2Evaluation Of Alveolar Bone Grafting In Unilateral Cleft Lip And Palate Patients Using A Computer-aided Diagnosis System.

Abstract Purpose: This study aimed to evaluate the trabecular bone changes after alveolar bone grafting in unilateral cleft lip and palate (UCLP) patients using a computer-aided diagnosis (CAD) system. Materials and Methods: The occlusal radiographs taken from 50 UCLP patients were surveyed retrospectively. The images were categorized as: 50 images in group 0 (before bone grafting), 33 images in group 1 (one month after bone grafting), 24 images in group 2 (2-4 months after bone grafting), 15 images in group 3 (5-7 months after bone grafting), and 21 images in group 4 (8 or more months after bone grafting). Each image was grouped as either "non-cleft side" or "cleft side". The CAD system was used five times for each side to calculate the pixel area based on the mathematical morphology. Significant differences were found using a Wilcoxon signed ranks test or paired samples t test. Results: The pixel area showed a significant difference between the "non-cleft side" and "cleft side" in group 0 (404.27±103.72/117.73±92.25; p=0.00), group 1 (434.29±86.70/388.31±109.51; p=0.01), and group 4 (430.98±98.11/366.71±154.59; p=0.02). No significant differences were found in group 2 (423.57±98.12/383.47±135.88; p=0.06) or group 3 (433.02±116.07/384.16±146.55; p=0.19). Conclusion: Based on the design of this study, alveolar bone grafting was similar to normal bone within 2-7 months postoperatively.

To perform a qualitative and quantitative assessment of the current available literature by conducting a systematic review and meta-analysis of randomized controlled clinical trials (RCTs), prospective controlled clinical trials (pCCTs), and retrospective cohort studies in order to elicit scientifically rigorous evidence on the post-treatment effects of primary management protocols using pre-surgical NAM as part of the protocol for treatment of unilateral cleft lip infants with or without cleft palate. More specifically, therapeutic effects on craniofacial development, dentoalveolar outcome, and nasolabial esthetics over a long-term follow-up of more than 18 months will be investigated.

The field of presurgical infant orthopedics for cleft lip and palate (CLP) is advancing through implementing computer-aided design computer-aided manufacturing (CAD/CAM) technology. Many studies have demonstrated that CAD/CAM nasoalveolar molding (NAM) achieves successful clinical outcomes and has potential to reduce the burden of care of conventional NAM. Centers worldwide have reported different protocols for conducting CAD/CAM NAM. These include different methods for digital alveolar molding, as well as design and fabrication of NAM plates. To gain a comprehensive outlook on CAD/CAM NAM, this scoping review aims to describe in detail the various CAD/CAM methods undertaken to conduct NAM in infants with unilateral CLP (UCLP). The information synthesized in this review could provide guidance to help centers integrate or improve their use of CAD/CAM technology to conduct NAM for their patients.

This Systematic Review is the final work of the Master in Dental Sciences at the University of Valencia (Spain). We would like to be able to publish the Systematic Review and for this reason we want to have its record. The present systematic review attempts to answer the question: Is it effective to receive presurgical orthopedic treatment in babies with cleft lip and palate? I intend to answer the following objectives 1.Determine the effectiveness of presurgical orthopedics in babies with cleft lip and palate. 2.Compare between the types of presurgical orthopedic treatments to determine which of them would be the most effective. 3. This article is from Journal of Anaesthesiology, Clinical Pharmacology , volume 30 . Abstract Background and Aims:: Children with facial clefts are usually difficult to intubate and it is considered safer to keep them spontaneously breathing while securing the airway. This prospective comparative study was conducted to evaluate endotracheal intubating conditions in pediatric patients undergoing cleft surgeries, without the use of muscle relaxants following induction with propofol and sevoflurane. Materials and Methods:: Sixty patients aged 1month to 3 years, were randomly allocated into two equal groups. Anesthesia was induced with sevoflurane 8% in oxygen in group 1 and with propofol 3 mg/kg in group 2. Laryngoscopy and intubation were attempted 150 s after induction in both groups and ease of laryngoscopy, position of vocal cords, degree of coughing, jaw relaxation, and limb movements were assessed and scored. Total score of 5 was considered excellent, 6-10 good, 11-15 poor, and 16-20 bad. Total score ≤ 10 was considered clinically acceptable, and >10 as clinically unacceptable. Chi-square and Wilcoxon Mann-Whitney tests were used to analyze data. Results:: There was no significant difference between groups when ease of laryngoscopy was compared. Sevoflurane induced patients had significantly better position of vocal cords at intubation and the propofol group had significantly more episodes of coughing. Significantly less number of patients had limb movements in sevoflurane group. There was no significant difference in degree of jaw relaxation between groups. The sevoflurane group had significantly better total scores and clinically acceptable intubating conditions. Conclusion:: Sevoflurane 8% in oxygen provides clinically acceptable intubating conditions without use of muscle relaxants in pediatric cleft patients.

