4Global Efficiency Of Local Immunization On Complex Networks.

This article is from Scientific Reports , volume 3 . Abstract Epidemics occur in all shapes and forms: infections propagating in our sparse sexual networks, rumours and diseases spreading through our much denser social interactions, or viruses circulating on the Internet. With the advent of large databases and efficient analysis algorithms, these processes can be better predicted and controlled. In this study, we use different characteristics of network organization to identify the influential spreaders in 17 empirical networks of diverse nature using 2 epidemic models. We find that a judicious choice of local measures, based either on the network's connectivity at a microscopic scale or on its community structure at a mesoscopic scale, compares favorably to global measures, such as betweenness centrality, in terms of efficiency, practicality and robustness. We also develop an analytical framework that highlights a transition in the characteristic scale of different epidemic regimes. This allows to decide which local measure should govern immunization in a given scenario.

This article is from Allergy, Asthma, and Clinical Immunology : Official Journal of the Canadian Society of Allergy and Clinical Immunology , volume 10 . Abstract Background: Over the last decade, our group has demonstrated that murine preconception immunization with allergens has a protective effect on allergy development in offspring. The murine model used in the present study allowed us to compare allergy induction by ovalbumin (OVA) and dust mite extract from Dermatophagoides pteronyssinus (Dp). Findings: Female mice were immunized with OVA or Dp. Pups from immunized and non-immune mothers were immunized at 3 days old (do) with the same antigen used for the maternal immunization. The offspring were analyzed at 20 do. Preconceptional immunization with OVA or Dp did not increase maternal IgE serum levels, although the immunizations induced an increase in allergen-specific IgG1 Ab levels. Offspring serum analyses revealed that maternal immunization with OVA suppressed IgE production only in offspring immunized with OVA. Both preconception immunization protocols inhibited cellular influx into the airways of immunized offspring compared with controls. Similar frequencies of offspring IgM + B cells were found in the OVA- and Dp-immunized groups compared with their respective control groups. Moreover, preconception immunization with OVA enhanced FcγRIIb expression on OVA-immunized offspring B cells. In contrast, decreased FcγRIIb expression was detected on Dp-immunized offspring B cells compared with cells from the offspring of non-immune mothers. Conclusions: Together, these results show that preconception OVA immunization and Dp immunization can inhibit allergy development but have opposite effects on FcγRIIb expression on offspring B cells.

11Knowledge, Attitude And Perception Towards Polio Immunization

Poliomyelitis is a highly contagious & viral disease that mainly affects young children. Despite the efforts of national and international organizations, polio has not been eradicated from Pakistan, which is one of three remaining polio-endemic countries in the world. Study aimed to assess the knowledge, attitude and practices among people regarding polio immunization in Lahore Pakistan. A cross-sectional study involving 300 participants was conducted from November 2018 to March 2019 in Lahore, Pakistan. Both male and female were included. Results showed that significant association was found between different statements of knowledge and age (p=0.005 and p=0.003), gender (p= 0.021), qualification (p=0.003, p<0.001, p= 0.021, p=0.001, and p=0.029), residential status (p=0.039, p=0.008 and p=0.001), employment status (p=0.030), monthly income (p=0.005, p=0.027, p=0.020,p=0.044 and p=0.002) and people having children less than 5 years of age (p=0.003). Significant association was found between different statements of attitude and gender (p=0.030), qualification (p=0.015and p=0.022), residential status (p=0.030), employment status ( p=0.011), monthly income (p=0.031) and people having children less than 5 years of age (p=0.033 and p=0.039). Significant association was also found between different statements of perception and age (p=0.008, p=0.001 and p=0.036), qualification (p=0.004, p=0.002, p= 0.001, p=0.002, and p=0.011, p=0.004, p=0.015, p= 0.001 and p=0.004), residential status (p=0.023, p=0.004, p=0.028 and p=0.014), employment status (p=0.015, p=0.007 and p=0.01), monthly income (p=0.048, p=0.021, and p=0.040) and people having children less than 5 years of age (p=0.037, p=0.027 and p=0.021). It is concluded that most of the participants had adequate knowledge & wanted the complete elimination of polio from Pakistan but there are still few people who have improper knowledge & they don’t believe in these campaigns which lead to failure of eradicating polio.

