Epileptogenesis is a gradual process by which normal brain transforms into one that sustains seizures. It is instigated by an inciting event (e.g. prolonged seizure called status epilepticus (SE), head injury, infection or stroke). This is followed by a variable (months to years in humans) latent period followed by the emergence of spontaneous seizures, with potential for later mood and learning disabilities. While the latent period is a time during which behavioral seizures are not observed, it is a period of tissue and cellular remodeling that sets up the development of chronic seizure activity, or epilepsy. In this grant, we have drawn expertise from other fields to discover new mechanistic insights into epileptogenesis. In the past year, we have expanded our understanding of molecular mechanisms, specifically Wnt signaling and an altered metabolism in the hippocampus of mice undergoing early epileptogenesis. In doing so, we have uncovered a new therapeutic drug combination that we had been developing for breast cancer treatment aimed at Wnt signaling. Surprisingly, the combination attenuates seizures in two different models of acute seizures as well as in a model of chronic seizure onset. This combination may have efficacy for preventing the onset of chronic seizures that define epileptic progression.

Community pharmacist plays an essential role in educating the epileptic patients about their disease and medications. Improving the patient’s awareness may lead to improve their compliance and decrease drug-drug interaction and ultimately improve their quality of life. Pharmacist can detect the emergence of health problems and can help prevent progression of comorbidities. Considering the complexity of treating epilepsy and the lack of information about pharmacists’ contributions to epilepsy management, pharmacist performed pharmaceutical counselling, pharmaco-therapeutic follow-up and systematic measurement and evaluation of findings and increase medication adherence of patient with epilepsy. This study was aimed to assess the community pharmacist's knowledge about epilepsy and their treatment by antiepileptic drugs in Libya. The design of the study is a cross sectional study. The knowledge was collected through a questionnaire which included 35 questions divided into three sections: demographic data, general information about epilepsy and information about epilepsy and antiepileptic drugs. The questionnaire was prepared and validated by consultant training in Ali Omer Asker Hospital in Tripoli for two months. The findings indicate that over 200 patients are altered viewed, of whom, only epileptic patients were observed. The majority of the participants were female, qualification degree BSc and years of experience from one to five years. Unfortunately, some of the participants (40.0%) had poor knowledge and about 60.0% of them had good knowledge. This study indicates that the importance of community pharmacist in Libya requires more improvement to achieve the existing function and that the impact of continuous study of everything related to diseases and medicines is important to obtain a qualified pharmacist who can become an effective agent for a change.

Magnetic resonance spectroscopy (MRS) is an MRI technique used to measure regional variations in neurochemistry and display the concentrations of various brain metabolites in preset regions of interest in the brain. In this study, we aimed to evaluate the correlation of qEEG abnormalities and MRS findings in patients with temporal lobe epilepsy (TLE). Material and Methods: Power spectrum analyses of each multi-channel EEG were performed in all patients with the concomitant MRS measures of Choline (Cho), total Creatine (Cr), N-acetyl aspartate (Naa) concentrations, and ratios of Cho/Cr, Naa/Cr, and Naa/Cho. Results: Mean qEEG band powers of anterior alfa, anterior beta, central alfa, central beta, posterior alfa, and posterior beta in the right temporal area was significantly larger in patients whose Naa/Cho+Cr ratio was lower than 0.71 compared to patients whose Naa/Cho+Cr ratio was equal and larger than 0.71. In addition, correlation analyses between Naa/Cho+Cr, Naa/Cho+Cr, and Naa/Cho ratios and qEEG band power values showed low-medium correlations. Discussion: A relationship detected between the biochemical changes of the epileptogenic focus due to neuronal dysfunction and the QEEG data may help prove the presence of the focal point.

This study aimed to describe demographic, seizure, and family characteristics associated with good and poor adaptation to childhood epilepsy and contrast them with demographic, asthma, and family characteristics associated with good and poor adaptation to childhood asthma. Children with asthma were selected as a comparison sample because epilepsy and asthma are both chronic medical conditions that are characterized by unpredictable episodes that generally require regular intake of medication and regular visits to a physician. Data were gathered from the children (ages 8-12), their mothers, and their school teachers. The theoretical framework for the selection of family characteristics was the Double ABCX Model of Family Adaptation and Adjustment in which three factors affecting coping are family demands or stressors on the family, family adaptive resources, and family definition or attitudes toward the condition and its impact. Results showed that family strength variables were strong predictors of child adaptation to both epilepsy and asthma. The mother's attitude was the strongest predictor of child adaptation for asthma but was not a strong predictor for epilepsy. (14 references.) (JDD)

