This article is from International Journal of Mental Health Systems , volume 6 . Abstract Objective: To quantify schizophrenia trialling activity in African countries and to describe the main features of these trials. Methods: We searched the Cochrane Schizophrenia Group Register, which contains 16,000 citations to 13,000 studies relating only to people with schizophrenia or schizophrenia-like illness, to identify schizophrenia trials conducted in Africa without time limitation. Results: A total of 38 trials met the inclusion criteria and were included in our analysis. Of the 54 countries of Africa, only 8 produced at least one trial: South Africa produced the majority of trials (20 out of 38 trials, 53%), followed by Nigeria (7 out of 38 trials, 18%) and Egypt (4 out of 38 trials, 11%). The majority of studies investigated the efficacy of pharmacological interventions, were short in duration, and employed a double-blind design. The quality of reporting was generally poor. We found six trials comparing antipsychotics from the WHO Essential List of Medicine versus new generation antipsychotics. In terms of efficacy and acceptability, these studies failed to show any advantage of newer antipsychotics over first-generation agents. Conclusions: We observed an impressive mismatch between the number of individuals with schizophrenia living in African countries, estimated to be around 10 million, and the overall number of patients included in African trials, which is less than 2,000. These few trials were of low quality and appeared not to reflect the real needs of the population. We argue that the concept of pragmatism should be introduced into the design of randomized trials in African countries. Pragmatic trials should investigate whether treatments, given in real-world circumstances, really have clinically meaningful effects.

Currently, all large ocean-going, mechanically linked tug-barge units are being operated in the integral or ship like mode where the tug and barge remain together at all times. This method of operation does not take advantage of the inherent flexibility available to tug-barge units--the ability to separate the propulsive unit (tug) from the cargo unit (barge). This separability can be used to operate the tugs and barges in a drop-and-swap mode where the tug drops off a loaded barge at a port and then swaps it for one that has completed its cargo operations. This method substantially increases tug utilization since the tug is not required to await cargo operations. However, this increased tug utilization is achieved at the expense of the extra barge units which remain in port for cargo operations. The question is whether in drop-and-swap operation the increased tug utilization outweighs the capital costs of the additional barges. To help answer this question, a computer model is developed to compare the total capital and operating cost of providing transport capacity for simple port pair trades with tugs and barges operated in the integral and drop-and-swap mode. From the model's output it can be determined which mode is more economical for a given trade. The model uses exhaustive enumeration to select among all feasible alternatives the number of tugs and barges, the barge size and form, and the tug-barge system speed that provides the desired transport capacity for a given port pair trade at the lowest required freight rate. The required freight rate calculation takes in account the tug, barge storage tank, and terminal facility annual operating and annualized capital costs. Most of the capital and operating cost elements are obtained from the literature. However practically no data are available for two of the most important cost factors, barge capital cons.

This article investigates how formulaic sequences fi t into a constructionist approach to grammar, which is a major post- Chomskyan family of approaches to linguistic structure. The author considers whether, in this framework, formulaic sequences represent a phenomenon that is suffi ciently diff erent to warrant special status or whether they might best be studied in terms of the larger set of all constructions found in language. Based on data drawn from a large corpus of Wikipedia texts, it is argued that it is extremely diffi cult to form a distinct class of formulaic sequences without creating highly arbitrary boundaries. On the other hand, based on existing theoretical claims that formulaic sequences are the basis of fi rst language acquisition, a marker of profi ciency in a language, critical to the success of communicative acts and key to rapid language processing, it is argued that formulaic sequences as constructions are nevertheless signifi cant enough to be the focus of research, and a theoretical category meriting particular attention. These fi ndings have key repercussions both for research primarily interested in formulaic language and phraseology as well as for construction grammatical research.
