3Unraveling Reward Processing In Schizophrenia And Bipolar Disorder: A Multi-level Examination Of The Positive Valence System

Introduction: The Positive Valence Systems (PVS) domain, a key focus of the Research Domains Criteria framework, divides reward-related processes into three constructs: reward responsiveness, reward learning and reward valuation. Difficulties with several of these reward constructs have been reported in patients with mood-psychosis spectrum disorders. This study aims to examine how performance on tasks corresponding to these three constructs covaries, and how performance relates to mood and psychotic symptoms in two samples of adults with mood-psychosis-spectrum-disorders, those at familial risk, and controls. Methods and proposed analyses: Data from two studies (N=278 and N=332) will be analyzed, which both include patients with a psychotic disorder or bipolar disorder, their first-degree relatives, and controls. PVS constructs are measured using the Multi-Armed Bandit Task, Effort-Expenditure for Rewards Task, and Monetary Incentive Delay Task. Depression, mania, and psychosis symptoms are measured with self-report and interview instruments. Confirmatory factor analysis will be used to examine covariation between PVS constructs and expanded to path analysis to test associations with symptoms.

4Longitudinal Dynamics Between Alcohol Use, Mood, And Functioning In Bipolar Disorder

Alcohol use disorders and problematic drinking are overrepresented in individuals with bipolar disorder (BD) compared with the general population. Co-occurring alcohol use disorder and bipolar disorder (BD+AUD) is associated with a host of negative outcomes including less time to remission, greater risk of rapid cycling, number of hospitalizations, and treatment nonadherence. The extant literature has largely concentrated on disentangling whether there are differences in illness trajectory amongst those who had BD onset first followed by AUD or vice versa. However, few studies to our knowledge have examined the temporal associations between dimensional measures of alcohol use, mood symptoms, and their cumulative impact on functioning across the course of bipolar disorder. Identifying temporal associations between alcohol use, mood, and functioning is essential for improved care of individuals with BD+AUD. For example, does a within-person change in alcohol use tend to precipitate a change in depressive or manic symptoms? Does a within-person change in depressive or manic symptoms tend precipitate a change in alcohol use (e.g., self-medication model)? Or, for some people is both true indicating a vicious cycle? Leverage the Prechter Longitudinal Study of Bipolar Disorder, we aim to answer these questions: 1) What are the overall temporal associations between alcohol use and mood in individuals across the bipolar spectrum, 2) Can subgroups of individuals can be identifyied by their temporal associations of alcohol use, and 3) what is the effect of these temporal associations on life functioning over time? Individuals in the Prechter Longitudinal Study completed dimensional measures of alcohol use (AUDIT) and social and occupational functioning (LFQ) as well as clinician rated symptom scales (YMRS, HDRS) every 6 months over the course of participation.

5Neurobiological Correlates Of Cognition In Bipolar Disorder And ADHD – Enhancing Diagnostic Accuracy In Women

Bipolar disorder (BD) and attention-deficit/hyperactivity disorder (ADHD) are prevalent neuropsychiatric conditions with overlapping symptoms, leading to frequent misdiagnoses. These contribute to delayed or insufficient treatment, affecting daily and occupational functioning. Women with BD are more often misdiagnosed, and females with ADHD receive diagnosis and treatment years later than males. This study aims to improve diagnostic accuracy for women by identifying shared and disorder-specific neurocognitive profiles and associated neurobiological markers in female individuals with BD and ADHD. The first aim is to identify differences in neurocognition between men and women across the BD-ADHD spectrum, focusing on executive function, working memory, attention, and processing speed. The second aim investigates neurobiological markers, such as cerebrospinal fluid biomarkers and MRI-derived brain metrics, specific to women with these conditions. By associating cognitive categories with neurobiological markers, the study aims to identify sex-specific profiles that enhance diagnostic precision. This research fills a critical gap, potentially improving tailored treatments for women, who have historically been underrepresented in clinical trials. Findings may also inform the development of AI-driven diagnostic tools for BD and ADHD.

