2Migraine With Aura And Estrogen Containing Birth Control: What's The Data On Risk?
By Dr. Chapa's Clinical Pearls.
Traditionally, migraines with aura were considered contraindicated for estrogen containing birth control. However, that recommendation was made in the 1960s and 1970s when the dose of estrogen in birth control pills was much higher than dosages used today. The risk of stroke is directly related to the amount of estrogen in birth control, and recent clinical opinions have suggested that ultra-low dose combination contraception is allowable. In this podcast, we will review the latest data and clinical opinions regarding the use of estrogen-containing birth control in patients with migraine with aura.

9Maternal Personality Profile Of Children Affected By Migraine.
By Esposito, Maria, Roccella, Michele, Gallai, Beatrice, Parisi, Lucia, Lavano, Serena Marianna, Marotta, Rosa and Carotenuto, Marco
This article is from Neuropsychiatric Disease and Treatment , volume 9 . Abstract Background: Empirical evidence of the important role of the family in primary pediatric headache has grown significantly in the last few years, although the interconnections between the dysfunctional process and the family interaction are still unclear. Even though the role of parenting in childhood migraine is well known, no studies about the personality of parents of migraine children have been conducted. The aim of the present study was to assess, using an objective measure, the personality profile of mothers of children affected by migraine without aura (MoA). Materials and methods: A total of 269 mothers of MoA children (153 male, 116 female, aged between 6 and 12 years; mean 8.93 ± 3.57 years) were compared with the findings obtained from a sample of mothers of 587 healthy children (316 male, 271 female, mean age 8.74 ± 3.57 years) randomly selected from schools in the Campania, Umbria, Calabria, and Sicily regions. Each mother filled out the Minnesota Multiphasic Personality Inventory – second edition (MMPI-2), widely used to diagnose personality and psychological disorders. The t-test was used to compare age and MMPI-2 clinical basic and content scales between mothers of MoA and typical developing children, and Pearson’s correlation test was used to evaluate the relation between MMPI-2 scores of mothers of MoA children and frequency, intensity, and duration of migraine attacks of their children. Results: Mothers of MoA children showed significantly higher scores in the paranoia and social introversion clinical basic subscales, and in the anxiety, obsessiveness, depression, health concerns, bizarre mentation, cynicism, type A, low self-esteem, work interference, and negative treatment indicator clinical content subscales (P < 0.001 for all variables). Moreover, Pearson’s correlation analysis showed a significant relationship between MoA frequency of children and anxiety (r = 0.4903, P = 0.024) and low self-esteem (r = 0.5130, P = 0.017), while the MoA duration of children was related with hypochondriasis (r = 0.6155, P = 0.003), hysteria (r = 0.6235, P = 0.003), paranoia (r = 0.5102, P = 0.018), psychasthenia (r = 0.4806, P = 0.027), schizophrenia (r = 0.4350, P = 0.049), anxiety (r = 0.4332, P = 0.050), and health concerns (r = 0.7039, P < 0.001) MMPI-2 scores of their mothers. Conclusion: This could be considered a preliminary study that indicates the potential value of maternal personality assessment for better comprehension and clinical management of children affected by migraine, though further studies on the other primary headaches are necessary.

