3A Case Study In The Identification Of Critical Factors Leading To Successful Implementation Of The Hospital Incident Command System

The Hospital Incident Command System (HICS) is widely used by the nation’s hospitals, yet there is a paucity of research and a lack of developed models to examine HICS implementation. A study of HICS implementation may benefit hospitals, provide insight for future revisions, and add to the body of knowledge about HICS. This case study examined the critical factors that lead to the successful implementation of HICS based upon Stanford Medicine’s Response to the Asiana plane crash of July 6, 2013. Four commonalities identified from the literature review formed a hypothesis for successful HICS implementation that was tested and supported. In addition to the lessons learned that supported the tested hypothesis, the documentation reviewed described highly competent individuals and cohesive teamwork. It was not possible to separate individual and team competence from the tested hypothesis. As a result of this study, six critical factors were identified from the supported hypothesis that form an HICS implementation model for future evaluation.

This study analyzed how Naval Hospital Pensacola (NHP) utilized the Hospital Emergency Incident Command System (HEICS) model during Hurricane Ivan. The author conducted a literature review using resources at the medical library of NHP and emergency management websites; interviewed the sector officers and Directors of the Office of Homeland Security for the military services; and analyzed emergency management policies and documents related to Hurricane Ivan. Based on this research, the author presented conclusions and recommendations on how HEICS can be used to improve hospital emergency management programs. Finally, the author developed an implementation plan, which outlines the responsibilities and procedures to be followed for HEICS implementation at all Navy medical treatment facilities

20A Wireless Local Area Network Command And Control System For Explosive Ordnance Disposal Incident Response

Navy Explosive Ordnance Disposal (EOD) teams currently use a hardwire intercom system for command and control when responding to EOD incidents. This system is archaic, awkward, and cumbersome. A modern information system will greatly improve safety and efficiency during EOD operations. This thesis presents a lightweight, ruggedized, field portable, wireless local area network (LAN) designed for use by U.S. Navy EOD teams during EOD incident responses. The information system provides a voice, video, and data link between the command post and the down range response team, thus offering significant improvements over current EOD command and control methods. The system components are commercial off the shelf technology (COTS) and are chosen based on specification analysis for performance, reliability, availability and cost. Calculations for traffic flow analysis, electromagnetic radiation (EMR) levels, and theoretical range are provided Based on the new capabilities provided by the system, revised response tactics for EOD teams are proposed that allow multiple render safe procedures to be conducted simultaneously by a singe team, thus providing a quantum leap in efficiency.

One basic expectation that citizens have of government is to restore order in the face of devastation. With each catastrophic incident, politicians and administrators step under the public microscope, as they attempt to bring order out of chaos. Failure to perform such a basic governmental function subjects officials and agencies to immense scrutiny with personal and organizational repercussions. In the quest for the answers to What can be done better next time and How do we prevent this from happening again elected officials and bureaucrats seek various policies. A recent example of this is the National Incident Management System (NIMS). NIMS is a fusion of Incident Command Systems (ICS) and broad governmental policy aimed at providing a systematic response to incidents. The fact that NIMS became policy was quite possibly a predictable event in an incremental series of events that began in the early 1970s. This thesis reveals how these predictable and incremental efforts have pushed our national frameworks into an increasing state of complexity with the potential of catastrophic failure. Further, this thesis recommends ways that will harvest success in the face of a catastrophic or disastrous incident without increasing complexity.

Homeland Security Presidential Directive 5 (28 February 2003) calls for a National Incident Management System (NIMS) to provide a consistent nationwide approach for federal, state, and local governments in preparedness for, response to, and recovery from domestic incidents. The Incident Command System (ICS) is nationally recognized as the platform of choice for unification of operations in emergency response. Although ICS is a well-known entity in traditional first responder circles such as fire and law enforcement, the first responders in an event involving bioterrorism would be public health providers along with hospital and community healthcare personnel (non-traditional first responders). The national public health system has the healthcare expertise related to biological agents, but it lacks the ability to take command and control of consequence management operations.

