Objective: To fulfill the promise of information technology in health care, automation must be made into a team player. Methods: Observational research in both the laboratory and field focused on how subjects program infusion devices. These programming activities were examined in detail for a set of tasks, using experienced clinicians as subjects. Observations were validated using U.S. Food and Drug Administration (FDA) incident reports and field studies. Results: The laboratory study shows that difficulties arise from poor coordination between the operator and the device as a result of device complexity. These findings are consistent with reports of device programming problems in the FDA's Manufacturer and User facility Device Experience (MAUDE) database. Conclusions: Problems with infusion devices are the result of difficulties that operators have with complex devices and their programming. Opportunities to improve device reliability lie primarily in improving the interface design, such as by making the device's state evident and improving menu structure navigation.

A model of team decision making based on an individual decision making model is developed. The team model focuses on how individual team members use informational cues to reach judgments that are incorporated into team decisions. The notion of distributed expertise illustrates the allocation of critical information across team members. The model specifically addresses the decision making processes of hierarchical teams in which there is one identified leader or decision maker for the team. In addition, this report describes an experimental paradigm for the study of team decision making under conditions of distributed expertise. Level of analysis issues in the study of team decision making are also discussed.

In this study, the role of communication and coordination in team decision making in a command and control task has been investigated. First, a framework has been developed in which all team-related entities are described. Next, we have developed an artificial world in which various events take place. This world is represented by a city consisting of various buildings in which citizens live. The events are series of fires. A fire-fighting organization has been set up to deal with this situation. This organization has to allocate a restricted amount of resources to fulfill its goal: to rescue as many lives as possible. Focus of research is on how to organize and support a team that observes the world and allocates the resources. These tasks have been analysed and a normative task structure has been described as a basis for performance measurements. Two experiments have been carried out. First, the hypothesis was tested that an experimental task (fire fighting) could not be carried out alone. Second, it was investigated whether teams that are restricted in their communication and coordination possibilities could perform as well as teams that can communicate and coordinate without restrictions. A third research topic was the role of distributing knowledge among the task performers. The following three main conclusions can be drawn from the experimental results. First, the fire-fighting task truly is a team task because for optimal task performance, crucial task elements should be carried out in parallel. Second, communication restricted to data exchange only leads to team performance decrease. Third, distributing knowledge over team members leads to an increase of parallel task execution and to better team performance.

The purpose of this project is to test our theoretical model. Our proposed dual-pathway model states that the consistency of team voice content increases decision-making speed by enhancing processing fluency. However, it may also adversely affect decision quality by constraining cognitive flexibility. In addition, leaders’ critical openness weakens the indirect effects of the consistency of team voice content on both decision speed and quality. An experiment investigating the effect of communication training and four group composition variables was performed with Patriot air defense teams for two different types of aircraft identification tasks. It was predicted that communication training would significantly enhance communication quantity and quality and, in turn, team performance for both tasks. Although the training did sometimes improve team communications processes, it did not improve team performance. The variable that had the biggest positive effect on communication quality and team performance was the number of hours a team had worked together. This effect was only found, however, for the type of task for which Patriot teams routinely train. It did not transfer to the less frequent and more cognitively stressing task where there is conflicting information about unknown aircraft, as in the U.S.S. Vincennes tragedy.

Protocol for a scoping review conducted as part of a PhD project at the Marie Curie Palliative Care Research Department, UCL (University College London).

Two experiments were conducted investigating the effects of gender and practice on team decision making while solving the Tower of Hanoi (TOH). The first experiment demonstrated that relevant practice on a 4-disk TOH wooden model, as compared to irrelevant practice using an Eight Puzzle, improves performance of individuals (20 females and 20 males) on a computerized 5-disk TOH and that males were faster than females at solving the TOH. The second experiment involved 84 two-person teams (84 females and 84 males) who solved the TOH in a design combining relevant practice gender (female, male, or neither) with gender mix (female-female, male-male, or female-male) resulting in 7 different treatment conditions (12 teams in each group). The results suggest that teams with males receiving relevant practice solve the puzzle in less overall moves and in a shorter overall time as compared to teams with females receiving relevant practice. Results from the analysis of the search space data suggest that the teams with females who have received relevant practice reverse more moves compared to teams with males receiving relevant practice. The verbal data indicate that males make more commands than females regardless of who receives relevant practice. These data were interpreted in the context of several theoretical approaches to the study of team decision making. Only 8 out of a possible 48 participants revealed to their teammate that they had some expertise (prior practice) in solving the TOH puzzle. This result is consistent with some of the aviation accident reports that indicate a crewmember had critical information or expertise that was not shared with the team and therefore a poor team decision was made. The application of the current methodology to more naturalistic situations is discussed.

This paper reports on the collaborative research plans for the last thrust of the Tactical Decision Making Under Stress program sponsored by the Office of Naval Research. Findings from the research will be used to develop principles and guidelines for combining training with decision support, and applications to advanced technology development research.

In planning and decision making tasks in which a lot of information has to be exchanged among team members under time pressure, it is of importance that team members are aware of each other's tasks and information needs and processing, because in that case team members can provide each other with the right information in time and without being explicitly asked for it. When well-coordinated teams anticipate on each other's information needs, this is taken as evidence that members of the team have developed a 'shared mental model'. In this report an experiment is described that has used a paradigm for team decision making with which shared mental models can be manipulated experimentally and their effects can be measured objectively. The task is carried out by two team members who are dependent on each other's information. By means of cross-training in each other's tasks, a shared mental model has been established by team members in the experimental group. The control group only received information about the team member's task on paper and was not cross-trained. The results showed that cross-training contributes to efficient communication by volunteering information without the other team member having to explicitly request it.

The focus of this work is to understand impactful aspects of team functioning as they solve complex problems, and propose the means to improve the performance of teams, under changing or adversarial conditions. By analyzing the characteristic differences between high performing and low performing teams, we identified behavior that characterized and differentiated the problem solving approach of the teams especially when the problem itself changes during solving. We then derived a simulated annealing-based algorithm that mimicked human team problem solving under these conditions; aspects of the algorithm also performed well as an optimization algorithm when solving a variety of hard numerical problems. Identification of the impact of team structure on problem solving behavior under changing conditions indicates that heterogeneous-grouped teams outperform homogeneous or non-grouped teams. Yet solving multiple problems sequentially is not as effective as breaking problems into smaller intermixed chunks, breaking fixation. Finally, a meta-analysis on the design literature where the impact of examples on problem solving was studied within the design process identified under what conditions examples will induce fixation or alternatively, inspiration.