14Migraine: The Evolution Of A Common Disorder
By Sacks, Oliver, 1933-2015

19A Review Migraine
By C Andly Chedrick | D N Ashritha | Harini R
Headache disorders are among the most common nervous system ailments, which are characterized by persistent headaches. Headache is a severe and disabling symptom of a few major headache disorders, including migraine, tension type headache, and cluster headache. Migraine is caused by spontaneous over activity and aberrant amplification in pain and other, primarily sensory, pathways in the brainstem. The current consensus is that the cause is predominantly neurological. According to current knowledge, a basic neuronal malfunction causes a series of intracranial and extra cranial alterations that cause migraine, comprising the four phases of premonitory symptoms, aura, headache, and postdrome. The effective diagnosis and treatment may help the patient improve in their quality of life and help to achieve an optimal therapeutic outcome. The scope of this review is to describe Migraine, types, etiology, pathophysiology, diagnosis, pharmacological treatment, non pharmacological treatment, and its prevention. 20Retrospective Chart Review: The Feasibility Of A Self-Administered Nasal Spray Targeting The Sphenopalatine Ganglion (SPG) In Treatment Of Chronic Migraine
By Sarah M Bobker
Objective: To describe the results of a self-administered anesthetic nasal spray developed to target the sphenopalatine ganglion (SPG) for the acute and preventive treatment of chronic migraine. Background: The SPG is a known migraine treatment target that may be anesthetized via minimally invasive in-office procedures. Historically, various noninvasive selfadministered intranasal anesthetic formulations have been developed for the treatment of migraine, with some studies suggesting implication of the SPG mechanism, however, this has needed further development and data on efficacy has been mixed and inconclusive. During COVID-19 pandemic clinic closures, we noticed a substantial need for improved at-home therapies for our university headache center patients with chronic migraine. Methods: We developed a compounded anesthetic nasal spray utilizing upward force and a supine delivery positioning with the aim of better targeting the SPG. Patients were instructed to use this treatment once per month as prevention and, additionally, up to four times per month as need acutely. Retrospective chart review was performed. Physician clinical judgment of patient reports was used to assign a dichotomous conclusion of the usefulness of such treatment for patients at an interval of 3 months (preventive effect) and acutely (acute effect). Adverse effects were also reviewed. Results differentiate treatment response in medication overuse headache (MOH) and peripartum subgroups as well. Results: 52 out of 66 (79%) patients reported improvement in overall headache frequency or intensity at 3 month follow up (preventive effect). 40 out of 53 (75%) patients who using the treatment acutely reported improvement in headache intensity (acute effect). Of the 6 patients also with MOH, 5 (83%) reported preventive effect and 4 (67%) reported acute effect. Of the 9 patients who were also peripartum, all 9 (100%) reported preventive effect and 8 (89%) reported acute effect. No significant adverse effects were reported. Conclusion: A self-administered nasal spray developed to target the SPG may be an effective and safe therapy for the acute and preventive treatment of chronic migraine. This delivery method suggests safe and potentially successful blockade of the SPG at home. Further developments in non-invasive intranasal techniques are warranted.

23Prevalence Of Bruxism In Children With Episodic Migraine - A Case-control Study With Polysomnography.
By Masuko, Alice Hatsue, Villa, Thais Rodrigues, Pradella-Hallinan, Marcia, Moszczynski, Alexander Joseph, de Souza Carvalho, Deusvenir, Tufik, Sergio, do Prado, Gilmar Fernandes and Coelho, Fernando Morgadinho Santos
This article is from BMC Research Notes , volume 7 . Abstract Background: Parents of children with migraine have described a higher prevalence of sleep bruxism and other sleep disturbances in their children. The objective of this study was to use polysomnography to investigate the prevalence of bruxism during sleep in children with episodic migraine relative to controls. Findings: Controls and patients were matched by sex, age, years of formal education, presence of snoring, arousals per hour, and respiratory events per hour.A total of 20 controls, between 6 and 12 years old, with no history of headache, recruited from public schools in Sao Paulo between 2009 and 2012, and 20 patients with episodic migraine recruited from the Headache Clinic at the Federal University of Sao Paulo between 2009 and 2012 underwent polysomnography. No intervention was performed before sleep studies.Among migraine patients, 27.5% experienced aura prior to migraine onset. The sleep efficiency, sleep latency, REM sleep latency, arousals per hour, percentage of sleep stages, and breathing events per hour were similar between groups. Five children (25%) with episodic migraine exhibited bruxism during the sleep study while this finding was not observed in any control (p = 0.045). Conclusions: Our data demonstrate that bruxism during sleep is more prevalent in children with episodic migraine. Further prospective studies will help elucidate the underlying shared pathogenesis between bruxism and episodic migraine in children.