12Inducing Immunity? Justifying Immunization Policies In Times Of Vaccine Hesitancy

Why immunization must be made mandatory in times of vaccine hesitancy, and how we can design and implement immunization policies in a practical, trustworthy, and democratic way. We live in perilous times when a significant number of citizens are either defiantly antivaccination or hesitant to accept vaccinations for themselves or for their children. In Inducing Immunity?, legal philosopher Roland Pierik and bioethicist Marcel Verweij explore ways to regulate collective immunization in as democratic a manner as possible. Approaching the problem as a matter of a conflict between the responsibility of government to protect public health and the basic right to freedom of citizens, Pierik and Verweij argue that John Stuart Mill's harm principle - the idea that individuals should be free to act so long as their actions do not harm others - offers a strong basis for coercive immunization policies. Covering childhood immunization policies, as well as vaccination programs aimed at adult citizens, the authors argue that a coercive immunization policy in any liberal democracy must first satisfy the principle of proportionality. This leads them to an in-depth exploration of the role of exemptions, the nature of coercion, and the contents of vaccination programs. In the final part of the book, the authors also discuss the importance and scope of freedom of speech, given how the current spread of misinformation has undermined confidence in vaccines. 21An Overview Of The National Immunization Policy Making Process: The Role Of The Korea Expert Committee On Immunization Practices.

This article is from Korean Journal of Pediatrics , volume 55 . Abstract The need for evidence-based decision making in immunization programs has increased due to the presence of multiple health priorities, limited human resources, expensive vaccines, and limited funds. Countries should establish a group of national experts to advise their Ministries of Health. So far, many nations have formed their own National Immunization Technical Advisory Groups (NITAGs). In the Republic of Korea, the Korea Expert Committee on Immunization Practices (KECIP), established by law in the early 1990s, has made many important technical recommendations to contribute to the decline in vaccine preventable diseases and currently functions as a NITAG. It includes 13 core members and 2 non-core members, including a chairperson. Core members usually come from affiliated organizations in internal medicine, pediatrics, obstetrics, microbiology, preventive medicine, nursing and a representative from a consumer group, all of whom serve two year terms. Non-core members comprise two government officials belonging to the Korea Centers for Disease Control and Prevention (KCDC) and the Korea Food and Drug Administration. Meetings are held as needed, but at least twice a year, and sub-committees are formed as a resource for gathering, analyzing, and preparing information for the KECIP meetings. Once the sub-committees or the KCDC review the available data, the KECIP members discuss each issue in depth and develop recommendations, usually by a consensus in the meeting. The KECIP publishes national guidelines and immunization schedules that are updated regularly. KECIP's role is essentially consultative and the implementation of their recommendations may depend on the budget or current laws.

22An Exploratory Qualitative Assessment Of Factors Influencing Childhood Vaccine Providers' Intention To Recommend Immunization In The Netherlands.

