This is a special bonus episode of the When Life Gives You Parkinson's podcast which we are calling \"Extra Dosage.\" Full episodes of Season 2 are available every other Wednesday. Every six months, I visit Dr. Jonathon Squires at UBC's Djavad Mowafaghian Centre for Brain Health. He is my neurologist and movement disorder specialist. Last week, my wife Rebecca and I sat down with him after our examination and interviewed him for the podcast. We talk about my symptom progression, my medicine regiment, DBS, biomarkers, what a cure will likely look like, stem cells, and the role stress might play in disease progression and symptom management.Even though my motor symptoms are beginning to appear on both my right and left side, in general the tremor and gate issues are stabilized year over year. I am however collecting non-motor symptoms with gusto including exhaustion, anxiety, active dreaming, insomnia, hot flashes, excessive sweating, constipation and bladder urgency. I regularly have difficulty recalling proper nouns and names or short-term instructions, weekend plans, etc. Rebecca noted to Dr. Squires that when I'm \"off,\" I often will be in a zone or bubble completely unaware of what's happening around me and not hearing what she says. I am also dealing with neuropathy, dystonia and dyskinesia.In an effort to tackle a few of the more annoying symptoms, Dr. Squires increased my carbidopa-levodopa again. I am now taking five times a day, every four hours, with an optional sixth time in the middle of the night. He also added a sedative, an anti-depressant, a laxative, and Vitamin B Complex. All in - vitamins, supplements, medications and probiotics, I am swallowing more than 200 pills and capsules a week. This report applies a NSBIT -developed probabilistic model to a body of social survey findings described by Fidell, Barber and Schultz (1988) to derive a dosage-effect relationship between outdoor noise exposure and the prevalence of annoyance in communities. The probabilistic model (Fidell, Schultz and Green, 1988) provides a means for independently estimating the contributions of acoustic and nonacoustic factors (the latter collectively termed 'response bias') to the observed prevalence of annoyance in communities. The results of the analysis described in this report permit construction of tools that environmental planners can use to make more sophisticated and defensible predictions of annoyance associated with noise of Air Force flight operations. Abstract — The objective of this experiment was to investigate the effect of storage and poultry manure dosage on soil nitrate (NO 3 - ) and ammonium (NH 4 + ) availability, N-uptake, and yield of Head lettuce (Lactuca sativa, L.) grown on Typic Calciaquoll. Data obtained from the experiment of seven treatments with four replications were subjected to Randomized Block Design. The observation conducted in three times i.e. 4 Week After Plant (WAP), 6 WAP, and 8 WAP. The treatments were: (1) without poultry manure (control), (2) dry poultry manure (DPM) with 12,5 g dosage, (3) DPM with 25 g dosage, (4) DPM with 37,5 g dosage, (5) fresh poultry manure (FPM) with 12,5 g dosage, (6) FPM with 25 g dosage, (7) FPM with 37,5 g dosage. The results of experiment showed that there were significantly effects of storage and poultry manure dosage on soil nitrate and ammonium, N-uptake, and yield of head lettuce. The treatment combination of DPM 37,5 g showed the highest value on soil nitrate and ammonium in 4 WAP, N-uptake in 6 and 8 WAP, and the yield of head lettuce in 6 and 8 WAP. The treatment combination of FPM 37,5 g showed the highest value on soil nitrate and ammonium in 6 and 8 WAP, N-uptake in 4 WAP, and the yield of Head lettuce in 4 WAP. Generally, it concluded that the dry poultry manure (DPM) had the better effects than the fresh poultry manure (FPM) on yield of head lettuce .