10Bipolar Disorder And Risky Decision-making

Individuals with bipolar disorder (BD) are usually observed making inappropriate risky decisions. However, whether and to what extent BD is associated with impairments in risky decision-making remains unclear. This project consists of 2 planned studies to examine this question. Study 1 will adopt diverse measures (including self-reported scales, behavioral tasks, and daily risky behaviors) in multiple domains to evaluate risky decision-making for BD patients and compare their performances to healthy controls. This study mainly explores whether BD influences risky decision-making and whether this impact remains across different phases and domains. Study 2 will regularly evaluate BD patients' risky decision-making using the same measures in Study 1 in order to explore the longitudinal impacts of BD on risky decision-making.

17Assessment Of Different Domain Impairments In Cognitive Functions And Functionalities Found In Euthymic Patients With Bipolar Disorder I / II - During The Early And Late Phases Of The Disease, Using The FAB And FAST Tests.

Objective: In this study, we focus on assessing two key predictors and outcomes in Bipolar Disorder (BD), which are: cognition and functionality performance, dividing them into two subgroups, and then researching for a correlation between them. Methods; Fifty patients with BD in a euthymic phase were divided into two subgroups (≤ 3 years (n=25) and ≥ 10 years (n=25) of the disease) and were then compared with healthy controls (n=25). Psychosocial functioning was assessed using the Short Functionality Assessment Test (FAST), and the Frontal Assessment Battery (FAB) test to assess frontal cognitive functions. Clinical and sociodemographic characteristics were analyzed using unilateral variance analysis, or the chi-square test. 18Neurocognitive Profiles In Treatment-resistant Bipolar I And Bipolar II Disorder Depression.

This article is from BMC Psychiatry , volume 13 . Abstract Background: The literature on the neuropsychological profiles in Bipolar disorder (BD) depression is sparse. The aims of the study were to assess the neurocognitive profiles in treatment-resistant, acutely admitted BD depression inpatients, to compare the neurocognitive functioning in patients with BD I and II, and to identify the demographic and clinical illness characteristics associated with cognitive functioning. Methods: Acutely admitted BD I (n = 19) and BD II (n = 32) inpatients who fulfilled the DSM-IV-TR criteria for a major depressive episode were tested with the MATRICS Consensus Cognitive Battery (MCCB), the Wechsler Abbreviated Scale of Intelligence, the National Adult Reading Test, and a battery of clinical measures. Results: Neurocognitive impairments were evident in the BD I and BD II depression inpatients within all MCCB domains. The numerical scores on all MCCB-measures were lower in the BD I group than in the BD II group, with a significant difference on one of the measures, category fluency. 68.4% of the BD I patients had clinically significant impairment (>1.5 SD below normal mean) in two or more domains compared to 37.5% of the BD II patients (p = 0.045). A significant reduction in IQ from the premorbid to the current level was seen in BD I but not BD II patients. Higher age was associated with greater neurocognitive deficits compared to age-adjusted published norms. Conclusions: A high proportion of patients with therapy-resistant BD I or II depression exhibited global neurocognitive impairments with clinically significant severity. The cognitive impairments were more common in BD I compared to BD II patients, particularly processing speed. These findings suggest that clinicians should be aware of the severe neurocognitive dysfunction in treatment-resistant bipolar depression, particularly in BD I. 24Dandy-Walker Variant Associated With Bipolar Affective Disorder.

This article is from Journal of Pediatric Neurosciences , volume 4 . Abstract The Dandy-Walker malformation is a congenital brain malformation, typically involving the fourth ventricle and the cerebellum. To date, the Dandy-Walker syndrome has not been described in association with bipolar disorder type I mania, and therefore we briefly report the case of a Dandy-Walker variant associated with acute mania. A 10-year-old boy was brought by his mother to the outpatient clinic of the Department of Psychiatry of a tertiary care hospital, with symptoms of mania. The MRI brain of the patient showed a posterior fossa cystic lesion, a giant cisterna magna communicating with the fourth ventricle and mild hypoplasia of the cerebellar vermis, with the rest of the structures being normal and no signs of hydrocephalus. These findings showed that the patient had a Dandy-Walker variant. He responded partially to valproate and olanzepine, which controlled the acute manic symptoms in the ward.