Abstract Background: Under the Dutch national immunization program (NIP), childhood vaccination is not mandatory, but its recommendation by childhood vaccine providers (CVP) is important for maintaining high vaccination coverage. We therefore examined factors related to providers' intentions to recommend vaccinations to parents of young children. Methods: We conducted four focus group discussions with nurses and physicians who provide vaccines to children 0-4 years old in diverse regions of the Netherlands. Three groups represented CVPs at child welfare centers (CWCs) serving the general population, with the fourth representing anthroposophical CWCs. Elements of the Theory of Planned Behaviour (TPB) were used to design the groups; thematic analysis was used to structure and analyze the dataset. Results: Four main themes emerged, including 1) perceived responsibility: to promote vaccines and discuss pros and cons with parents (although this was usually not done if parents readily accepted the vaccination); 2) attitudes toward the NIP: mainly positive, but doubts as to NIP plans to vaccinate against diseases with a low perceived burden; 3) organizational factors: limited time and information can hamper discussions with parents; 4) relationship with parents: crucial and based mainly on communication to establish trust. Compared to CVPs at standard CWCs, the anthroposophical CWCs spent more time communicating and were more willing to adapt the NIP to individual cases. Conclusions: Our qualitative assessment provides an overview of beliefs associated with providers' intention to recommend vaccinations. They were motivated to support the NIP, but their intentions to recommend vaccinations were affected by the perceived relevance of the vaccines, practical issues like limited time and by certain types of resistant parents. These results will inform future studies to test the magnitude and relative impact of these factors.

Abstract Background: Haemophilus influenzae type b (Hib) vaccine and pneumococcal conjugate vaccine (PCV) are relatively expensive, newly introduced vaccines in China. This study evaluates the impact of residency and urbanicity on Hib vaccine and PCV coverage for children aged 2 to 7 years living in Shanghai, China, in August 2012. Methods: In this exploratory cohort study, a sample of children aged 2 to 7 years, all of whom were eligible to have received the complete series of Hib vaccine and PCV, was obtained from the Shanghai Immunization Program Information System. Three measures of vaccination coverage for Hib vaccine and PCV were examined: dose 1 coverage, series completion, and timeliness of dose 1 vaccination. Multivariable binomial regression was used to estimate the difference in vaccination coverage between locals and the floating population. Results: Dose 1 coverage was 50.9% for Hib vaccine and 11.4% for PCV for the 28,141 abstracted pediatric records. For both vaccines, dose 1 coverage was higher in locals than in the floating population. The disparity in coverage between locals and the floating population was greater in suburban areas than urban areas. Of all children who received dose 1, 79.7% completed the Hib vaccine series, and 91.3% completed the PCV series. Timely dose 1 coverage was 8.2% for Hib vaccine and 0.5% for PCV. Conclusion: Low vaccination coverage and extremely low levels of timely dose 1 vaccination indicate that current vaccination efforts are inadequate to reduce the burden of Hib and pneumococcal disease among Chinese children, especially infants. Government funding of the Hib vaccine and PCV through the Expanded Program on Immunization would increase uptake and could also ensure that improvement in the timeliness of administration and series completion is targeted for all demographic groups.

This article is from PLoS ONE , volume 9 . Abstract Haemophilus influenzae type b (Hib) is now recognized as an important pathogen in Asia. To evaluate disease susceptibility, and as a marker of Hib transmission before routine immunization was introduced in Kathmandu, 71 participants aged 7 months–77 years were recruited and 15 cord blood samples were collected for analysis of anti-polyribosylribitol phosphate antibody levels by enzyme-linked immunosorbent assay. Only 20% of children under 5 years old had levels considered protective (>0.15 µg/ml), rising to 83% of 15–54 year-olds. Prior to introduction of Hib vaccine in Kathmandu, the majority of young children were susceptible to disease.

Abstract An efficient algorithm that can properly identify the targets to immunize or quarantine for preventing an epidemic in a population without knowing the global structural information is of obvious importance. Typically, a population is characterized by its community structure and the heterogeneity in the weak ties among nodes bridging over communities. We propose and study an effective algorithm that searches for bridge hubs, which are bridge nodes with a larger number of weak ties, as immunizing targets based on the idea of referencing to an expanding friendship circle as a self-avoiding walk proceeds. Applying the algorithm to simulated networks and empirical networks constructed from social network data of five US universities, we show that the algorithm is more effective than other existing local algorithms for a given immunization coverage, with a reduced final epidemic ratio, lower peak prevalence and fewer nodes that need to be visited before identifying the target nodes. The effectiveness stems from the breaking up of community networks by successful searches on target nodes with more weak ties. The effectiveness remains robust even when errors exist in the structure of the networks.

