No Curvature, No Globe     ?

The world's oil reserve is a finite resource which is rapidly being depleted; inevitably we will run out. It is the United States Governments fundamental responsibility to ensure we are prepared to face an oil-constrained, and eventually, an oil-free world. Currently, the US imports 2/3 of its oil from other nations, many of which are unstable and openly hostile to us. This leaves the US vulnerable to the whims of these nations and their leaders. To eliminate this threat, the US needs to commit sufficient resources to the development of technologies which are sustainable and will allow us to reduce and eventually eliminate the need for petroleum.

The term frailty describes a state of reduced function and health wherein individuals become more vulnerable to the effects of acute stressors. Within the general population, frailty effects an estimated 5-13% of individuals age 30 years and over. Within a clinical setting, 20-50% of middle and older aged individuals are affected. Frail individuals experience diminished strength, endurance and physiological function. Such burdens predispose these individuals to fractures, falls, and physical limitations further decreasing quality of life. Ultimately, the all-encompassing effects of frailty increase risk of hospitalization, morbidity and mortality from otherwise minor and survivable external stressors. The value of integrating frailty in research and clinical practice has been noted over recent years. Frailty was initially introduced in geriatric and family medicine where it was shown to predict adverse outcomes. More recently, frailty has been incorporated into procedural specialties like cardiology, orthopedics and general surgery. Within these areas, frailty has been associated with post-operative mortality, increased rates of readmission, surgical complications, post-operative morbidity, and prolonged recovery time. Such research suggests that the pre-operative measure of frailty could provide information to help minimize peri- and post-operative risks. Although frailty has been incorporated into certain surgical specialties, its use in plastic surgery is in its infancy. However, with a shift to more outpatient procedures and an aging patient population, the need for frailty within plastic surgery is evident. Outpatient surgical procedures have experienced a rapid increase in the past 40 years. Such changes warrant an increased level of risk assessment to ensure an uneventful post-operative course. Furthermore, older adults (>65 years) account for 32.2% of all outpatient surgeries in the United States. As the prevalence of frailty increases with age, this change in population warrants the use of a specific pre-operative risk assessment. Currently, it is common for risk assessment and pre-operative consultations to be at the discretion of the surgeon. However, it would be preferable to have a standardized method of assessment, such as a frailty index, to guide clinical care. The application of frailty measures into plastic surgery could identify high-risk patients and, if necessary, guide the design of surgical and post-surgical plans. A frailty measure could be applied in all areas of plastic surgery including medically necessary, urgent, and elective procedures. For medically necessary procedures, such as a patient with melanoma, a frailty index could help predict the post-operative recovery prognosis. In more urgent cases, such as an elderly patient with a single digit amputation, a frailty index may help identify the most appropriate course of action. For example, information from such a measure could help a surgeon decide between replantation or revision amputation. In elective procedures, such as breast reduction surgery, the development and use of a validated frailty index could allow for enhanced risk stratification. For instance, if a patient was identified as high-risk, the surgeon may choose to adjust the care they provide. More specifically, arrangements could be made to admit the patient post-operatively, or an otherwise minimal surgery could be performed in the main operating room. If a patient is identified as very high risk, the decision may be made to delay surgery until the patient is optimized, or not offer surgery at all. Given the changing atmosphere of plastic surgery, the use of a frailty index could be beneficial to patients, surgeons, and researchers. The purpose of this article is to provide a systematic review of common measures of frailty cited within the plastic surgery literature. Considerations and directions for future research within the plastic surgery specialty are then suggested.

The Active Component (AC) of the Air Force is aggressively examining solutions to its fighter pilot shortage, a deficit currently estimated at approximately 500 pilots and growing. The consequences are significant shortfalls in test positions, initial training instructor pilots, and staff positions. While many solutions have been proposed, further information needs to be garnered surrounding how these alternatives may impact the Air National Guard (ANG). This research seeks to examine the first and second order effects of insufficient numbers of AC pilots on ANG fighter squadron aircrew management dynamics. In the fiscal and political climate of sustaining todays Air Force, substantial funding increases or fundamental changes to the ANG component are unlikely. It is then appropriate to ask, what happens to ANG fighter squadrons readiness when an increased number of AC fighter pilots may be joining the ANG or a decreased number of fighter pilots may be available to affiliate? The scenario-planning framework is used to examine the required elements that make up the fighter force enterprise, followed by comparative examples that illustrate logical outcomes. Nearly all the current intellectual content on the subject is oriented toward problem identification tied to studies of alternatives and effects on regular Air Force (RegAF) personnel. The aim of this research is to analyze the potential benefits or detriments of the current RegAF pilot shortage to ANG manning.