This article is from Scandinavian Journal of Trauma, Resuscitation and Emergency Medicine , volume 20 . Abstract Background: Hospitals are cornerstones for health care in a community and must continue to function in the face of a disaster. The Hospital Incident Command System (HICS) is a method by which the hospital operates when an emergency is declared. Hospitals are often ill equipped to evaluate the strengths and vulnerabilities of their own management systems before the occurrence of an actual disaster. The main objective of this study was to measure the decision making performance according to HICS job actions sheets using tabletop exercises. Methods: This observational study was conducted between May 1st 2008 and August 31st 2009. Twenty three Iranian hospitals were included. A tabletop exercise was developed for each hospital which in turn was based on the highest probable risk. The job action sheets of the HICS were used as measurements of performance. Each indicator was considered as 1, 2 or 3 in accordance with the HICS. Fair performance was determined as < 40%; intermediate as 41-70%; high as 71-100% of the maximum score of 192. Descriptive statistics, T-test, and Univariate Analysis of Variance were used. Results: None of the participating hospitals had a hospital disaster management plan. The performance according to HICS was intermediate for 83% (n = 19) of the participating hospitals. No hospital had a high level of performance. The performance level for the individual sections was intermediate or fair, except for the logistic and finance sections which demonstrated a higher level of performance. The public hospitals had overall higher performances than university hospitals (P = 0.04). Conclusions: The decision making performance in the Iranian hospitals, as measured during table top exercises and using the indicators proposed by HICS was intermediate to poor. In addition, this study demonstrates that the HICS job action sheets can be used as a template for measuring the hospital response. Simulations can be used to assess preparedness, but the correlation with outcome remains to be studied.

13. ABSTRACT (maximum 200 words) The Incident Command System (ICS) resulted from the need for a new approach to the problem of managing wildfires in the early 1970s. The events of September 11, 2001, led to issuing of Homeland Security Presidential Directive (HSPD) 5 requiring agencies to adopt ICS as their incident management system. However, in events of national significance since then, internal communications have not performed well, causing numerous response problems. In addition, public information systems have failed to meet the community's expectations and keep the public informed about the size, scope, and impact of the emergency. Three models of possible solutions for addressing the problem were assessed. Model 1 consists of expanding the Communications Unit within the Logistics Section. Model 2 expands and clearly defines the duties, roles and responsibilities of the Public Information Officer. Model 3 merges all communications functions into one section directly under the Incident Commander. Metrics were designed around the management characteristics of the ICS and were assessed utilizing a defined scale. The research found that the creation of the Communication Section would provide the most benefits towards improving communications. However, that model may be difficult to implement due to resistance to strategic change.

The availability and interoperability of communications at an incident scene have long been recognized as high-priority problems that need to be addressed to improve our nation's Homeland Security and preparedness. This thesis describes a proposed methodology to address these issues at the Incident Command level while enhancing situational awareness and information sharing. The thesis analyzes the results of a research project funded by the Department of Homeland Security at the University of Louisville's IT Research Center for Homeland Security. The problem being addressed is that the decision-maker with the boots on the ground, the Incident Commander, needs relevant information in the early stages of the emergency at the incident scene and an efficient way to communicate with other resources. The research project fielded a prototype solution based on readily available commercial off-the-shelf components integrated in a man-portable configuration to provide maximum flexibility, lower costs, and ease of operations. A proposed concept of operations in various prevention and response environments was also recommended in the thesis after analyzing the results of several field exercises and interviews with users.