Abstract Dengue viruses (DENVs) cause approximately 390 million cases of DENV infections annually and over 3 billion people worldwide are at risk of infection. No dengue vaccine is currently available nor is there an antiviral therapy for DENV infections. We have developed a tetravalent live-attenuated DENV vaccine tetravalent dengue vaccine (TDV) that consists of a molecularly characterized attenuated DENV-2 strain (TDV-2) and three chimeric viruses containing the pre-membrane and envelope genes of DENV-1, -3, and -4 expressed in the context of the TDV-2 genome. To impact dengue vaccine delivery in endemic areas and immunize travelers, a simple and rapid immunization strategy (RIS) is preferred. We investigated RIS consisting of two full vaccine doses being administered subcutaneously or intradermally on the initial vaccination visit (day 0) at two different anatomical locations with a needle-free disposable syringe jet injection delivery devices (PharmaJet) in non-human primates. This vaccination strategy resulted in efficient priming and induction of neutralizing antibody responses to all four DENV serotypes comparable to those elicited by the traditional prime and boost (2 months later) vaccination schedule. In addition, the vaccine induced CD4+ and CD8+ T cells producing IFN-γ, IL-2, and TNF-α, and targeting the DENV-2 NS1, NS3, and NS5 proteins. Moreover, vaccine-specific T cells were cross-reactive with the non-structural NS3 and NS5 proteins of DENV-4. When animals were challenged with DENV-2 they were protected with no detectable viremia, and exhibited sterilizing immunity (no increase of neutralizing titers post-challenge). RIS could decrease vaccination visits and provide quick immune response to all four DENV serotypes. This strategy could increase vaccination compliance and would be especially advantageous for travelers into endemic areas.

Abstract Background: Validated measures that can accurate describe young adults’ HPV vaccination attitudes and how these relate to vaccination intention and receipt are needed for developing interventions to improve low HPV vaccination levels. The Carolina HPV Immunization Attitudes Scale (CHIAS) is a validated measure of these outcomes that was originally designed for parents. Objective: To assess the performance of the CHIAS among young adult women using an exploratory factor analysis. Methods: A convenience sample of 139 young adult women (age 18–26 years) were given the CHIAS measure at baseline. Factor analysis was used to determine attitudinal factor groupings and the association of these factors with HPV vaccination intention. A 6-month follow up assessment examined the stability of the CHIAS over time and the association of baseline vaccine factors with vaccine receipt. Results: Five factors loaded on to the CHIAS in young adults - “Barriers,” “Harms,” “Effectiveness,” “Risk Denial” and “Uncertainty,” - which was similar to the factor loadings of CHIAS for parents. “Harms” was the factor most consistently associated with vaccination intention at all time points assessed. Only 5 women had received or made an appointment to receive the vaccine at the 6-month follow-up. Conclusions: In terms of categorizing HPV vaccination attitudes, the CHIAS appears to have similar performance among young adults as in parents. However, additional studies are needed to assess the utility of the CHIAS for predicting HPV vaccine receipt among the young adult population.