24Migraine And Tension-Type Headache In Children And Adolescents Presenting To Neurology Clinics.
By Tavasoli, Azita, Aghamohammadpoor, Mehran and Taghibeigi, Meygol
This article is from Iranian Journal of Pediatrics , volume 23 . Abstract Objective: Headache is one of the most common neurologic problems in children and adolescents. Primary headache including migraine and tension-type headache comprises the vast majority of headaches and are associated with marked incidence, prevalence, and individual and social cost. We aimed to assess demographic characteristics and to compare some factors related to primary headaches in children/ adolescents presented to neurology clinics of Tabriz University of Medical Sciences. Methods: Children from 4 to 15 years of age with the diagnosis of primary headache (migraine or tension-type headaches) who presented to the neurology clinics affiliated to Tabriz University of Medical Sciences, Tabriz, Iran from March 2009 to October 2011 are included in this cross-sectional study. Data regarding the type of headache, history of atopy, peripartum asphyxia, and breast feeding, family history of headache and the socioeconomic status of the family were collected. The diagnosis was based on the international headache society diagnostic criteria for the primary headache disorders. Findings: One hundred ninety children (107 females) with primary headache (88 patients with migraine and 102 patients with tension type headache) enrolled in the study. Peripartum asphyxia, history of atopy, family history of headache and low socioeconomic status (SES) were more common in patients with migraine (P-values: 0.007, 0.01, 0.001, 0.003; respectively). Conclusion: Physicians need to extent their knowledge regarding the primary headaches. Peripartum asphyxia, history of atopy, headache in parents and low SES have been shown in the present study to be more prevalent in patients with migraine as compared to tension-type headache.

25Linking A Genetic Defect In Migraine To Spreading Depression In A Computational Model.
By Dahlem, Markus A., Schumacher, Julia and Hubel, Niklas
This article is from PeerJ , volume 2 . Abstract Familial hemiplegic migraine (FHM) is a rare subtype of migraine with aura. A mutation causing FHM type 3 (FHM3) has been identified in SCN1A encoding the Nav1.1 Na+ channel. This genetic defect affects the inactivation gate. While the Na+ tail currents following voltage steps are consistent with both hyperexcitability and hypoexcitability, in this computational study, we investigate functional consequences beyond these isolated events. Our extended Hodgkin–Huxley framework establishes a connection between genotype and cellular phenotype, i.e., the pathophysiological dynamics that spans over multiple time scales and is relevant to migraine with aura. In particular, we investigate the dynamical repertoire from normal spiking (milliseconds) to spreading depression and anoxic depolarization (tens of seconds) and show that FHM3 mutations render gray matter tissue more vulnerable to spreading depression despite opposing effects associated with action potential generation. We conclude that the classification in terms of hypoexcitability vs. hyperexcitability is too simple a scheme. Our mathematical analysis provides further basic insight into also previously discussed criticisms against this scheme based on psychophysical and clinical data.

27Migraine Without Aura Is Not Associated With Incomplete Circle Of Willis: A Case-control Study Using High-resolution Magnetic Resonance Angiography.
By Ezzatian-Ahar, Shabnam, Amin, Faisal Mohammad, Obaid, Hayder Ghani, Arngrim, Nanna, Hougaard, Anders, Larsson, Henrik B W and Ashina, Messoud
This article is from The Journal of Headache and Pain , volume 15 . Abstract Background: The circle of Willis is an important source of collateral blood flow to maintain adequate cerebral perfusion, particularly in the posterior circulation. Some studies report a relationship between incomplete circle of Willis and migraine, whereas other studies show no difference between the prevalence of incomplete circle of Willis in migraineurs and controls. In the present study we compared the prevalence of incomplete circle of Willis in female migraine patients without aura to female healthy non-migraine controls.Using 3-Tesla magnetic resonance angiography we recorded three-dimensional time-of-flight angiograms in 85 female participants (48 migraine patients without aura [median age 28 years] and 37 healthy controls [median age 25 years]). The images were subsequently analysed blindly by a neuroradiologist to detect incomplete circle of Willis. Findings: We found no difference between the prevalence of incomplete circle of Willis in patients, 20/47 (43%), and controls, 15/37 (41%), p = 0.252. Post hoc analysis showed a significant relationship between age and prevalence of incomplete circle of Willis, p = 0.003. Conclusion: We found no relationship between migraine without aura and incomplete circle of Willis.