One basic expectation that citizens have of government is to restore order in the face of devastation. With each catastrophic incident, politicians and administrators step under the public microscope, as they attempt to bring order out of chaos. Failure to perform such a basic governmental function subjects officials and agencies to immense scrutiny with personal and organizational repercussions. In the quest for the answers to What can be done better next time? and How do we prevent this from happening again? elected officials and bureaucrats seek various policies. A recent example of this is the National Incident Management System (NIMS). NIMS is a fusion of Incident Command Systems (ICS) and broad governmental policy aimed at providing a systematic response to incidents. The fact that NIMS became policy was quite possibly a predictable event in an incremental series of events that began in the early 1970s. This thesis reveals how these predictable and incremental efforts have pushed our national frameworks into an increasing state of complexity with the potential of catastrophic failure. Further, this thesis recommends ways that will harvest success in the face of a catastrophic or disastrous incident without increasing complexity.

This thesis argues that the current Incident Command System (ICS) is inadequate for activating the National Guard Military Support Detachment - Rapid Assessment Initial Detection (RAID) Teams, which are vital for responding to domestic terrorism. The current ICS does not allow first responders to contact National Guard units directly during a WMD incident. First responders must send a request via their Emergency Operation Center (EOC), through the State Emergency Management Division (EMD), to the state Governor's office for approval. The Governor can then activate the National Guard to respond to the incident. This process is unnecessarily time-consuming. Serious jurisdictional issues and turf wars may emerge between first responders and RAID teams, at precisely the time when close coordination is most necessary. RAID teams should be recognized as operational units, and given the ability to initiate their response plan upon receipt of an alert from the local EOC. With the approval of the RAID team's higher headquarters they may then deploy. Appendix A provides a guide to suggested changes in existing regulations. This thesis also proposes changes in education and training that should help alleviate other problems associated with rapid response, including the risk of jurisdictional conflicts and Good Samaritan casualties.

The goal of the effort is to enhance emergency management training through computer-supported simulation. The Army's research on simulation-based unit training and tactical decision making can benefit NFA. This report summarizes results of an initial effort to transfer Army experience to the Academy. The report describes NFA simulation methodology as a baseline from which future upgrades will be made and recommends ways to introduce computer-aiding to support management of simulation exercises and performance assessment. The recommendations address the near-term goal of introducing computer-supported simulations at the NFA campus and the long-range goal of distributing simulation nationwide. (KAR) P. 3

On 15 May 1975, U.S. military forces successfully recaptured the U.S. container ship Mayaguez and her crew from Cambodian Khmer Route forces. Considered a strategic success, the effort was hampered by numerous flaws at the operational level. Because of the strategic implications of hostage rescue missions, it is imperative that the application of operational art be done in the most thorough manner possible. This paper will examine some of the failures of the Mayaguez incident at the operational level with specific focus on the operational functions of command control, communications and intelligence. Lessons learned will be determined and checked for applicability today in the hope of preventing the shortfalls identified in Mayaguez from occurring in the future.

Despite the increasing attention given to the need for schools to be prepared to respond in a variety of crisis situations, there is a lack of information about how to coordinate with multiple agencies following a crisis. This article describes the U. S. Department of Homeland Security's (2004) National Incident Management System and its Incident Command System (ICS), which provides a common set of concepts, principles, terminology, and organizational processes to facilitate crisis response activities. The traditional school crisis team structure is compared to the ICS structure and the overlap and integration of the two are highlighted. Two case scenarios are presented to illustrate how the school crisis team may operate in compliance with the ICS in different crisis situations. (Contains 2 figures.)

