodgate for novel approaches to cancer. We also further developed the OcnCre/iDTR model and discovered a novel bone marrow adipose tissue (BMAT) phenotype. We then explored reasons for this phenotype, and found evidence that sclerostin derived from osteocytes could potentially be one of the osteokines driving BMAT accumulation. We explored the use of metformin to decrease BMAT. Lastly, we found that using an anti-sclerostin antibody, we were able to decrease tumor burden and increase bone, as well as decrease BMAT, suggesting that this antibody could have a 2-pronged attack on the BM in myeloma.

Multiple myeloma is a plasma cell cancer that leads to a dysregulated bone remodeling process. We present a partial differential equation model describing the dynamics of bone remodeling with the presence of myeloma tumor cells. The model explicitly takes into account the roles of osteoclasts, osteoblasts, precursor cells, stromal cells, osteocytes, and tumor cells. Previous models based on ordinary differential equations make the simplifying assumption that the bone and tumor cells are adjacent to each other. However, in actuality, these cell populations are separated by the bone marrow. Our model takes this separation into account by including the diffusion of chemical factors across the marrow, which can be viewed as communication between the tumor and bone. Additionally, this model incorporates the growth of the tumor and the diminishing bone mass by utilizing a ``moving boundary.'' We present numerical simulations that qualitatively validate our model's description of the cell population dynamics.

Clinically, ARN is characterized by anterior uveitis, vitritis, retinal necrosis beginning in the peripheral retina, and occlusive vasculitis involving the retina and choroid [1,2]. Onset is typically unilateral with visual loss, often with ocular or periocular discomfort. Sequential bilateral involvement occurs in up to one-third of cases, usually within 3 months, but may be delayed for several years. The diagnosis of ARN syndrome is based on a clinical examination and a characteristic fundoscopic appearance. Diagnostic vitrectomy or retinal biopsy may be indicated in some atypical cases.

Introduction: Multiple myeloma (MM) is a neoplastic plasma cell disorder characterized by clonal proliferation of malignant plasma cells in the bone marrow. Incidence of MM is increasing rapidly in Asian countries. In a developing country like India diagnosis, risk stratification and access to novel treatment is still a major hurdle Objective: To study the clinico-pathological features of newly diagnosed multiple myeloma patients. Methods: This was a cross sectional hospital based retrospective study conducted in a hospital in eastern India. During the four years of study, 74 cases of multiple myeloma were diagnosed on the basis of International Myeloma Working Group: Criteria for the classification of multiple myeloma and related disorders. The clinical and laboratory characteristics of newly diagnosed patients were studied. Results: Out of the 74 patients, 69% were men and 31% were women. Median age at diagnosis was 55 years (range: 32-81 years). The commonest symptoms observed were fatigue and bone pain. Anemia was present initially in 81% of patients, hypercalcemia in 31.4%, and a serum creatinine level of 2 mg/dL or more in 24.6% patients. The most common monoclonal gammopathy was IgG kappa (44%). Diffuse pattern of bone marrow infiltration, the most common pattern observed in 59% patients. Conclusion: MM patients had male preponderance. Median age of patients was lower compared to western countries with higher percentage of patients with age less than 40 years. Quality of life in MM patients can be improved significantly if there is access to early diagnosis, risk stratification and novel treatment.

Multiple myeloma, a type of plasma cell cancer, is associated with many health challenges, including damage to the kidney by tubulointerstitial fibrosis. We develop an ordinary differential equation (ODE) model which captures the qualitative behavior of the cell populations involved. Specifically, we model the interaction between cells in the proximal tubule of the kidney and free light chains produced by the myeloma monoclonal protein.

Scientific literature indicates a considerable prevalence of osteolytic lesions caused by multiple myeloma in the structures of the head and neck region. The findings reinforce that CBCT can contribute to the early diagnosis of the disease. There seems to be a need for studies with more robust evidence to consolidate this knowledge.

Myeloma is the second most common blood cancer in the U.S. and the most prevalent among African Americans. It is also the blood cancer with the greatest racial disparity in incidence and prevalence; Black Americans are not only at twice the risk of developing this type of cancer when compared to white Americans and other ethnic groups, they are also more likely to be diagnosed at a younger age.My guests are Laura Ortiz-Ravick, Senior Manager of Outreach & Health Promotion at The Leukemia & Lymphoma Society (LLS) and myeloma survivor Kelvin Yates.