This article is from Translational Psychiatry , volume 2 . Abstract The genetic basis for bipolar disorder (BPD) is complex with the involvement of multiple genes. As it is well established that cyclic adenosine monophosphate (cAMP) signaling regulates behavior, we tested variants in 29 genes that encode components of this signaling pathway for associations with BPD type I (BPD I) and BPD type II (BPD II). A total of 1172 individuals with BPD I, 516 individuals with BPD II and 1728 controls were analyzed. Single SNP (single-nucleotide polymorphism), haplotype and SNP × SNP interactions were examined for association with BPD. Several statistically significant single-SNP associations were observed between BPD I and variants in the PDE10A gene and between BPD II and variants in the DISC1 and GNAS genes. Haplotype analysis supported the conclusion that variation in these genes is associated with BPD. 28Executive Function In Patients With Bipolar Disorder

The aim of this study is to examine the executive functioning in patients with bipolar disorder. Past studies have shown that - on group level - bipolar patients show worse performance regarding executive functioning than healthy controls and that their performance is negatively impacted by the amount of mood episodes they experience. One test of executive functioning that seems to reliably discriminate between bipolar patients and healthy controls is the Trail Making Test. Two independent cohorts who underwent the Trail Making Test - one of them repeatedly - will be studied in order to investigate whether patients perform worse than healthy controls at baseline and over time as well as whether mood episodes negatively impact their performance.

33Increased M1/decreased M2 Signature And Signs Of Th1/Th2 Shift In Chronic Patients With Bipolar Disorder, But Not In Those With Schizophrenia.

This article is from Translational Psychiatry , volume 4 . Abstract We here present data on immune gene expression of chemokines, chemokine receptors, cytokines and regulatory T-cell (T-reg) markers in chronic patients suffering from either schizophrenia (SCZ, N=20) or bipolar disorder (BD=20) compared with healthy controls (HCs, N=20). We extracted RNA from peripheral blood mononuclear cells and performed real-time (RT)-PCR to measure mRNA levels of chemokines, chemokine receptors, cytokines and T-reg markers. All the analyses were Bonferroni-corrected. The classical monocyte activation (M1) markers il6, ccl3 were significantly increased in BD as compared with both HC and SCZ patients (P=0.03 and P=0.002; P=0.024 and P=0.021, respectively), whereas markers of alternative (M2) monocyte activation ccl1, ccl22 and il10 were coherently decreased (controls: P=0.01, P=0.001 and P=0.09; SCZ subjects: P=0.02, P=0.05 and P=0.011, respectively). Concerning T-cell markers, BD patients had compared with HC downregulated ccr5 (P=0.02) and upregulated il4 (P=0.04) and compared with both healthy and SCZ individuals downregulated ccl2 (P=0.006 and P=0.003) and tgfβ (P=0.004 and P=0.007, respectively). No significant associations were found between any immune gene expression and clinical variables (prior hospitalizations, Brief Psychiatric Rating Scale, medications' dosages and lifetime administration). Although some markers are expressed by different immune cell types, these findings suggest a coherent increased M1/decrease M2 signature in the peripheral blood of BD patients with potential Th1/Th2 shift. In contrast, all the explored immune marker levels were preserved in SCZ. Further larger studies are needed to investigate the relevance of inflammatory response in BD, trying to correlate it to psychopathology, treatment and outcome measures and, possibly, to brain connectivity.

This article is from The Application of Clinical Genetics , volume 7 . Abstract Bipolar disorder is a common, complex genetic disorder, but the mode of transmission remains to be discovered. Many researchers assume that common genomic variants carry some risk for manifesting the disease. The research community has celebrated the first genome-wide significant associations between common single nucleotide polymorphisms (SNPs) and bipolar disorder. Currently, attempts are under way to translate these findings into clinical practice, genetic counseling, and predictive testing. However, some experts remain cautious. After all, common variants explain only a very small percentage of the genetic risk, and functional consequences of the discovered SNPs are inconclusive. Furthermore, the associated SNPs are not disease specific, and the majority of individuals with a “risk” allele are healthy. On the other hand, population-based genome-wide studies in psychiatric disorders have rediscovered rare structural variants and mutations in genes, which were previously known to cause genetic syndromes and monogenic Mendelian disorders. In many Mendelian syndromes, psychiatric symptoms are prevalent. Although these conditions do not fit the classic description of any specific psychiatric disorder, they often show nonspecific psychiatric symptoms that cross diagnostic boundaries, including intellectual disability, behavioral abnormalities, mood disorders, anxiety disorders, attention deficit, impulse control deficit, and psychosis. Although testing for chromosomal disorders and monogenic Mendelian disorders is well established, testing for common variants is still controversial. The standard concept of genetic testing includes at least three broad criteria that need to be fulfilled before new genetic tests should be introduced: analytical validity, clinical validity, and clinical utility. These criteria are currently not fulfilled for common genomic variants in psychiatric disorders. Further work is clearly needed before genetic testing for common variants in psychiatric disorders should be established.