This article is from Journal of Research in Medical Sciences : The Official Journal of Isfahan University of Medical Sciences , volume 16 . Abstract BACKGROUND:: Applying an effective management system in emergency incidents provides maximum efficiency with using minimum facilities and human resources. Hospital Emergency Incident Command System (HEICS) is one of the most reliable emergency incident command systems to make hospitals more efficient and to increase patient safety. This research was to study requirements, barriers, and strategies of HEICS in hospitals affiliated to Isfahan University of Medical Sciences (IUMS). METHODS:: This was a qualitative research carried out in Isfahan Province, Iran during 2008-09. The study population included senior hospital managers of IUMS and key informants in emergency incident management across Isfahan Province. Sampling method was in non-random purposeful form and snowball technique was used. The research instrument for data collection was semi-structured interview; collected data was analyzed by Colaizzi Technique. RESULTS:: Findings of study were categorized into three general categories including requirements (organizational and sub-organizational), barriers (internal and external) of HEICS establishment, and providing short, mid and long term strategies. These categories are explained in details in the main text. CONCLUSIONS:: Regarding the existing barriers in establishment of HEICS, it is recommended that responsible authorities in different levels of health care system prepare necessary conditions for implementing such system as soon as possible via encouraging and supporting systems. This paper may help health policy makers to get reasonable framework and have comprehensive view for establishing HEICS in hospitals. It is necessary to consider requirements and viewpoints of stakeholders before any health policy making or planning.

45Flexibility Of The Incident Command System To Respond To Domestic Terrorism

This thesis argues that the current Incident Command System (ICS) is inadequate for activating the National Guard Military Support Detachment - Rapid Assessment Initial Detection (RAID) Teams, which are vital for responding to domestic terrorism. The current ICS does not allow first responders to contact National Guard units directly during a WMD incident. First responders must send a request via their Emergency Operation Center (EOC), through the State Emergency Management Division (EMD), to the state Governor's office for approval. The Governor can then activate the National Guard to respond to the incident. This process is unnecessarily time-consuming. Serious jurisdictional issues and \"turf wars\" may emerge between first responders and RAID teams, at precisely the time when close coordination is most necessary. RAID teams should be recognized as operational units, and given the ability to initiate their response plan upon receipt of an alert from the local EOC. With the approval of the RAID team's higher headquarters they may then deploy. Appendix A provides a guide to suggested changes in existing regulations. This thesis also proposes changes in education and training that should help alleviate other problems associated with rapid response, including the risk of jurisdictional conflicts and \"Good Samaritan\" casualties.

The Incident Command System (ICS) was developed to address the failure of a unified response in a multi-agency response to wildfires. Since its implementation, emergency responders are better trained and have effectively adapted to the complexity of multi-agency response. Recent events such as 9-11, Hurricane Katrina, and the earthquake in Haiti demonstrate the increasing complexity of the nature and effects of today's disasters. The need to include military units in emergency response operations appears to be growing and may become the future standard. A new Civil-Military operations paradigm is defined but not yet fully developed into an efficient and effective multi-agency response team. This paper will explicate research that utilizes lessons learned and current theory to develop an improved ICS model that integrates the current civil ICS with the military Command and Control (C2) system. The Hospital Incident Command System (HICS) is widely used by the nation s hospitals, yet there is a paucity of research and a lack of developed models to examine HICS implementation. A study of HICS implementation may benefit hospitals, provide insight for future revisions, and add to the body of knowledge about HICS. This case study examined the critical factors that lead to the successful implementation of HICS based upon Stanford Medicine s Response to the Asiana plane crash of July 6, 2013. Four commonalities identified from the literature review formed a hypothesis for successful HICS implementation that was tested and supported. In addition to the lessons learned that supported the tested hypothesis, the documentation reviewed described highly competent individuals and cohesive teamwork. It was not possible to separate individual and team competence from the tested hypothesis. As a result of this study, six critical factors were identified from the supported hypothesis that form an HICS implementation model for future evaluation.

In 2018, the Colorado Department of Transportation was hit with a ransomware attack that resulted in the first-ever state emergency declaration for a cyber attack. Cyber attacks against the nation and its infrastructure are expected to increase, yet no extensive research exists on the United States' designated response framework for them. This thesis investigated the application of the Incident Command System (ICS) in significant cyber incidents and how the system may be improved for these events. A mixed method study consisting of case studies, senior leader interviews, and a quantitative survey was used to evaluate ICS specific to the framework’s eight core concepts. The research includes findings on variables that impact the effectiveness of response frameworks in cyber events. Recommendations are made to improve cyber response.