A novel, simple and economic reverse phase high performance liquid chromatography (RP-HPLC) method has been developed for the estimation of Nelaribine in bulk and tablet dosage form with greater precision and accuracy. Separation was achieved on Cosmiscil C18 column (150X4.6mm i.d.,5µm) in isocratic mode using Triflouro acetic acid PH-3.6 buffer and Acetonitrile in the ratio of 90:10(v/v) as mobile phase, pumped in to the column at flow rate of 1.0 mL min-1and the detection of eluent from the column was carried out using variable wavelength UV detector at 248 nm. The total run time was 15 min and the column was maintained at ambient temperature. The retention time of Nelaribine was 4.003 min. The standard curves were linear over the concentration range of 25-150 µg/ml with R2 0.999 and the LOD and LOQ values for Nelaribine were 0.04 µg/ml and 0.12 µg/ml , respectively. The percentage recovery was found to be 101.76 - 98.72 %, the % RSD was found to be 0.43. The percentage amount of a marketed tablet formulation of Nelaribine was found to be 101.2 %. The method was validated as per ICH guidelines. Validation studies demonstrated that the proposed RP-HPLC method is simple, specific, rapid, reliable and reproducible. Hence the proposed method can be applied for the routine quality control analysis of Nelaribine in bulk and tablet dosage forms. Abstract This study was done to determine whether high or low dose ofanti-snake venom (ASV) is better incoagulopathy invictims of envenoming by vipers. This retrospective study was conducted on the 154 patients (Mean age ± SD, Range) of viper snake bites who were referred to the emergency ward of Razi Hospital, Ahvaz, Iran over 2 years period (2004-2006).According to the treatment dosage the patients were divided in two groups include group 1(78 cases), low dose regimen and group 2 (76 cases), high dose one. In group 1, the treatment was performed by administration of 4 to 6 vials of ASV through intravenous infusion.In group 2, the patients were given 5 to 10 vials of ASV as an initial dose. In low dose regimen, the number of received packed red blood cell was higher (14 vs. 3) in comparison with high dose group. The number of ASV vials the patients received was 5.5and 21.06 in group 1 and 2, respectively (5.5±1.7 vs. 21.06±10.89; p < 0.01).The difference in frequency of coagulopathy complications, and need for using packed red blood cell were statistically significant(96.2% and 17.9% in group 1 vs. 34.2% and 3.9% in group 2, p < 0.01).It seems that cautious usage of high dose of ASV (10-20 vials) without very special concerns about the cost, dose, and without hazardous side effects is essential for the routine management of sever snake envenoming.

A simple, precise and accurate multiple analytical method has been developed for the simultaneous estimation of Chlorzoxazone and Nimesulide in bulk and tablet formulations by reversed-phase liquid chromatographic and UV-Visible spectrophotometric techniques. The chromatographic separation was achieved on C18 analytical column. A mixture of Methanol 0.1 Ortho-phosphoric acid 75 25 was used as mobile phase, at a flow rate of 1mL min and detection wavelength at 295 nm. The retention time of Chlorzoxazone and Nimesulide was found to be 4.69 and 5.45 min respectively. The linear dynamic ranges for HPLC were from 2-10 µg mL and for simultaneous equation method, derivative spectroscopy, Q-ratio Absorbance method, Dual wavelength it was 10-30 µg mL for both Chlorzoxazone and Nimesulide. The percentage recovery obtained for Chlorzoxazone and Nimesulide were 100.93 and 102.19 respectively for RP-HPLC, 9.7 and 100.1 for simultaneous equation method of CZ and NIM respectively, 99.97 and 99.78 for derivative spectroscopy of CZ and NIM respectively, 101.37 and 99.48 for Q-ratio Absorbance method of CZ and NIM respectively, 100.13 and 99.96 for dual wavelength method of CZ and NIM respectively. The validation of the proposed methods were carried out for linearity, accuracy, precision, limit of detection, limit of quantitation and robustness. Among all DoE techniques, response surface methodology (RSM) is the one most frequently used. Progress of computer science has had an impact on pharmaceutical development as well. Simultaneous with the implementation of statistical methods, machine learning tools took an important place in drug formulation. Twenty years ago, the first papers describing application of artificial neural networks in optimization of modified release products appeared. Since then, a lot of work has been done towards implementation of new techniques, especially Artificial Neural Networks (ANN) in modeling of production, drug release and drug stability of modified release solid dosage forms. The aim of this paper is to review artificial neural networks in evaluation and optimization of modified release solid dosage forms.