36Adjunctive Long-acting Risperidone In Patients With Bipolar Disorder Who Relapse Frequently And Have Active Mood Symptoms.

Abstract Background: The objective of this exploratory analysis was to characterize efficacy and onset of action of a 3-month treatment period with risperidone long-acting injection (RLAI), adjunctive to an individual's treatment regimen, in subjects with symptomatic bipolar disorder who relapsed frequently and had significant symptoms of mania and/or depression. Methods: Subjects with bipolar disorder with ≥4 mood episodes in the past 12 months entered the open-label stabilization phase preceding a placebo-controlled, double-blind study. Subjects with significant depressive or manic/mixed symptoms at baseline were analyzed. Significant depressive symptoms were defined as Montgomery-Åsberg Depression Rating Scale (MADRS) ≥16 and Young Mania Rating Scale (YMRS) < 16; manic/mixed symptoms were YMRS ≥16 with any MADRS score. Subjects received open-label RLAI (25-50 mg every 2 weeks) for 16 weeks, adjunctive to a subject's individualized treatment for bipolar disorder (mood stabilizers, antidepressants, and/or anxiolytics). Clinical status was evaluated with the Clinical Global Impressions of Bipolar Disorder-Severity (CGI-BP-S) scale and changes on the MADRS and YMRS scales. Within-group changes were evaluated using paired t tests; categorical differences were assessed using Fisher exact test. No adjustment was made for multiplicity. Results: 162 subjects who relapsed frequently met criteria for significant mood symptoms at open-label baseline; 59/162 (36.4%) had depressive symptoms, 103/162 (63.6%) had manic/mixed symptoms. Most subjects (89.5%) were receiving ≥1 medication for bipolar disorder before enrollment. Significant improvements were observed for the total population on the CGI-BP-S, MADRS, and YMRS scales (p < .001 vs. baseline, all variables). Eighty-two (53.3%) subjects achieved remission at the week 16 LOCF end point. The subpopulation with depressive symptoms at open-label baseline experienced significant improvement on the CGI-BP-S and MADRS scales (p < .001 vs. baseline, all variables). Subjects with manic/mixed symptoms at baseline had significant improvements on the CGI-BP-S and YMRS scales (p < .001 vs. baseline, all variables). No unexpected tolerability findings were observed. Conclusions: Exploratory analysis of changes in overall clinical status and depression/mania symptoms in subjects with symptomatic bipolar disorder who relapse frequently showed improvements in each of these areas after treatment with RLAI, adjunctive to a subject's individualized treatment. Prospective controlled studies are needed to confirm these findings.

Abstract Objective: Research of electroencephalograph (EEG) power spectrum and mean frequency has shown inconsistent results in patients with schizophrenic, schizoaffective and bipolar disorders during medication when compared to normal subjects thus; the characterization of these parameters is an important task. Methods: We applied quantitative EEG (qEEG) to investigate 38 control, 15 schizophrenic, 7 schizoaffective and 11 bipolar disorder subjects which remaine under the administration of psychotropic drugs (except control group). Absolute spectral power (ASP), mean frequency and hemispheric electrical asymmetry were measured by 19 derivation qEEG. Group mean values were compared with non parametrical Mann-Whitney test and spectral EEG maps with z-score method at p < 0.05. Results: Most frequent drug treatments for schizophrenic patients were neuroleptic+antiepileptic (40% of cases) or 2 neuroleptics (33.3%). Schizoaffective patients received neuroleptic+benzodiazepine (71.4%) and for bipolar disorder patients neuroleptic+antiepileptic (81.8%). Schizophrenic (at all derivations except for Fp1, Fp2, F8 and T6) and schizoaffective (only at C3) show higher values of ASP (+57.7% and +86.1% respectively) compared to control group. ASP of bipolar disorder patients did not show differences against control group. The mean frequency was higher at Fp1 (+14.2%) and Fp2 (+17.4%) in bipolar disorder patients than control group, but no differences were found in frequencies between schizophrenic or schizoaffective patients against the control group. Majority of spectral differences were found at the left hemisphere in schizophrenic and schizoaffective but not in bipolar disorder subjects. Conclusion: The present report contributes to characterize quantitatively the qEEG in drug treated schizophrenic, schizoaffective or bipolar disorder patients.