Abstract Background: Immunization coverage levels in Guatemala have increased over the last two decades, but national targets of ≥95% have yet to be reached. To determine factors related to undervaccination, Guatemala’s National Immunization Program conducted a user-satisfaction survey of parents and guardians of children aged 0–5 years. Variables evaluated included parental immunization attitudes, preferences, and practices; the impact of immunization campaigns and marketing strategies; and factors inhibiting immunization. Methods: Based on administrative coverage levels and socio-demographic indicators in Guatemala’s 22 geographical departments, five were designated as low-coverage and five as high-coverage areas. Overall, 1194 parents and guardians of children aged 0–5 years were interviewed in these 10 departments. We compared indicators between low- and high-coverage areas and identified risk factors associated with undervaccination. Results: Of the 1593 children studied, 29 (1.8%) were determined to be unvaccinated, 458 (28.8%) undervaccinated, and 1106 (69.4%) fully vaccinated. In low-coverage areas, children of less educated (no education: RR = 1.49, p = 0.01; primary or less: 1.39, p = 0.009), older (aged >39 years: RR =1.31, p = 0.05), and single (RR = 1.32, p = 0.03) parents were more likely to have incomplete vaccination schedules. Similarly, factors associated with undervaccination in high-coverage areas included the caregiver’s lack of education (none: RR = 1.72, p = 0.0007; primary or less: RR = 1.30, p = 0.05) and single marital status (RR = 1.36, p = 0.03), as well as the child’s birth order (second: RR = 1.68, p = 0.003). Although users generally approved of immunization services, problems in service quality were identified. According to participants, topics such as the risk of adverse events (47.4%) and next vaccination appointments (32.3%) were inconsistently communicated to parents. Additionally, 179 (15.0%) participants reported the inability to vaccinate their child on at least one occasion. Compared to high-coverage areas, participants in low-coverage areas reported poorer service, longer wait times, and greater distances to health centers. Conclusions: Generally, immunization barriers in Guatemala are related to problems in accessing and attaining high-quality immunization services rather than to a population that does not adequately value vaccination. We provide recommendations to aid the country in maintaining its achievements and addressing new challenges.

This article is from Veterinary Research , volume 45 . Abstract Campylobacter jejuni is the most common cause of bacterium-mediated diarrheal disease in humans worldwide. Poultry products are considered the most important source of C. jejuni infections in humans but to date no effective strategy exists to eradicate this zoonotic pathogen from poultry production. Here, the potential use of passive immunization to reduce Campylobacter colonization in broiler chicks was examined. For this purpose, laying hens were immunized with either a whole-cell lysate or the hydrophobic protein fraction of C. jejuni and their eggs were collected. The Moral Equivalent of War, the last public utterance of William James, is significant as expressing the opinions of a practical psychologist on a question of growing popular interest. For the past fifteen years the movement for promoting international peace has been enlisting the support of organizations and individuals the world over. That this is a question on which much may be said for the opposition, James, though a pacificist, admits with his usual fair-mindedness, pointing out that militarism is the sole nourisher of certain human virtues that the world cannot let die, and that until the peace party devises some substitute, some moral equivalent, for the disciplinary value of war, their utopian goal is neither desirable nor possible. His own solution is advanced not as a practical measure, but merely as an illustration to show that the world is full of opportunities for the peaceful development and continuation of the martial qualities of human life. <BR><BR> This essay was written for general dissemination as a publication of the American Association for International Conciliation, February, 1910. The recently widowed Adela Gereth, a lover of beauty and passionate collector of fine objects, strikes up a friendship with the young Fleda Vetch, when both of them find themselves guests in the tasteless house of Brigstock family. Mrs. Gereth fears that her son Owen, an honorable but somewhat unimaginative young man, may take up with one of the Brigstock girls, and indeed he presently announces his engagement to Mona, the eldest daughter. That means that Mrs. Gereth will have to leave Poynton, the beautiful house that she and her husband filled with the furniture, china, tapestries, and other objects that they lovingly collected over the years. It is not so much possessiveness that drives Mrs. Gereth to want to maintain control over them (or so she claims, at any rate), but rather the sense that she will have failed if Mona, understanding and appreciating nothing of what Poynton contains, should become Owen's wife and take charge. The story and its developing conflicts are seen largely from the point of view of Fleda Vetch, the young woman who, her moral and aesthetic sensibilities tuned perhaps as finely as any of James's protagonists, finds herself caught in the middle. 'Pragmatism' contains a series of public lectures held by William James in Boston 1906–7. James provides a popularizing outline of his view of philosophical pragmatism while making highly rhetorical and entertaining lashes towards rationalism and other competing schools of thought. James is especially concerned with the pragmatic view of truth. True beliefs should be defined as, according to James, beliefs that can successfully assist people in their everyday life. This is claimed to not be relativism. That reality exists is argued to be a fact true beyond the human subject. James argues, nevertheless, that people select which parts of reality are made relevant and how they are understood to relate to each other. Charles Sanders Peirce, widely considered to be the founder of pragmatism, eventually chose to separate himself intellectually from James, renaming his own theory to ‘pragmaticism’.

