Evidence is presented that the pure opiate antagonist naloxone rapidly increases blood pressure and decreases mortality associated with shock caused by endotoxemia, hemorrhage, and spinal-cord transection. Shock studies conducted in rats, cats, and dogs demonstrate these therapeutic effects of naloxone across a variety of species. Additionally, naloxone treatment significantly improves neurologic recovery (paralysis) after spinal-cord injury in the cat. Results from additional experiments provide evidence for the site and mechanisms of these therapeutic effects of naloxone.

In order to investigate the pathophysiological role of endogenous morphine-like substances (endorphins for short) in shock, we studied cynomolgus monkeys and dogs subjected to hemorrhagic or endotoxic shock. Blockade of opiate receptors with naloxone improved cardiovascular function (mean arterial pressure, cardiac output, and myocardial contractility) in both species and both models but requires correction of acidosis and hypothermia. Shock is associated with elevations in plasma levels of Beta-endorphin and Beta-lipotropin. Using different sites of injection and various pharmacological and anatomical ablations, we have shown that naloxone's beneficial effects in hemorrhagic shock are due to potentiation of the effects of released catecholamines on cardiac opiate receptors. The myocardial depression found in shock is due to an endorphin-induced attenuation of catecholamine effects on the heart. We believe this is mediated by interaction with cardiac receptors and is expressed via G-protein activation of adenylate cyclase and cyclic-AMP. This hypothesis needs to be tested by biochemical determination of these substances, and our observations need to be extended to endotoxic shock. Nevertheless, naloxone and other antiendorphin substances may be important in the treatment of shock by reversing one of the important pathophysiological mechanisms of cardiovascular depression. In order to investigate the possible role and involvement of endogenous morphine-like substance (endorphins) in shock we studied cynomulgus monkeys subjected to hemorrhagic and endotoxemic shock. Blockade of opiate receptors with naloxone improved cardiovascular performance and survival in endotoxemic shock but produced only mild improvement in cardiovascular function and no increased survival in hemorrhagic shock. We conclude that endorphin systems are activated in and contribute to the cardiovascular depression in primate shock. Further work in his area is justified including development of a better primate hemorrhagic shock model.

In order to investigate the role of endorphins (short for endogenous morphine-like substances) in the pathophysiology of cardivascular depression in primate shock we subjected cynomulgus monkeys to hemorrhage or endotoxemia. Blockade of opiate receptors with naloxone improved cardiovascular performance(mean arterial pressure and left ventricular contractility) and survival in both endotoxic and hemorrhagic shock--provided pH and temperature were maintained. Physiological antagonism of endorphins with thyrotropin-releasing hormone resulted in improved cardiovascular function in both forms of shock and increased survival in hemorrhage. Intact adrenal glands and cardiac autonomic innervation are necessary for naloxone's full effects to obtain, at least in the dog. Central nervous system injection of an endorphin produced bradycardia and hypotension which were reversed by naloxone, but central injection of naloxone did not reverse hemorrhagic shock in the monkey. Opiates failed to release histamine in vivo (rat) and in vitro (rat peritoneal mast cells). The mechanisms of endorphin-mediated cardiovascular dysfunction in shock and its reversal by naloxone are central and not peripheral and involve central nervous system centers controlling autonomic nervous system function or integration.