Abstract Cleft lip and cleft palate are the most common craniofacial anomalies that have an incidence of 0.28 to 3.74 per 1000 live births globally. Due to the great advancements in the field of medical science, these anomalies can today be corrected. However, it cannot be ignored that the parents of these patients may face psychological stress due to the cleft defects in the baby. Also, these conditions may cause financial difficulties to the parents and cause anxiety to the mother about the proper feeding of their infant. Feeding problems can range from excessive air intake to failure to thrive. As the management of such cases is lengthy and includes a multi-disciplinary team approach, it is the role of the Pediatrician/Pedodontist to educate the mother about the proper feeding techniques. In this article, we have reviewed and highlighted the various traditional and advanced devices and techniques which help in the successful management of these individuals.

—EEC syndrome is a very rare syndrome having ectrodactyly, ectodermal dysplasia and cleft lip/ palate as cardinal signs with other variable associated features. Very few cases being reported. It is mostly inherited in an autosomal dominant manner. Such cases need coordinated multidisciplinary approach for treatment. A case of incomplete EEC syndrome having ectrodactyly and cleft palate with absence of the signs of ectodermal dysplasia with no systemic anomalies was reported. Other features noted in this case were syndactyly of fingers and toes with bilateral entropion.

14Easy Method Of Centralized Fixation Of Endotracheal Tube In Cleft Lip And Palate Surgery.

Abstract As we all know that fixation of endotracheal tube is very important aspect in cleft palate and maxillofacial surgery. During cleft palate and oral surgery various methods of fixation and modified tubes are deviced to make surgery safer and ergonomically better. Our method consist of 3 point fixation of tube (RAE) with dynaplast, which is freely available, cheap and good Adhesive quality. Dynaplast divided into 3 phalanges (one central and two lateral) and one portion undivided as central limb. This undivided central limb is fixed in centre of chin and other 3 phalanges wrap around tube on either side. This fixation totally takes away any lateral movements of tube. This method can be used with any tube (RAE/ Oxford/Flexometallic). Our method is described for its simplicity, ease and convinence and result which impart universally similar results with all different members of our anesthetist team.

17Interdisciplinary Treatment Of An Adult With A Unilateral Cleft Lip And Palate.

This article is from Journal of Orthodontic Science , volume 3 . Abstract The management of cleft lip and palate (CLP) requires an interdisciplinary team providing comprehensive care. The present report presents an interdisciplinary approach for the care of a cleft patient. A 17-year-old male patient presented with a a chief complaint of “unpleasant appearance of my teeth” and a history of surgical repair of unilateral CLP on the left side. He presented with Class III molar relationships, Class II canine relationships, crossbite related to maxillary right first premolar and lateral incisor, severe maxillary and mandibular crowding, maxillary anterior tooth size deficiency, congenitally missing upper left lateral incisor. Patient was treated with a pre-adjusted edgewise appliance in conjunction with extraction of multiple teeth and distalization of the lower right first molar using a temporary anchorage device. In addition, alveolar bone graft and implant were placed to restore the missing upper left lateral incisor and a final esthetic work was performed for anterior teeth. The case was finished with Class I molar and canine relationships, minimal overjet and overbite. Total treatment time was about 31 months with satisfactory results. Post-treatment evaluation after 8 months showed stable results.