28Effects Of Long-Term Acupuncture Treatment On Resting-State Brain Activity In Migraine Patients: A Randomized Controlled Trial On Active Acupoints And Inactive Acupoints.
By Zhao, Ling, Liu, Jixin, Zhang, Fuwen, Dong, Xilin, Peng, Yulin, Qin, Wei, Wu, Fumei, Li, Ying, Yuan, Kai, von Deneen, Karen M., Gong, Qiyong, Tang, Zili and Liang, Fanrong
This article is from PLoS ONE , volume 9 . Abstract Background: Acupuncture has been commonly used for preventing migraine attacks and relieving pain during a migraine, although there is limited knowledge on the physiological mechanism behind this method. The objectives of this study were to compare the differences in brain activities evoked by active acupoints and inactive acupoints and to investigate the possible correlation between clinical variables and brain responses. Methods and Results: A randomized controlled trial and resting-state functional magnetic resonance imaging (fMRI) were conducted. A total of eighty migraineurs without aura were enrolled to receive either active acupoint acupuncture or inactive acupoint acupuncture treatment for 8 weeks, and twenty patients in each group were randomly selected for the fMRI scan at the end of baseline and at the end of treatment. The neuroimaging data indicated that long-term active acupoint therapy elicited a more extensive and remarkable cerebral response compared with acupuncture at inactive acupoints. Most of the regions were involved in the pain matrix, lateral pain system, medial pain system, default mode network, and cognitive components of pain processing. Correlation analysis showed that the decrease in the visual analogue scale (VAS) was significantly related to the increased average Regional homogeneity (ReHo) values in the anterior cingulate cortex in the two groups. Moreover, the decrease in the VAS was associated with increased average ReHo values in the insula which could be detected in the active acupoint group. Conclusions: Long-term active acupoint therapy and inactive acupoint therapy have different brain activities. We postulate that acupuncture at the active acupoint might have the potential effect of regulating some disease-affected key regions and the pain circuitry for migraine, and promote establishing psychophysical pain homeostasis. 32Failure Of Feedback As A Putative Common Mechanism Of Spreading Depolarizations In Migraine And Stroke
By Markus A. Dahlem, Felix M. Schneider and Eckehard Schoell
The stability of cortical function depends critically on proper regulation. Under conditions of migraine and stroke a breakdown of transmembrane chemical gradients can spread through cortical tissue. A concomitant component of this emergent spatio-temporal pattern is a depolarization of cells detected as slow voltage variations. The velocity of ~3 mm/min indicates a contribution of diffusion. We propose a mechanism for spreading depolarizations (SD) that rests upon a nonlocal or non-instantaneous feedback in a reaction-diffusion system. Depending upon the characteristic space and time scales of the feedback, the propagation of cortical SD can be suppressed by shifting the bifurcation line, which separates the parameter regime of pulse propagation from the regime where a local disturbance dies out. The optimisation of this feedback is elaborated for different control schemes and ranges of control parameters.

34DTIC ADA612867: Novel Therapeutic Targets For Chronic Migraine
By Defense Technical Information Center
Chronic migraine is a disabling disorder that affects millions of individuals worldwide, and may result from traumatic brain injury. The purpose of this study is to use rodent models of basic migraine mechanisms to characterize new potential treatments for chronic migraine. The scope of the research is to investigate multiple novel potential drug treatments on migraine-related brain excitability, pain-sensing mechanisms, and behavior. The major findings of the research in the second year of the project are that the medications memantine causes a small reduction in chronic migraine-related hyperalgesia in mice. Amiloride also prevents the development chronic migraine-related hyperalgesia in mice. These results support the clinical investigation of amiloride and related drugs as treatments for chronic migraine. We have also found that the drug memantine inhibits cortical spreading depression, but acute treatment with memantine does not inhibit nociceptive signaling in the brainstem. These results are consistent with the potential efficacy of memantine as a preventive therapy, but not as an acute therapy for migraine. We have also found that delta opioid receptor agonists inhibit both cortical spreading depression and migraine related pain behaviors, also supporting the clinical investigation of these compounds. Overall, we have made significant progress in the characterization of novel potential therapies for chronic migraine.