Abstract In pre-surgical evaluation of epilepsy, there has been an increased interest in the study of electroencephalogram (EEG) activity outside the 1-70 Hz band of conventional frequency activity (CFA). Research over the last couple of decades has shown that EEG activity in the 70-600 Hz range, termed high frequency oscillations (HFOs), can be recorded intracranially from all brain regions both interictally and at seizure onset. In patients with epilepsy, HFOs are now considered as pathologic regardless of their frequency band although it may be difficult to distinguish them from the physiologic HFOs, which occur in a similar frequency range. Interictal HFOs are likely to be confined mostly to the seizure onset zone, thus providing a new measure for localizing it. More importantly, several studies have linked HFOs to underlying epileptogenicity, suggesting that HFOs can serve as potential biomarkers for the illness. This article is from Frontiers in Systems Neuroscience , volume 8 . Abstract Temporal lobe epilepsy (TLE) and mild cognitive impairment (MCI) are both subject to intensive memory research. Memory problems are a core characteristic of both conditions and we wonder if there are analogies which would enrich the two distinct research communities. In this review we focus on memory decline in both conditions, that is, the most feared psychosocial effect. While it is clear that memory decline in MCI is highly likely and would lead to the more severe diagnosis of Alzheimer's disease, it is a debate if TLE is a dementing disease or not. As such, like for MCI, one can differentiate progressive from stable TLE subtypes, mainly depending on the age of onset. Neuroimaging techniques such as volumetric analysis of the hippocampus, entorhinal, and perirhinal cortex show evidence of pathological changes in TLE and are predictive for memory decline in MCI. Several studies emphasize that it is necessary to extend the region of interest—even whole-brain characteristics can be predictive for conversion from MCI to Alzheimer's disease. Electroencephalography is increasingly subject to computational neuroscience, revealing new approaches for analyzing frequency, spatial synchronization, and information content of the signals. These methods together with event-related designs that assess memory functions are highly promising for understanding the mechanisms of memory decline in both TLE and MCI populations. Finally, there is evidence that the potential of such markers for memory decline is far from being exhausted. Similar structural and neurophysiological characteristics are linked to memory decline in TLE and MCI. We raise the hope that interdisciplinary research and cross-talk between fields such as research on epilepsy and dementia, will shed further light on the dementing characteristics of the pathological basis of MCI and TLE and support the development of new memory enhancing treatment strategies.

Abstract Despite major advances in neuroimaging, no lesion is visualized on MRI in up to a quarter of patients with drug-resistant focal epilepsy presenting for presurgical evaluation. These patients demonstrate poorer surgical outcomes than those with lesion seen on MRI. Accurate localization of the seizure onset zone (SOZ) is more difficult in MRI-negative patients and often requires invasive EEG recordings. Positron emission tomography (PET) and magnetoencephalography (MEG) have been proposed as clinically relevant tools to localize the SOZ prior to intracranial EEG recordings. However, there is no consensus regarding the optimal gold standard that should be used for assessing the performance of these presurgical investigations. Here, we review the current knowledge concerning the usefulness of PET and MEG for presurgical assessment of MRI-negative epilepsy. Beyond the individual diagnostic performance of MEG and of different PET tracers, including [18F]-fluorodeoxyglucose, [11C]flumazenil, and markers of 5-HT1A receptors, recent data suggest that the combination of PET and MEG might provide greater sensitivity and specificity than that of each of the two individual tests in patients with normal MRI.

Abstract Background:: Epilepsy is a common, chronic neurological disorder that affects more than 40 million people worldwide. Epilepsy is characterized by interictal and ictal functional disturbances. The presence of interictal epileptiform discharges (IEDs) can help to confirm a clinical diagnosis of epilepsy, and their location and characteristics can help to identify the epileptogenic zone or suggest a particular epilepsy syndrome. The aim of this study is to determine the factors that affect IEDs. Materials and Methods:: Poisson marginal model was done on 60 epileptic patients who were referred to Shefa Neurological Research Center, Tehran, for Video-Electroencephalogram (V-EEG) monitoring from 2007 to 2011. The frequency of IEDs was assessed by visual analysis of interictal EEG samples for 2 h. Results:: The results show that among age, epilepsy duration, gender, seizure frequency and two common anti-epileptic drugs (Valproic acid and Carbamazepine), only age and epilepsy duration had statistical significant effect on IED frequency. Conclusion:: Investigating the factors affecting IED is not only of theoretical importance, but may also have clinical relevance as understanding the evolution of interictal epileptogenesis may lead to the development of therapeutic interventions. Generalized estimating equation is a valid statistical technique for studying factors that affect on IED. This research demonstrates epilepsy duration has positive and age has negative effect on IED which means that IED increases with epilepsy duration and decreases with increasing age. So for monitoring IED, we should consider both age and epilepsy duration of each patient.
