Cognitive reserve (CR) describes an individual’s ability to adapt cognitive processes in response to brain atrophy (Stern et al., 2020). CR has been reported to protect against cognitive impairment in multiple sclerosis (MS), at least in some cognitive domains (Santangelo, Altieri, Enzinger, et al., 2019; Santangelo, Altieri, Gallo, et al., 2019). However, MS is typically diagnosed between the ages of 20-40 years (Dobson & Giovannoni, 2019). People with MS (pwMS) commonly experience a range of symptoms and MS-related aspects (e.g., fatigue, depression, anxiety) which may impact engagement with these enriching environments, and thus CR-building as measured through traditional proxies (Boeschoten et al., 2017; Marrie et al., 2015; Oliva Ramirez et al., 2021). Our recent systematic review (Stein et al., 2023) revealed that approximately half of the studies investigating CR in MS employed measures which exclusively assessed enrichment prior to the MS-diagnosis (e.g., pre-MS education or leisure activities). Of the studies examining continuous enrichment accumulated throughout the lifespan (i.e., including post-diagnosis), only half accounted for the potential impact of common MS symptoms or MS-related aspects in their analyses. The aim of this cross-sectional survey-based study is to gain a more accurate understanding of CR-building in MS by (1) investigating both pre-MS/early life and continuous/lifespan enrichment in pwMS and controls, and (2) examining the role of common MS symptoms and MS-related aspects in CR-building.