35DTIC ADA590494: Mechanism And Therapy For The Shared Susceptibility To Migraine And Epilepsy After Traumatic Brain Injury (TBI)
By Defense Technical Information Center
Our proposal studies the natural history and mechanisms of increased brain excitability leading to migraine and epilepsy after traumatic brain injury. In the 18 months since this grant was funded, we have performed experiments spanning nearly the whole range of our proposed work. The most important milestones in this project year have been: 1. Successful implementation of very challenging post-TBI in vivo two-photon imaging and in vivo whole cell recording. 2. Identification of novel TBI-specific phenotypes from our in vivo whole cell recordings. 3. Resolution of technical difficulties in telemetry recordings, and recording of both acute and chronic datasets. 4. Progressive implementation of histological techniques, with confirmation thus far of expected TBI phenotypes. 5. Implementation of post- TBI pain behavior measurements.

38Methylation Of Migraine-Related Genes In Different Tissues Of The Rat.
By Labruijere, Sieneke, Stolk, Lisette, Verbiest, Michael, de Vries, Rene, Garrelds, Ingrid M., Eilers, Paul H. C., Danser, A. H. Jan, Uitterlinden, Andre G. and MaassenVanDenBrink, Antoinette
This article is from PLoS ONE , volume 9 . Abstract 17ß-Estradiol, an epigenetic modulator, is involved in the increased prevalence of migraine in women. Together with the prophylactic efficacy of valproate, which influences DNA methylation and histone modification, this points to the involvement of epigenetic mechanisms. Epigenetic studies are often performed on leukocytes, but it is unclear to what extent methylation is similar in other tissues. Therefore, we investigated methylation of migraine-related genes that might be epigenetically regulated (CGRP-ergic pathway, estrogen receptors, endothelial NOS, as well as MTHFR) in different migraine-related tissues and compared this to methylation in rat as well as human leukocytes. Further, we studied whether 17ß-estradiol has a prominent role in methylation of these genes. Female rats (n = 35) were ovariectomized or sham-operated and treated with 17β-estradiol or placebo. DNA was isolated and methylation was assessed through bisulphite treatment and mass spectrometry. Human methylation data were obtained using the Illumina 450k genome-wide methylation array in 395 female subjects from a population-based cohort study. We showed that methylation of the Crcp, Calcrl, Esr1 and Nos3 genes is tissue-specific and that methylation in leukocytes was not correlated to that in other tissues. Interestingly, the interindividual variation in methylation differed considerably between genes and tissues. Furthermore we showed that methylation in human leukocytes was similar to that in rat leukocytes in our genes of interest, suggesting that rat may be a good model to study human DNA methylation in tissues that are difficult to obtain. In none of the genes a significant effect of estradiol treatment was observed.

39Pharmacoinformatics Elucidation Of Potential Drug Targets Against Migraine To Target Ion Channel Protein KCNK18.
By Sehgal, Sheikh Arslan, Hassan, Mubashir and Rashid, Sajid
This article is from Drug Design, Development and Therapy , volume 8 . Abstract Migraine, a complex debilitating neurological disorder is strongly associated with potassium channel subfamily K member 18 (KCNK18). Research has emphasized that high levels of KCNK18 may be responsible for improper functioning of neurotransmitters, resulting in neurological disorders like migraine. In the present study, a hybrid approach of molecular docking and virtual screening were followed by pharmacophore identification and structure modeling. Screening was performed using a two-dimensional similarity search against recommended migraine drugs, keeping in view the physicochemical properties of drugs. LigandScout tool was used for exploring pharmacophore properties and designing novel molecules. Here, we report the screening of four novel compounds that have showed maximum binding affinity against KCNK18, obtained through the ZINC database, and Drug and Drug-Like libraries. Docking studies revealed that Asp-46, Ile-324, Ile-44, Gly-118, Leu-338, Val-113, and Phe-41 are critical residues for receptor–ligand interaction. A virtual screening approach coupled with docking energies and druglikeness rules illustrated that ergotamine and PB-414901692 are potential inhibitor compounds for targeting KCNK18. We propose that selected compounds may be more potent than the previously listed drug analogs based on the binding energy values. Further analysis of these inhibitors through site-directed mutagenesis could be helpful for exploring the details of ligand-binding pockets. Overall, the findings of this study may be helpful for designing novel therapeutic targets to cure migraine.