Abstract Disruption of functional connectivity may be a key feature of bipolar disorder (BD) which reflects disturbances of synchronization and oscillations within brain networks. We investigated whether the resting electroencephalogram (EEG) in patients with BD showed altered synchronization or network properties. Resting-state EEG was recorded in 57 BD type-I patients and 87 healthy control subjects. Functional connectivity between pairs of EEG channels was measured using synchronization likelihood (SL) for 5 frequency bands (δ, θ, α, β, and γ). Graph-theoretic analysis was applied to SL over the electrode array to assess network properties. BD patients showed a decrease of mean synchronization in the alpha band, and the decreases were greatest in fronto-central and centro-parietal connections. In addition, the clustering coefficient and global efficiency were decreased in BD patients, whereas the characteristic path length increased. We also found that the normalized characteristic path length and small-worldness were significantly correlated with depression scores in BD patients. These results suggest that BD patients show impaired neural synchronization at rest and a disruption of resting-state functional connectivity.

The main domains affected are verbal memory, attention, and executive functions. Deficits in these areas as well as difficulties to get functional remission seem to be increased associated with illness progression. Several studies have found a strong relationship between neurocognitive impairment and low functioning in bipolar disorder, as previously reported in other illnesses such as schizophrenia. Cognitive remediation strategies, adapted from work conducted with traumatic brain injury patients and applied to patients with schizophrenia, also need to be adapted to individuals with bipolar disorders. Early intervention using functional remediation, involves neurocognitive techniques and training, but also psychoeducation on cognition-related issues and problem-solving within an ecological framework.

Abstract Despite the impact of schizophrenia and mood disorders, which in extreme cases can lead to death, recent decades have brought little progress in the development of new treatments. Recent studies have shown that Reelin, an extracellular protein that is critical for neuronal development, is reduced in schizophrenia and bipolar disorder patients. However, data on a causal or protective role of Reelin in psychiatric diseases is scarce. In order to study the direct influence of Reelin's levels on behavior, we subjected two mouse lines, in which Reelin levels are either reduced (Reelin heterozygous mice) or increased (Reelin overexpressing mice), to a battery of behavioral tests: open-field, black–white box, novelty-suppressed-feeding, forced-swim-test, chronic corticosterone treatment followed by forced-swim-test, cocaine sensitization and pre-pulse inhibition (PPI) deficits induced by N-methyl--aspartate (NMDA) antagonists. These tests were designed to model some aspects of psychiatric disorders such as schizophrenia, mood, and anxiety disorders. We found no differences between Reeler heterozygous mice and their wild-type littermates. However, Reelin overexpression in the mouse forebrain reduced the time spent floating in the forced-swim-test in mice subjected to chronic corticosterone treatment, reduced behavioral sensitization to cocaine, and reduced PPI deficits induced by a NMDA antagonist. In addition, we demonstrate that while stress increased NMDA NR2B-mediated synaptic transmission, known to be implicated in depression, Reelin overexpression significantly reduced it. Together, these results point to the Reelin signaling pathway as a relevant drug target for the treatment of a range of psychiatric disorders.

Mania, a defining symptom of Bipolar Disorder I, poses significant challenges in timely detection and intervention as it is a dangerous condition for both the individual and others (e.g. engagement in risky behaviors, self-harm tendencies, endangerment of others). Therefore, this project seeks to synthesize current research on active and passive sensing technologies focusing on detecting and understanding manic states. It is part of the larger research project DAMOS (DAtabase for MObile Sensing Studies; associated OSF project osf.io/5ukt9).