Background: Myelomatosis accounts for about 10% of haematological malignancies according to previous studies in different climes. The impact of some complications on treatment choice as well as survival outcomes is significant and worthy of study. Aims: To describe complications observed at presentation in this cohort of myeloma patients and to assess the associations of these complications with certain laboratory and clinical parameters. Results: There were 37 males and 29 females, median age 61 years (Range 37-85). Median values at presentation was Hb - 8g/dL, serum albumin 37 mg/dL and calcium 2.5 mmol/L. Bence Jones proteinuria was present in 50% (17/34), majority (78.8% -26/33) had IgG myeloma and 7 (21.2%) IgA. They had nephropathy 19% (4/21), osteolytic lesions 34.7% (17/49), pathological fractures 56.1% (23/41), anaemia 91.4% (53/58) and hypercalcaemia 42% (21/50). commonest presenting symptom was bone pain in 87.2% (48/55). There was significant correlation between hypercalcaemia and haematocrit (r=0.338, p=0.039) and platelet count (r= 0.331, p= 0.029). No relationship was found between hypercalcaemia and beta -2 microglobulin (r= 0.046, p= 0.0834) or serum immunoglobulin (r= -0.071, p= 0.745). Pathological fractures showed no significant association with haematocrit (r= -0.137, p= 0.453), platelet (r= -0.097, p= 0.642), beta -2 microglobulin (r= -0.037, p= 0.865) and serum immunoglobulin (r= -0.132, p= 0.545). Chi square value for the occurrence of renal Impairment and hypercalcaemia was found to be significant r= 8.048, p= 0.007 as well as pathological fractures (r=8.048, p=0.005). Conclusions: Myeloma is prevalent in males 1.5:1. Bone pain and anaemia are the commonest features at presentation. Bence Jones proteinuria is only seen in about half of the patients and lytic lesions in about a third. Pathological fractures, anaemia or hypercalcaemia at presentation, were not associated with tumor stage (β-2 microglobulin and albumin levels). Significant association exists between the occurrence of renal impairment and pathological fractures.

MP (melphalan and prednisolone) plus thalidomide/lenalidomide, MP plus bortezomib, and lenalidomide/thalidomide plus dexamethasone are used regimens for elderly refractory/relapsed multiple myeloma (R/R MM) patients. However, few clinical trials have compared different regimens directly. Therefore, we conducted a network meta-analysis to provide the evidence of efficacy of different regimens in geriatric patients with R/R MM.

This article is from Journal of Medical Case Reports , volume 6 . Abstract Introduction: Extraskeletal presentation at diagnosis or during the course of multiple myeloma is a rare event. The prognosis is usually very poor. At the moment there is no agreed gold standard for the treatment of this presentation. Case presentation: A 79-year-old Caucasian woman was treated at our hospital for right atrial myeloma localization. Our patient showed the following signs and symptoms of congestive heart failure: dyspnea, hypotension, cyanosis and facial edema. Surgery was not considered feasible due to the extent of the disease. Our patient underwent external-beam radiation therapy using an intensity modulated technique, thus obtaining a persistent complete remission. Our patient has been in continuous complete local remission for 25 months since the end of radiotherapy. Conclusion: The role of radiotherapy is not defined in multiple myeloma with extraskeletal presentation. Our regimen seems to be effective in controlling the disease in this patient.This case report adds to the existing literature as it describes an unusual presentation of the disease and a new therapeutic approach to this rare presentation of multiple myeloma.

However, the application of the approved doses may be difficult in some patients due to adverse effects. Experimental design: Therefore, we evaluated the efficacy and safety of lenalidomide in 10 patients with relapsed and refractory MM who received a reduced dose due to leukopenia (4), polyneuropathy (1), muscle cramps (1), thrombocytopenia (1), renal insufficiency (1), at the request of patient (1), as continuous therapy (1), either from the beginning (2) or during treatment (8). They received lenalidomide at a mean (median) daily dose of 14 (15) mg/d once a day (days 1–21 every 28 days) in combination with dexamethasone at a mean (median) dose of 17.6 (28) mg per day (4–40 mg) on days 1–4, 9–12 and 17–20. Results: Mean (median) duration of treatment with lenalidomide was 15.1 (15) months. Partial response or better was reported in seven and minimal response or better was reported in eight patients. Mean (median) values for time-to-progression (TTP) and for progression-free survival (PFS) were 8.7 (4) months. Mean overall survival (OS) has not been reached, all patients are still alive. Conclusion: In conclusion, dose-reduced lenalidomide is an effective and well tolerated treatment for patients with recurrent or refractory MM who cannot tolerate full doses.