40Galeata: Chronic Migraine Independently Considered In A Medieval Headache Classification.
By Guerrero-Peral, Angel Luis, de Frutos Gonzalez, Virginia and Pedraza-Hueso, Maria Isabel
This article is from The Journal of Headache and Pain , volume 15 . Abstract Background: Chronic migraine is a quite recent concept. However, there are descriptions suggestive of episodic migraine since the beginning of scientific medicine. We aim to review main headache classifications during Classical antiquity and compared them with that proposed in the 11th century by Constantine the African in his Liber Pantegni, one of the most influential texts in medieval medicine. Method: We have carried out a descriptive review of Henricum Petrum's Latin edition, year 1539. Results: Headache classifications proposed by Aretaeus of Cappadocia, Galen of Pergamun and Alexander of Tralles, all of them classifying headaches into three main types, considered an entity (called Heterocrania or Hemicrania), comparable to contemporary episodic migraine. In ninth book of Liber Pantegni, headaches were also classified into three types and one of them, Galeata, consisted on a chronic pain of mild intensity with occasional superimposed exacerbations. Conclusion: In Liber Pantegni we have firstly identified, as a separate entity, a headache comparable to that we currently define as chronic migraine: Galeata.

41Association Of Single Nucleotide Polymorphisms Of CACNA1A Gene In Migraine.
By Bashir, Aadil, Saleem, Shiekh, Wani, Maqbool, Rasool, Roohi, Wani, Irfan Yousuf, Gulnar, Azhara and Verma, Sawan
This article is from Indian Journal of Human Genetics , volume 20 . Abstract INTRODUCTION:: Migraine is a chronic, neurovascular polygenic disease where genetic and environmental factors are involved in its etiology. Dysfunction of neuronal ion transportation can provide a model for predisposition for common forms of migraine. Mutations in genes encoding ion channels disturb the rhythmic function of exposed tissue that may also explain the episodic nature of migraine. Our aim was to study the single nucleotide polymorphisms of CACNA1A gene in migraine patients. MATERIALS AND METHODS:: The subjects were the patients of migraine, in the age range of 18-80 years, diagnosed by a Neurologist, as per the diagnostic criteria of International Headache Society (IHS) Classification 2004 after excluding other causes of headache by clinical examination and relevant investigations. The controls were the age and sex matched healthy persons from the same population excluding the relatives of patients. Only those patients and the controls, who voluntarily participated in the study, were taken and their blood samples were taken for the study. Deoxyribonucleic acid (DNA) extraction was performed according to the manufacturer's protocol for Qiagen DNA extraction kits (Qiagen, Hilden, NRW, Germany). DNA content was quantified by spectrophotometric absorption (Nanodrop Spectrophotometer, BioLab, Scoresby, VIC, Australia). Polymerase chain reaction was performed using an iCycler Thermal Cycler (Bio.Rad, Hercules, CA, USA). The polymorphic analysis of CACNA1A gene was carried out by two methods: Restriction fragment length polymorphism and sequencing. RESULTS:: The study included a total of 25 patients of migraine, diagnosed on out-patient department basis as per IHS Classification 2004 and compared with age and sex matched 25 healthy controls. Most of the patients 23 (92%) were below the age of 50 years. 20 of the patients (80%) were females and 5 (20%) were males. The polymorphic analysis of CACNA1A gene revealed the presence of only the wild form of the gene for the codon E993V in both case and control groups. CONCLUSION:: In our study, we could not find any polymorphism of CACNA1A gene in the selected patients. Instead the wild type of genotype was found in both patients and controls. This negative result presented here, implies that if the CACNA1A gene is involved in typical migraine (with and without aura), its contribution is very modest and therefore difficult to discern. Nevertheless, there are other genes that could be considered potential candidates for typical migraine susceptibility for which further research is needed.