18Naïve Listener Judgements Of Speech Sound Change During Speech Intervention For Children With Cleft Lip And Palate Using A Visual Analogue Scale, Compared To Expert Ratings

Children with cleft palate+/-cleft lip (CP+/-L) can present with speech difficulties that have been shown to benefit from both articulatory and phonological therapy approaches. Instrumental methods, such as ultrasound or electropalatography, have been helpful in identifying the types of articulatory errors these children produce but they are not readily available in most clinical settings. The visual analogue scale (VAS) has the potential to offer a low-cost alternative in detecting subtle changes in children’s speech but it has not been tested for children with children with cleft palate+/-cleft lip (CP+/-L), undergoing speech therapy. This study will present an observational analysis investigating how a large number of naïve listeners evaluate speech sound changes occurring in speech therapy in two children with unilateral Cleft Lip and Palate (UCLP) during consecutive speech therapy sessions. The listeners will use a VAS and their judgements will be compared to traditional evaluation using Percentage target Consonants Correct (PCC) judged by a cleft specialist Speech and Language Therapist (SLT). PCC intra-rater reliability will be checked for all items with at least one other cleft specialist SLT. In addition, it will be explored if a narrow phonetic transcription can also show any gradient changes in the children’s speech during therapy, relative to VAS and PCC.

21Occurrence Of Consonant Production Errors In Liquid Phonemes In Children With Operated Cleft Lip And Palate.

Abstract Information about the prevalence of consonant production errors, including compensatory articulations (CA), in individuals with cleft lip and palate (CLP) who speak Brazilian Portuguese is limited, particularly regarding liquid sounds. The literature primarily reports the occurrence of CA for plosive and fricative sounds, since occurrence of CAs in sounds that require higher amounts of oral air pressure is expected. While the use of CA during liquid sound production is not expected, clinical experience suggests that individuals with CLP present with inadequate backing, elevation, and anteriorization of the tongue as well as tongue clicks during production of /r/ and /l/.Objectives: Describe the occurrence of consonant error productions during liquid sounds for children with CLP; compare the occurrence between children operated with the Furlow and von Langenbeck techniques for palatoplasty; and compare the occurrence between children operated between 9-12 months and 15-18 months of age at primary palatoplasty. Material and Methods: A sample of 397 children (237 males and 160 females) with operated unilateral CLP was studied. In this group, 163 underwent palatoplasty with the Furlow procedure and 234 with the modified von Langenbeck procedure. Age at palatoplasty was between 9 and 12 months for 189 children and between 15 and 18 months for 208 children. Data about production of /l/, /r/, /R/, /λ/ and consonant clusters /l/ and /r/ were obtained from speech pathology records. Speech pathologists registered the speech findings after an auditory-perceptual evaluation of the participants at the sixth year of age. Results: The use of middorsum palatal place (MDP) of production was identified for 2% of the sample. Tongue anteriorization of the /l/ production was observed for 55% of the children. No significant difference was found related to surgical technique, but children operated earlier developed the use of the consonant cluster /r/ sooner than children operated later (p=0.040). Conclusion: We found a low occurrence of use of cleft related CA during attempts of production of liquid phonemes, and the variable age at primary palatoplasty significantly interfered with the acquisition of consonant cluster /r/.

22Association Between Polymorphism Of TGFA Taq I And Cleft Lip And/or Palate: A Meta-analysis.