Fox in your mind right now. You probably are picturing the guy from Back to the Future or The Good Wife. Think about when you've seen him interviewed on TVnews programs or when he testified before congress. Those uncontrolled, involuntary movements Michael J. Fox is known for are not symptoms of Parkinson's. It's called dyskinesia. It's a reaction to long-term levodopa use. Yes, the gold standard drug treatment for Parkinson's, when taken over a long period of time, will effectively treat some of your PD symptoms like tremor, slowness and stiffness, but cause you to move uncontrollably. The Michael J. Fox Foundation describes it as fidgeting, writhing, wriggling, head bobbing or body swaying. It is important to note that not everyone with Parkinson's will develop this complication. It is more common with people who are diagnosed with Young Onset Parkinson's Disease (YOPD) or under the age of 50. There was no guarantee I would experience dyskinesia. I didn't particularly want it. And in the end, I didn't really have a vote.   After my family returned from World Parkinson Congress in Japan in mid-June, I was jet lagged and my med times and dosages were a bit out of whack. That's when dyskinesia took hold. It had only ever really happened once before. About six months ago. I do sometimes sway and jerk a bit here and there, but I am a relative stranger to experiencing a full-on, dyskinesia episode.   It was Saturday night, my family was at the dinner table and I started to sway and rock. Slowly at first and then gradually faster and more erratic. My head started to go to the right, my shoulder popped to the left, my neck jerked back and forth. And it repeated over and over again.  About 5 minutes into the dyskinesia, I asked my wife Rebecca to record a video.The dyskinesia lasted 45 minutes. I posted the video on Facebook, Twitter and Instagram accounts. For friends and family who've not witnessed many physical symptoms of my Parkinson's, this came as a shock. For people familiar with PD, my episode was rather mild.Two days later, I was at a manager's off-site meeting with 18 of my colleagues and dyskinesia set in again. It lasted an hour. And then it happened a day later during my exercise class. I was doing a bear crawl and it was as if the bear was drunk. I couldn't tell my feel what to do anymore. It lasted 30 minutes.      In this Extra Dosage episode, I turn to friends and fellow people with Parkinson's who've been dealing with dyskinesia longer than I have. Becca Miller, 45, is a psychologist at a community mental health center in New Haven, Connecticut. She was diagnosed with Parkinson's disease six years ago. She began to have dyskinesia two years ago.  Jim Smerdon, 45, has been living in Vancouver, B.C., with Parkinson's since 2007. His dyskinesia was so severe he opted for Deep Brain Stimulation (DBS) surgery in 2014 in search of relief. The objective of this present is to develop a simple, precise, accurate stability indicating method  for the estimation of deferiprone in formulation by using RP-HPLC. The separation was  achieved on Inertsil ODS C18, 250x 4.6mm, 5µm i.d. column using 60 volumes of Mixed  Phosphate  buffer(KH2PO4+K2HPO4) pH 3.0 and 40 volumes of  methanol as mobile phase and  at a flow rate of 1.0  mL/min. Detection was carried out using a PDA detector at 280nm. The  method was validated for accuracy, precision, specificity, linearity and sensitivity. The total  chromatographic analysis time per sample was about 5 min with deferiprone eluting at retention  time of about 4.980 min. The method was validated as per ICH guide lines. Stability studies  reported absence of impurities at the peak retention time. The drug was stable to different  conditions like acidic, alkali, thermal, oxidative and photolytic conditions. Validation studies  demonstrated that the proposed HPLC method is simple, specific, rapid, reliable and  reproducible. The standard curves were linear over the concentration range of 75-125µg/mL.  The LOD and LOQ values for deferiprone were 3.91 and 11.8 µg/mL, respectively. The  percentage recovery was found to be 97.3 to 98.0 and the %RSD for precision was found to be  0.5. The high recovery and low relative standard deviation confirm the suitability of the  proposed  method for the determination  of  deferiprone in  bulk  and  capsule  dosage  forms