42Epigone Migraine Vertigo (EMV): A Late Migraine Equivalent.
By PAGNINI, P., VANNUCCHI, P., GIANNONI, B. and PECCI, R.
This article is from Acta Otorhinolaryngologica Italica , volume 34 . Abstract Migrainous headache is determined by pathogenetic mechanisms that are also able to affect the peripheral and/or central vestibular system, so that vestibular symptoms may substitute and/or present with headache. We are convinced that there can be many different manifestations of vestibular disorders in migrainous patients, representing true different clinical entities due to their different characteristics and temporal relashionship with headache. Based on such considerations, we proposed a classification of vertigo and other vestibular disorders related to migraine, and believe that a particular variant of migraine-related vertigo should be introduced, namely "epigone migraine vertigo" (EMV): this could be a kind of late migraine equivalent, i.e. a kind of vertigo, migrainous in origin, starting late in the lifetime that substitutes, as an equivalent, pre-existing migraine headache. To clarify this particular clinical picture, we report three illustrative clinical cases among 28 patients collected during an observation period of 13 years (November 1991 - November 2004). For all patients, we collected complete personal clinical history. All patients underwent standard neurotological examination, looking for spontaneous-positional, gaze-evoked and caloric induced nystagmus, using an infrared video camera. We also performed a head shaking test (HST) and an head thrust test (HTT). Ocular motility was tested looking at saccades and smooth pursuit. To exclude other significant neurological pathologies, a brain magnetic resonance imaging (MRI) with gadolinium was performed. During the three months after the first visit, patients were invited to keep a diary noting frequency, intensity and duration of vertigo attacks. After that period, we suggested that they use prophylactic treatment with flunarizine (5 mg per day) and/or acetylsalicylic acid (100 mg per day), or propranolol (40 mg twice a day). All patients were again recommended to note in their diary the frequency and intensity of both headache and vertigo while taking prophylactic therapy. Control visits were programmed after 4, 12 and 24 months of therapy. All patients considerably improved symptoms with therapy: 19 subjects (68%) reported complete disappearance of vestibular symptoms, while 9 (32%) considered symptoms very improved. The subjective judgement was corroborated by data from patients diaries. We conclude that EMV is a clinical variant of typical migraine-related vertigo: a migraineassociated vertigo, headache spell independent, following a headache period, during the lifetime of a patient.

43Ophthalmoplegia Starting With A Headache Circumscribed In A Line-shaped Area: A Subtype Of Ophthalmoplegic Migraine?
By Wang, Yu, Wang, Xian-Hong, Tian, Miao-Miao, Xie, Cheng-Juan, Liu, Ying, Pan, Qing-Qing and Lu, Ya-Nan
This article is from The Journal of Headache and Pain , volume 15 . Abstract Recurrent painful ophthalmoplegic neuropathy (RPON), formerly named ophthalmoplegic migraine (OM), is a rare condition characterized by the association of unilateral headaches and the ipsilateral oculomotor nerve palsy. The third cranial nerve is most commonly involved in the recurrent attacks. But it is still debated whether a migraine or an oculomotor neuropathy may be the primary cause of this disorder. Here, we report an elder patient who had a recurrent ophthalmoplegia starting with an unilateral headache circumscribed in an area shaped in a line linking the posterior-parietal region and the ipsilateral eye. And the headache had couple of features similar to that of migraine, such as past history of recurrent migraine attacks, accompaniments of nausea, vomiting, and phonophobia, response to flunarizine and sodium valproate. We may herein report a subtype of OM but not a RPON. This case report indicates that OM may exist as an entity and some OM may be wrongly grouped under the category of RPON in the current international headache classification.

