Our guest is filmmaker Kenneth Jefferson, whose new documentary "Fatal Error" has been called "the most profound film on the internet" by Reelblack One. "Fatal Error" is a short documentary that focuses on a major cultural issue within the black community that many are aware of but have either ignored or have never truly thought about. This fatal error has led to the destruction of the black community from within. How? By glorifying "pimp and ho" culture, gang affiliation, violent role models, gangsta rap, drug use and trafficking, and a life of crime. Jefferson, director of "Fatal Error" believes that if this cultural issue is resolved, that it will trigger a mass awakening in the black community that will have major effects in the United States and the world as a whole. The film "Fatal Error" is the first part of a multitiered plan meant to influence a radical cultural shift that the filmmaker believes is on par with other ideological revolutions such as Martin Luther’s 95 Theses or The Enlightenment. Kenneth Jefferson is a middle school teacher and social activist who made "Fatal Error" as a labor of love, in hopes of saving Black America from itself -- before it's too late. Since analytic resources were limited, it was decided to focus on vehicular accidents that: (a) were Army-responsible in terms of accountability and prevention; and (b) had the best information in terms of quality and quantity. It was decided to select on-duty vehicular accidents because the Army is clearly responsible for them. From these on-duty accidents, those which resulted in a fatality were selected because their reports were expected to have better information than reports of less severe accidents. Also, the number of fatal on-duty accidents was small enough to permit a cause-factor analysis of each report. It was expected that drivers would be frequently cited as accident cause factors so the analysis was directed toward driver error.

Civil flights (non-military) in the United States (U.S.) are classified as either General Aviation (GA) or air carrier operations. General aviation activities include recreational flying, flight instruction, agricultural operations, sightseeing, and business travel (U & Baker, 1999). The aircraft involved in GA flying may be piloted by a variety of people with a valid pilot license and approved medical history, but belonging to a wide range of age groups. The aircraft flown by GA pilots include airplanes, helicopters, balloons, and gliders. Each year, the National Transportation Safety Board (NTSB) records about 2000 GA crashes, which claim about 750 lives (National Safety Council, 1997). Between 1990-1996, GA accounted for 93% of all aviation crashes and 78% of all aviation fatalities. In fact, a pilot flying under the auspices of general aviation is nearly 10 times more likely to be involved in an accident than a military air crewmember (USAF, 2003), more than 11 times more likely than a commercial air crewmember (NTSB 2003b; NTSB, 2003c), and almost 50 times more likely to be involved in a fatal crash than large scale commercial pilots (NTSB, 2003a).