This article is from BMC Oral Health , volume 14 . Abstract Background: Cleft lip and palate (CL/P) is one of the most common malformations in humans. Transforming growth factor alpha (TGFA) is a well characterized mammalian growth factor which might contribute to the development of CL/P. This meta-analysis aimed to summarize the association between the TGFA Taq I polymorphisms and CL/P. Methods: We retrieved the relevant articles from PubMed, EMBASE, ISI Web of Science and SCOPUS databases. Studies were selected using specific inclusion and exclusion criteria. The odds ratios (ORs) and their 95% confidence intervals (95% CIs) were calculated to assess the association between TGFA Taq I polymorphism and CL/P risk. Meta-analyses were performed on the total data set and separately for the major ethnic groups, disease type and source of control. All analyses were performed using the Stata software. Results: Twenty articles were included in the present analysis. There is a significant association between the TGFA Taq I polymorphism and CL/P (C1C2 vs C1C1: OR = 1.67, 95% CI = 1.23-2.25, C2C2 + C1C2 vs C1C1C1: OR = 1.52, 95% CI = 1.15-2.01; C2 vs C1:OR = 1.41, 95% CI = 1.12-1.78). Stratified analyses suggested that the TGFA Taq I polymorphism was significantly associated with CL/P in Caucasians (C1C2 vs C1C1: OR = 1.95, 95% CI = 1.34-2.86; C2C2 + C1C2 vs C1C1: OR = 1.68, 95% CI = 1.18-2.38; C2 vs V1: OR = 1.52, 95% CI = 1.14 -2.02). Conclusion: TGFA Taq I polymorphism may be associated with the risk of CL/P.

This article is from Bioinformatics , volume 28 . Abstract Motivation: For the analysis of rare variants in sequence data, numerous approaches have been suggested. Fixed and flexible threshold approaches collapse the rare variant information of a genomic region into a test statistic with reduced dimensionality. Alternatively, the rare variant information can be combined in statistical frameworks that are based on suitable regression models, machine learning, etc. Although the existing approaches provide powerful tests that can incorporate information on allele frequencies and prior biological knowledge, differences in the spatial clustering of rare variants between cases and controls cannot be incorporated. Based on the assumption that deleterious variants and protective variants cluster or occur in different parts of the genomic region of interest, we propose a testing strategy for rare variants that builds on spatial cluster methodology and that guides the identification of the biological relevant segments of the region. Our approach does not require any assumption about the directions of the genetic effects.Results: In simulation studies, we assess the power of the clustering approach and compare it with existing methodology. Our simulation results suggest that the clustering approach for rare variants is well powered, even in situations that are ideal for standard methods. The efficiency of our spatial clustering approach is not affected by the presence of rare variants that have opposite effect size directions. An application to a sequencing study for non-syndromic cleft lip with or without cleft palate (NSCL/P) demonstrates its practical relevance. The proposed testing strategy is applied to a genomic region on chromosome 15q13.3 that was implicated in NSCL/P etiology in a previous genome-wide association study, and its results are compared with standard approaches.Availability: Source code and documentation for the implementation in R will be provided online. Cleft lip and/or palate is one of the most common congenital craniofacial malformations. In the early months of life, alveolar and nasal reconstruction presents a major surgical challenge. Neonatal treatment aims to close the lip and/or palate, often preceded by presurgical maxillary orthopedics (PSO) to realign maxillary segments and exclude the tongue from the cleft. Since McNeil introduced PSO in the 1950s, various modifications and devices have been proposed, yielding controversial results.The effectiveness of nasoalveolar molding remains debated due to inconsistencies in surgical timing, subjective outcome assessments, and the need for long-term follow-up on speech, hearing, and language development. Given these controversies and the lack of standardized evaluation criteria, a scoping review is warranted to synthesize existing evidence, compare different approaches, and clarify the current understanding of nasoalveolar molding in cleft treatment.