It plays on the great Jamesian theme of the American abroad, who finds himself in an older, and some would say richer, culture that that of the United States, with its attractions and dangers. Here the protagonist is Lambert Strether, a man in his fifties, editor of a small literary magazine in Woollett, Massachusetts, who arrives in Europe on a mission undertaken at the urging of his patron, Mrs. Newsome, to bring back her son Chadwick. Neither the name of Shakespeare nor that of Stratford appears directly in this short piece by James, and yet both are absolutely central to his plot. The story has to do with Mr. and Mrs. Gedge, tempted away from a dreary northern town library, which he runs, to become the wardens – caretakers and tour guides – of the house where the greatest writer of the English language was born, and in which he grew up.<br /><br /> Or did he? There is, after all, a paucity of facts about His life (in James's text, that pronoun is always capitalized, as befits a deity) and only the slenderest of historical evidence about the existence of such a man. No matter; what is important is the myth of his life, and the myth needs to be cared for and fostered so that crowds upon crowds of tourists may come, and, with a proper reverence, worship at His Birthplace.<br /><br /> And yet it is only myth, and the more he thinks of it, the unhappier poor honest Gedge becomes (to Mrs. Gedge, however, a job is a job, and too much speculation on reality might perhaps lead to dismissal). James himself was high skeptical about the Shakespeare question (who actually did write all those plays?) But that's not the point here. Rather the story has to do with the making of a shrine, the selling of its wares (commodification, to use a fancy word) and the priesthood needed to protect the myths necessary to its existence.<br><br>What should the skeptical Gedge do about it? Published as a serial in 1875, Roderick Hudson is James's first important novel. The theme of Americans in Europe, so important in much of James's work, is already central to the story. Hudson is a young law student in Northampton, Massachusetts, who shows such surprising ability as a sculptor that the rich Rowland Mallett, visiting a cousin in Northampton, decides to stake him to several years of study in Rome, then a center of expatriate American society. The story has to do not only with Roderick's growth as an artist and the problems it brings, but also as a man susceptible to his new environment, and indeed his occasional rivalries with his American friend and patron.<br>Interestingly enough, at least two of the book's characters made sufficient impression on James that he brought them back in other books: the French-Italian-Yankee sculptor, Gloriani, who appears later in The American, and Christina Light, the American who becomes the subject of Princess Casamassima, one of James's later (and much longer) novels.<br>(Introduction by Nicholas Clifford)

Henry James wrote a number of ghost stories -- The Turn of the Screw being the most famous. Did he believe in ghosts himself, as did many of his contemporaries? It's generally possible to find earthly interpretations, Freudian and other, for his ghosts. Sir Edmund Orme, though, is unquestionably a real ghost -- except of course that James's unnamed narrator tells the story in the voice of yet a third man, and the narrator himself passes no judgments on the factual nature of what he is reporting (there's a resemblance here to The Turn of the Screw). The story has to do with two love affairs in two generations, and Sir Edmund, real or imagined, plays a role in each. The Portrait of a Lady is a novel by Henry James, first published as a serial in The Atlantic Monthly and Macmillan's Magazine in 1880–81 and then as a book in 1881. It is one of James's most popular long novels, and is regarded by critics as one of his finest. The Portrait of a Lady is the story of a spirited young American woman, Isabel Archer, who "affronts her destiny" and finds it overwhelming.