44Does The Response To Contrast Saturate In Those With Migraine?
By Louise O'Hare, Paul Hibbard and Arnold Wilkins
High contrast stimuli are uncomfortable, and this is thought to be due to excessive metabolic cost due to inefficient neural processing. Those with light sensitivity, such as occurs in photosensitive epilepsy and migraine, tend to be particularly susceptible to visual discomfort, and there is convergent evidence that they have a particularly excitable visual cortex. Increasing the contrast makes a stimulus stronger, and increases demand on metabolic resources. This occurs up to a limit, as the electrical response to contrast saturates in normal observers (Porciatti et al., 2000). However, in those with photosensitive epilepsy, the contrast response does not saturate but continues to increase (Porciatti et al., 2000), which may be the root of the issues. Photosensitive epilepsy and migraine share features in common (Rogowski, 2012). The saturation of the contrast response has not been studied in individuals with migraine, although the response is known to be abnormally large. We propose to measure VEPs in response to striped patterns of increasing contrast to find this plateau both in individuals with migraine and control groups. We plan to test whether coloured filters can help reduce the saturation level in individuals with migraine. fMRI BOLD responses have been shown to decrease in amplitude with the use of coloured filters (Huang et al., 2011). Participants will choose their own filter using a colorimeter, and a control filter will be selected by computer algorithm, to ensure that any effects are not seen as a result of the filters per se, but the specific colour choice.

49Comparison Between Intravenous Sodium Valproate And Subcutaneous Sumatriptan For Treatment Of Acute Migraine Attacks; Double-Blind Randomized Clinical Trial.
By Rahimdel, Abolghasem, Mellat, Ali, Zeinali, Ahmad, Jafari, Elahe and Ayatollahi, Parisa
This article is from Iranian Journal of Medical Sciences , volume 39 . Abstract Background: Sodium valproate (SV) has been approved for migraine prophylaxis and its intravenous form is used to treat acute migraine attacks. We compared the efficacy and safety of intravenous SV and subcutaneous Sumatriptan in managing acute migraine attacks. Methods: This double-blind randomized clinical trial divided 90 patients into two groups: one group received 400 mg of intravenous SV and the second group received 6 mg of subcutaneous Sumatriptan. Headache severity before treatment and half an hour, one hour, and two hours after treatment was measured based on the VNRS in the groups. Associated symptoms, i.e., photophobia, phonophobia, nausea, and vomiting, were assayed on admission and 2 hours after treatment. Side effects of the drugs were checked 2 hours after injection. Obtained data from the groups were compared. Results: In both groups, pain decrement at the mentioned time points was significant (P 0.05), indicating the similar effect of both drugs on pain improvement. In the SV group, photophobia, phonophobia, nausea, and vomiting were improved significantly, while in the Sumatriptan group, only photophobia and vomiting were decreased significantly, indicating the advantage of SV in improving the associated symptoms. Nausea, vomiting, facial paresthesia, and hypotension were more significantly frequent in the Sumatriptan group than in the SV group (P

50Psychotherapy Versus Usual Care In Pediatric Migraine And Tension-type Headache: A Single-blind Controlled Pilot Study.
By Balottin, Umberto, Ferri, Matteo, Racca, Michela, Rossi, Maura, Rossi, Giorgio, Beghi, Ettore, Chiappedi, Matteo and Termine, Cristiano
This article is from Italian Journal of Pediatrics , volume 40 . Abstract Background: Despite growing interest in psychotherapy in child and adolescent headache, efficacy studies in this research field have focused mainly on cognitive-behavioral therapies. Whereas relaxation and cognitive-behavioral techniques, in particular, have been found to reduce the intensity and frequency of headache in children and adolescents, data on psychodynamic psychotherapy in this population are lacking. Our aim was to explore the effectiveness of a brief psychodynamic psychotherapy program in the treatment of idiopathic headache in childhood and adolescence. Methods: Thirty-three newly diagnosed idiopathic headache sufferers aged 6–18 years, consecutively referred to our outpatient services, were randomized to receive either a brief cycle of psychodynamic psychotherapy (eight sessions administered at two-week intervals) or usual care (clinical interview, neurological examination, counselling, symptomatic therapy). The two groups were evaluated at baseline (T0) and at six months (T1) to be assessed for headache characteristics (i.e. frequency, intensity and duration), quality of life (i.e. the EuroQoL score), patient’s global health status (i.e. the Clinical Global Impression score), and emotional-behavioral symptoms (i.e. Child Behavior Checklist scores). Results: The two groups were fairly similar with reference to the main demographic and clinical variables. The T0/T1 comparison showed a statistically significant improvement in headache frequency (p = 0.005), intensity (p