35Revolutionizing Cleft Lip And Palate Management Through Artificial Intelligence: A Scoping Review

Purpose: Not much is known about the applications of artificial intelligence (AI) in cleft lip and/or palate. We aim to perform a scoping review to synthesize the literature in the last 10 years on integrating AI in the approach to this condition and highlight aspects of research into its prediction, diagnosis, and treatment. Methods: A search was performed via PubMed, Science Direct, Scopus, and LILACS from 2014-2024, in which 649 articles were identified, and 3 studies were identified via the snowball method; the title and abstract were identified, and 35 articles were obtained for full reading. Finally, 25 studies were selected after applying the inclusion and exclusion criteria to execute this review. Results: The articles reviewed included different types of studies, with observational and experimental studies being frequent and systematic reviews and narratives being less frequent. Similarly, there was evidence of a generalized distribution, with a greater concentration in the United States. These studies were analyzed according to the use of AI applied to cleft lip/palate, obtaining 6 subcategories, including diagnosis, prediction, treatment, and education, in which different types of AI models were included, most frequently using deep learning and machine learning. Conclusion: These technologies promise to optimize the care of patients with this condition. Although current advances are promising, further research is essential to expand and refine their beneficial use. AI has driven significant advances in various stages of the cleft lip and/or palate approach, integrating tools such as assisted algorithms, genetics-based predictive models, and advanced surgical planning.

The field of presurgical infant orthopedics for cleft lip and palate (CLP) is advancing through implementing computer-aided design computer-aided manufacturing (CAD/CAM) technology. Many studies have demonstrated that CAD/CAM nasoalveolar molding (NAM) achieves successful clinical outcomes and has potential to reduce the burden of care of conventional NAM. Centers worldwide have reported different protocols for conducting CAD/CAM NAM. These include different methods for digital alveolar molding, as well as design and fabrication of NAM plates. To gain a comprehensive outlook on CAD/CAM NAM, this scoping review aims to describe in detail the various CAD/CAM methods undertaken to conduct NAM in infants with unilateral CLP (UCLP). The information synthesized in this review could provide guidance to help centers integrate or improve their use of CAD/CAM technology to conduct NAM for their patients.

43A Retrospective Study Of Cleft Lip And Palate Patients' Satisfaction After Maxillary Distraction Or Traditional Advancement Of The Maxilla.

This article is from Journal of Oral & Maxillofacial Research , volume 3 . Abstract Objectives: To compare cleft lip and palate patients' satisfaction with aesthetics and functional parameters after conventional advancement of the maxilla or by the use of distraction osteogenesis. Material and methods: Case series observational study. Group of distraction osteogenesis (DO) consisted of 15 patients treated with distraction osteogenesis while group conventional (CONV) included 10 patients treated with traditional advancement of the maxilla. Patients were asked to fill out a questionnaire about their subjective evaluation of satisfaction with facial aesthetics and functional parameters on a continuous visual analog-scale (VAS) when the treatment was finished. Results: The total response rate was 76%. Preoperatively the two groups did not differ significantly according to group characteristics. At follow-up both groups were satisfied with aesthetics and functional parameters. The DO group was less satisfied with the duration of the treatment than the CONV group. There were no statistically significant differences among the groups regarding functional parameters or facial aesthetics. Conclusions: Cleft lip and palate patients experienced a high level of satisfaction with functional parameters and aesthetics as a result of surgical maxillary advancement. The patients treated with distraction osteogenesis were less satisfied with the duration of the treatment. Further studies are needed.

Abstract The Van der Woude syndrome is a rare autosomal dominant developmental malformation usually associated with lower lip pits, cleft lip and palate. These congenital lip pits appear, clinically, as a malformation in the vermilion border of the lip, with or without excretion. Obligate carriers of this dominant mutation may have lip pits alone, cleft(s) alone, clefts and pits or neither. Here, we present a case report of a 10 year-old male patient with Van der Woude syndrome and a brief literature on its treatment modalities.

Abstract Feeding a neonate with a complete cleft lip and palate is difficult pursuit due to communication between oral cavity and nasal cavity. A multidisciplinary approach is required to manage the complex problems involved in case of such neonates and their families. Present case is of a 1-day-old neonate having complete bilateral cleft lip and palate for which palatal obturator was constructed. A stepwise simple, easy and uncomplicated procedure for making accurate impressions, maxillary cast and fabrication of palatal obturator in infants with cleft lip and palate has been presented.