Abstract Introduction:: A simple, rapid, accurate, precise, and economical UV spectrophotometric method for the simultaneous determination of metoprolol succinate (METO) and olmesartan medoxomil (OLME) in a combined tablet dosage form using the simultaneous equation method has been developed. Materials and Methods:: The method is based on the simultaneous equations for analysis of both the drugs using distilled water as a solvent. METO has absorbance maxima at 221 nm and OLME has absorbance maxima at 257 nm in distilled water. Results:: The linearity was obtained in the concentration range of 5–25 μg/ml and 4–20 μg/ml for METO and OLME, respectively. The concentrations of the drugs were determined by using the simultaneous equations method. The mean recovery was 100.90 ± 1.76 and 100.26 ± 0.71 for METO and OLME, respectively. Conclusion:: The method was found to be simple, accurate, and precise and was applicable for the simultaneous determination of METO and OLME in the pharmaceutical tablet dosage form. The results of analysis have been validated statistically and by recovery studies.

Abstract Background:: Selective laser trabeculoplasty (SLT) is a widely used treatment for open angle glaucoma, producing sustained reductions of intraocular pressure (IOP). The aim of this study was to evaluate the long-term relationship between SLT energy dosage and IOP reduction. Materials and Methods:: A retrospective review was performed for patients receiving primary SLT therapy, with inclusion of subjects treated with 360° of SLT. Energy settings were collected upon treatment and IOP was collected at baseline up to 36 months. Pearson's correlation coefficient was used to determine whether there was a significant correlation between SLT energy and IOP reduction at all time points. Kaplan-Meier analysis with log-rank test was performed to determine the differences in IOP reduction ≥20% from baseline among those treated with low ( 105 mJ) energy SLT. Results:: A total of 104 eyes (75 patients) were included. The mean total SLT energy was 93.73 mJ (standard deviation (SD) = 21.83 mJ, range: 34.4-122 mJ). A significant positive correlation (P ≤ 0.05) between the amount of energy delivered and IOP reduction was found at all time points. Log-rank test showed a significant difference in IOP reduction ≥20% from baseline between the three energy groups, with low energy patients experiencing failure at an earlier time (P = 0.05). Conclusions:: Within the range of total energy examined, there is a positive correlation between total energy used and amount of pressure reduction achieved at up to 3 years of follow-up. This may be useful in determining the optimal energy dosage for maximum effect for patients receiving SLT.

Isocratic separation was achieved on an Enable-C18G (250 mm × 4.6 mm i.d., particle size 5 μm) column at room temperature, the mobile phase consisted of acetonitrile:0.01M TBAHS (50:50, v/v) at a flow rate of 1.0 ml/min, the injection volume was 20 μl, and PDA detection was carried out at 215 nm. The drug was subjected to acid and alkali hydrolysis, oxidation, photolysis, and heat as stress conditions. The method was validated for specificity, linearity, precision, accuracy, robustness, and system suitability. The method was linear in the drug concentration range of 10–300 μg/ml with the correlation coefficient being 0.999. The RSD for repeatability and intermediate precision was well below 2%. The mean recoveries were between 100.52–101.68% for trospium chloride.

Abstract An experiment was conducted to evaluate the efficacy of porcine follicle stimulating hormone (pFSH) dosage based on body weight (BW) on ovarian responses of crossbred does. Thirty donor does were divided into 3 groups getting pFSH dosages of 3, 5, and 8 mg pFSH per kg BW, respectively, and were named as pFSH-3, pFSH-5 and pFSH-8, respectively. Estrus was synchronized by inserting a controlled internal drug release (CIDR) device and a single injection of prostaglandin F2α (PGF2α). The pFSH treatments were administered twice a day through 6 decreasing dosages (25, 25, 15, 15, 10, and 10% of total pFSH amount; decreasing daily). Ovarian responses were evaluated on Day 7 after CIDR removal. After CIDR removal, estrus was observed 3 times in a day and pFSH treatments were initiated at 2 days before the CIDR removal. All does in pFSH-5 and pFSH-8 showed estrus signs while half of the does in pFSH-3 showed estrus signs. No differences (p>0.05) were observed on the corpus luteum and total ovarian stimulation among the treatment groups, while total and transferable embryos were higher (p
